Pirate-Themed Reading Log

Suggested Use

My fourth grade team prints this sheet and staples it inside a colored manila folder. Then we hole-punch it and ask students to keep it in their 3-ring binders. We ask students to read 100 minutes a week. Students do a running total of minutes. On Friday, I take the log up and check their books & minutes. I run off lots of blank, hole-punched copies and keep at my desk in case students need additional forms. At the end of every quarter, I staple all logs together and file in case I need the data later. Please feel free to use this however works best in your classroom!
