Preparing for Cohort Review Cohort preparation begins when cases are first reported

Preparation continues with regular case management

Case managers complete the demographic information on cohort forms

The remaining information is added as case information becomes known:

• During patient interview

• Through home assessment

• Chart review

• Clinic visits

• Contact elicitation and investigation

• Laboratory reports
