Bringing Public Health and Primary Care Together:

The Practical Playbook National Meeting

May 22 – 24, 2016 | Hyatt Regency | Bethesda, MD About the National Meeting The Practical Playbook’s inaugural National Meeting will be a milestone event towards advancing robust collaborations that improve population health. It will be a highly interactive meeting, focused on providing practical knowledge and tools to facilitate collaboration between public health and primary care practitioners and organizations. By bringing together key stakeholders across sectors - representing professional associations, community organizations, government agencies and academic institutions - the National Meeting will help to catalyze a national movement, accelerate collaborations by fostering skill development; and connect like-minded individuals and organizations to facilitate the exchange of ideas to drive population health improvement. Participants Learning Objectives Meeting attendees will participate in stimulating discussions; peer presentations; and hands-on workshops to enhance their practical skills and knowledge of public health/primary care collaborations to improve population health. Participant learning objectives include:

v Big Ideas: Inspirational and aspirational ideas to inform collaboration projects v Practical Knowledge: “How to” content and actionable take-aways to enhance participants’ ability to plan,

implement and sustain successful collaborations v Technical Skills: How to use data, tools, technology and resources (including The Practical Playbook) to aid

collaboration projects v Strategic Understanding: Understanding of the bigger picture related to the paradigm shift in health care, including

the implications for collaborations to improve population health Content Themes Content in the National Meeting will be geared towards providing participants practical and actionable knowledge, tools and resources that demonstrate how to design, implement, manage and sustain successful collaborations. Content will primarily focus on the following themes:

v Advancing the Population Health Movement v Best Practices for Collaboration v Using Data, Tools, Technology and Resources v Metrics and Evaluations for Population Health Collaborations v Sustaining and Funding Collaborations

Meeting Format The National Meeting will feature opportunities for networking and sharing ideas with peers; an informative poster session; plenary and keynote sessions; a World Café session; and multiple practical workshops.

Overview of Meeting Sessions Plenary session and keynote addresses will set the stage for practical workshops. These informative and engaging sessions will feature national thought leaders and industry experts who will inspire and educate participants, sharing their insights and expertise on what it takes to build successful public health/primary care collaborations in the changing health care environment. Interactive break-out workshops will follow each plenary and will provide participants with opportunities to explore and discuss in more depth and greater specificity the content introduced in the plenaries. Workshops may include case study presentations, panel discussions and/or featured speakers who will present content and facilitate hands-on learning among participants. Plenary Sessions and Workshop Topics MONDAY, MAY 23:

• Keynote #1: The Paradigm Shift from Healthcare to Population Health (working title) o This address will set macro-context for the overall shift in the health sector and illuminate important

themes/trends that may advance or hinder the paradigm shift. Participants will understand the critical influences, opportunities and challenges to collaboration and become better informed on the big-picture issues.

• Opening Plenary: Best Practices for Collecting, Sharing and Using Data to Support Collaborations (working

title) o This panel discussion or featured speaker session will feature best practices for collecting and sharing data

across sectors, and how data can be used to plan and execute relevant and impactful collaborations.

• Break-Out Session Series 1: Workshops on Data (Workshop topics are suggestions and may be modified based on speaker suggestions and submissions)

o Data Collection: Examples of best practices for how to identify what data to collect, what resources are available to support data collection and what tools/systems/infrastructures can support effective data collection

o Data Sharing: Examples of best practices for overcoming challenges and taking advantage of opportunities in data sharing. Participants will learn methods, practices and systems for effective data sharing across public health, primary care and communities, within the context of HIPPA and other policies

o Using Data Effectively to Enhance Population Health: Examples of how different sectors can interpret and use data from partner sectors (e.g. how can primary care practitioners interpret and use public health data and vice versa). Participants will be able to cite examples of how different data sets from different sources can be interpreted and used to design and measure effective interventions

o Using Data to Build a Business Case for Collaboration: Content focused on how to use data to report impact and tell a compelling story of how the collaboration is addressing community health needs and improving public health. Content in this session would also focus on building an evidentiary base for collaboration: projection, documentation, measuring and reporting impact of collaborations on population health

• Luncheon & Plenary: Models of Collaboration (working title)

o This moderated panel discussion will feature examples of best practices and real-life experiences from BUILD Health grantees (or other exemplars) who can represent different types of population health collaborations (e.g. public health/health system; health system/non-health community partner; local /state partnerships.)

• Break-Out Session Series 2: Exploring Models of Collaboration (Workshop topics are suggestions and may be modified based on speaker suggestions and submissions)

o Partnering with Public Health: Participants will discuss and apply best practices for forging successful collaborations with public health departments/organizations, from both the primary care and non-health sector perspective, with a focus on identifying and overcoming challenges, opportunities in designing and managing collaborations

o Working with Non-Health Sector Partners: Best practices for collaborating with non-health sector and/or non-traditional partners in the community. Participant will be able to cite specific tools and strategies for identifying, engaging and collaborating with community partners

o Working with Primary Care Partners: Participants will discuss and apply best practices for forging successful collaborations with primary care systems and practitioners from both the public health and non-health sector perspectives, with a focus on identifying and overcoming challenges, opportunities in designing and managing collaborations

o Session TBD: Possible topics include: § Working with insurers § Engaging state and regional partners § How to use collaborations to address health disparity

• Break-out Session Series 3: Models and Methods of Collaboration (cont’d) (Workshop topics are suggestions and may be modified based on speaker suggestions and submissions)

o The Principles of Collaboration: Using case studies, demonstrate how core principles of integration accelerate that guide the development of successful collaborations.

o Developing and using Common Vision, Mission and Goals: Examples of best practices and methods for aligning partners from various sectors around the goals, vision and mission of collaboration. Feature a successful alignment and lessons learned

o Managing Collaborations: Best practices for managing multiple partners working collaboratively. Content may address effective leadership styles, internal and external communication, logistics, and operational issues related to managing a collaboration

o Tools, Methods and Resources to Support Collaboration: Overview/introduction to various tools, methods and resources available to support and enhance collaboration

TUESDAY, MAY 24:

• Plenary: Shoots and Ladders: The Opportunities and Challenges in Forging Successful Public Health and Primary Care Collaborations (working title)

o Content will address pathways to success (ladders) and vulnerabilities to set-backs and failures (chutes). Content will also focus on the challenges, opportunities and best practices for ensuring the sustainability of public health-primary care collaboration.

• World Café Facilitated by Ahmed Calvo, MD, MPH, Health Resources and Services Administration (Tentative)

o A moderated World Café session will allow participants to opportunity to engage in a variety of stimulating discussions with peers and thought leaders to share ideas, solve challenges, and advance the population health conversation

• Break-out Session Series 4: Sustainability and Funding (Workshop topics are suggestions and may be modified based on speaker suggestions and submissions)

o Funding the Future: Best practices and resources for sourcing sustainable funding for public health/primary care collaborations.

o Sustaining Population Health Outcomes: Discussion of best practices and methods for ensuring positive population health trends are maintained and improved upon.

o Taking Collaborations to Scale: Strategies and best practices for sustaining and growing collaborations to scale and overcoming barriers and challenges. Could include topic related to growing a grassroots healthcare movement, working locally, state-wide and nationally, etc.

o Session TBD

• Lunch & Closing Keynote: Creating a Long-Term Sustainable Vision for Public Health-Primary Care Collaboration (working title)

o This session will focus on the shared vision public health and primary care practitioners can create and promote together to increase momentum in the national paradigm shift in health care.

All suggestions and submissions will be reviewed by a programming committee and final selections made based on relevance and quality of the proposed content, speaker experience, and other criteria determined by the programming committee and production team.
