What is a Care Plan (Chronic

Disease Management)? Care planning is an essential part of healthcare, but is

often misunderstood or regarded as a waste of time.

People with a chronic medical condition* may be able to

access Medicare benefits to cover allied health services to

help manage your condition.

Care planning provides a ‘road map’ of sorts, to guide all

who are involved with your care. The first step in care plan-

ning is to gain an accurate and comprehensive assessment

that includes consideration of your unique abilities, physical,

social and emotional needs, and cultural preferences. Re-

viewing your care plan is the best way to assess if it is work-

ing or needs to amended—this generally happens 3-6

monthly.

Eligibility

Anyone with a chronic or terminal condition can have a GP

Management plan.

* A chronic condition has been,

or is likely to be present for at

least six months

Other Requirements A person may be referred for

allied health services and Med-

icare rebates may apply if they

are being managed by their

‘usual’ doctor and have:

A GP management plan

(GPMP) also referred to as a ‘care plan’ & a team care

arrangement (TCA) - a plan developed by your doctor &

nurse with you and at least two other care providers in-

volved in your ongoing care. So when you next receive a

call from our Nurse you will be well versed on what this

represents. Your Health, your future!

The sooner a skin cancer is identified and treated, the better your chance of avoiding surgery or, in the case of a serious melanoma or other skin cancer, potential dis-figurement or even death. It is a good idea to talk to our Skin Doctors about your level of risk and for advice on early detection. It’s important to know your skin and

what is normal for you, so that you notice any changes.

Skin cancers rarely hurt and are more frequently seen than felt. Develop a regular habit of check-ing your skin for new spots and

changes to existing freckles or moles.

Your Visit

If you require care outside of our hours, patients of our

clinic can contact our locum service who will provide our

patients with their medical care. Call 132660 for details.

For ALL MEDICAL EMERGENCIES call 000 for an

ambulance.

Fees

Appointments are essential. A standard appointment is 10-15 minutes, but

if you think you may need longer please feel free to request a longer ap-

pointment when making your booking. We value your time and do our best

to keep any waiting to a minimum where we can. Please remember,

sometimes there are unexpected delays and our doctors are devoting much

needed time to another patient’s care, as they will dedicate to you. Please

always inform our reception staff if your condition is urgent or if you are in a

hurry as we may be able to have you seen by an alternative doctor.

After Hours

We are a private billing clinic, however, we do bulk bill patients with a current

Concession and Medicare Card, for all services which are covered under

Medicare. For patients without a valid Medicare card a cash fee will apply

payable at the time of consultation. Please speak with our receptionist for

information about these fees. Other allied health services may attract a fee.

Recalls & Reminders As part of our commitment to providing quality care to you and your family,

we may send you correspondence informing you of health assessments

which are appropriate for you. We will also send you letters and SMS to

remind you of routine tests that are due. If you do not wish to be part of our

recall and reminder system, please inform our reception staff.

Home Visits

Where possible, we ask that you attend the clinic to see your doctor as we

have all the necessary equipment and support needed to manage your

care. We do appreciate however that there are times when patients are not

able to attend the clinic. Please contact reception staff if you require a

home visit as not all doctors are able to review patients outside of the clinic.

Your Privacy

All patient information is private, and the confidentiality of patient information

must be maintained at all times. The rights of every patient are respected.

All information collected by this practice in providing a health service is

deemed to be private and confidential. This practice complies with the Fed-

eral and State privacy regulations including the Privacy Act 1998, the Privacy

Amendment (Private Sector) Act 2000 and Victorian Health Records Act

provide grounds for disciplinary action or dismissal. Every employee of this

practice is aware of the policy and has signed a privacy statement as part of

to be binding on employees even after their employment has terminated.

Compliments, Suggestions Complaints We are committed to providing high quality patient care and are always looking for

areas to improve. If you would like to tell us about something we have done right, if you have a suggestion to make, if you have any con-

cerns or you are unsatisfied with any aspect of your visit—please don't be shy. Contact our caring General Manager or complete a sugges-

tions/feedback form located in the waiting area. We would appreciate the opportunity to resolve any grievance you may have with the

clinic, however if you are not satisfied with our response, you may contact the Health Services Commissioner on 1800 136 066 or email
