Dear Practical Nursing Applicant:

Thank you for your interest in oTECH's Practical Nursing Program. Attached you will find the nursing application, estimated costs, and additional forms necessary to complete the application process. In order to be considered as a prospective practical nursing student, you must provide the following. NOTE: BE ADVISED APPL YING FOR OUR PROGRAM DOES NOT GUARANTEE YOU A SEAT IN THE NURSING CLASS.

1. FIRST STEP REGISTER ONLINE AT OTEC.OSCEOLASCHOOLS.NET

2. BRING IN TWO PROOFS OF RESIDENCY DOCUMENTS, FOR A LIST OF DOCUMENTS PLEASE SEE ATTACHED

3. BRING IN COLLEGE DEGREE AND HIGH SCHOOL TRANSCRIPT (Standard H.S. Diploma) FOR FURTHER INFORMATION ON TRANSCRIPT SEE NUMBER 6.

4. PRE-ADMISSION TESTING-

8 TABE (Test of Adult Basic Education) test fee is $25.00. Required scores are 11.0 in Reading, Math, and Language. TABE is given as a walk-in, please bring your ID and $25 and be here 15 minutes prior to testing. See enclosure for days and times.

8 H ESI (Health Education System, Inc.) fee is $55.00, each retake is $50. An Essay will be given after thetest. Required scores are

administration. After the second test administration, a prospective student can retake the exam within another 14 days before the third administration. After the third test administration, a prospective student must wait 30 days before the fourth administration.

8 After the second administration please note you will be required to take your third and fourth attempt at our St. Cloud location.

5. APPLICATIONPrint clearly in black ink or type, mark N/A if a section does not apply to you. Use a separate sheet of 8-1/2 x 11if the space available is insufficient and answer all sections accurately and completely. If you are not selectedfor this class, you will be able to reapply and update your original application. Additional fees for reapplicationmay be incurred.

6. COLLEGE TRANSCRIPTS• It is your responsibility to ensure that oTECH receives official transcripts from the colleges or nursingschools you have attended. They have to be mailed from these educational institutions directly to oTECH'sHealth Science Department. When you request transcripts, provide your social security number, FloridaStudent ID number (if you graduated from a Florida Public School after July 1, 1988) and the name(s) you usedwhile attending that institution. Some institutions require a fee for this service. Contact the individualinstitutions to determine how to obtain a transcript.

HIGH SCHOOL TRANSCRIPTS REQUIRED 6 For out of the country high school diplomas, they will need to be translated and evaluated, we use

www.wes.org for translation and evaluation. 6 Out of state high school transcripts/diplomas will be reviewed and approved on an individual basis.

7. REFERENCES• References (3) can be hand carried into the school, but it must be in a sealed_envelo..Qe _and_ the_ envelopemust_ be_signed _by_ the _person com_gletin.9_ the _reference. Reference letter can also be mailed. References mustbe from a supervisor, co-workers, or from a teacher that you have known for at least one year. Please no familyor friends.

You must have a full drug screening done at Anylabtestnow! The results of the drug screen must be in prior tothe interview being scheduled. The fee is approximately $30 and all results must be negative. !Drugi�creenings from other locations will not be accepted.I It may take 3-5 days to obtain the results. You don'tneed a form to go there, just let the facility know that you are taking a drug screen for the oTECH nursingprogram.

PHYSICAL EXAM- a physical exam is required (form is attached).

10. TITERS- This is a blood test that assesses the presence of antibodies in the immune system that are fromprevious vaccinations. Please have this test done for VARICELLA, MEASLES, MUMPS, RUBELLA, ANDHEPT ATIS B. This can be done wherever offered, ANYLABTESTNO_wt QT at your doctor's office.

11. BACKGROUND CHECKS- MUST BE DONE PRIOR TO ACCEPTANCE-Any student who has been arrested, convicted or found guilty of a crime regardless ofadjudication should consult with the Director of Nursing. There is a possibility the offense mayprevent admission into the program. (Form is attached in this packet).
