Post Liver Transplant Care Anna Crosswaite & Jackie Jennings, Clinical Nurse Specialists Issues for Post Liver Transplant Patients Increased risk of cardiovascular disease Increased risk of skin cancers & sun damage Adherence to taking medications Increased risk of obesity Increased rates of diabetes Because of these issues facing post transplant patients, it’s important they have a good understanding of these risks to be able to continue to live a healthy, happy life. At three months after their transplant patients are reviewed by the post transplant nurse specialist who will discuss these issues and make appropriate recommendations. They are then seen again at a year to discuss outcomes and recap the information given at the three month consultation. Patient Advice Line In April 2016 a dedicated phone and answer machine was set up to allow patients to have a point of contact. Patients can ring, leave a message, which is then responded to as soon as possible. This has improved patient experience; meaning they don’t need to ring the ward or other departments to nd answers to their questions. Post Liver Transplant Protocols We developed protocols to ensure all the patients received the same approach to care. This was in conjunction with appropriate professionals to ensure the advice given was in line with national standards. Our post transplant protocols cover: Cardiovascular (Heart) risk Hypertension (Blood Pressure) Renal (Kidney) surveillance Skin surveillance Contraception Immunosuppression (Anti-rejection) Currently we are developing an obesity protocol. These protocols are also shared with GPs and local hospital specialists, to ensure our patients receive a collaborative patient centred approach to care.
