Editor’s Desk

On behalf of KMPIP family I am grateful to you all for regularly reading our news letter & appreciating our work & efforts.

Friends we know that people have become more conscious about their health and fitness. We receive regular inquiry regarding the beneficial effects of exercise ,Yoga etc. in fitness. .

A project carried out by Dr. R. Harihara Prakash & his team on ‘fitness for princess’ presented at WCPT conference was well appreciated.

Along with this fitness trend, a new concept of ‘Exercise Is Medicine’ known as EIM has emerged recently. EIM has initiated by America college of sports medicine. It is a new tool developed by the multidisciplinary EIMC advisory Council to assist the primary health care professionals to assess and prescribe physical activities for healthy and diseased person especially in communities.

Due to advancement in Medical science and technology the life span of human beings has increased but it is equally important that we as responsible health professionals give them the healthy and fruitful life. To achieve this it is very important that people involved in health care educate and appraise the patients on various aspects of healthy lifestyle, important warning sign & underlying causes of obesity and chronic disease and morbidity and mortality associated with them and when to approach the member of healthcare system.

The purpose of EIM is to make physical activity and exercise a standard part of global disease prevention. It advocates for physical activity to be considered as vital sign by the entire healthcare provider during every visit and counsel them appropriately. If ‘physical activity’ is prescribed in proper dose, it may turn out to be best prescription for most chronic conditions.

I fully agree with concept of EIM but I really doubt whether any healthcare provider can prescribe the right dose of exercise. If prescription for exercise is wrong then like medicine it may not be effective or overdose will lead to adverse effect.

Since physiology, pathophysiology & pathomechanics it part of training of Physiotherapists, I feel that they can appropriately prescribe the proper exercise. If we want to reach to larger population then in my opinion physiotherapist should be appointed at all PHC & community health centre and healthcare units. Prescription & education aspect with regards to prevention of illness & keeping oneself fit will easily be taken care of than.

Combined movement therapy This method of examination and treatment of the spine was developed by Brian Edwards, who is the author of ‘Manual of Combined Movements’ which is nowadays frequently practised in assessment by physiotherapists who does manual therapy.

For the majority of individuals normal daily activities involve complex movements through a range of cardinal planes rather than simple movements such as flexion, rotation and side flexion. A simple activity such as looking over the shoulder to reverse the car often involves combinations of the movements

including cervical rotation, lateral flexion and extension. Likewise, lifting items into the boot of the car can involve lumbar flexion, side flexion and rotation. Hence it makes sense to examine patients through a similar combination of planes of movement to determine the true nature of a patient’s dysfunction. The physical examination will then be more functional and relates more to the presenting movement disorder.

Combined movement theory utilises a biomechanical approach to describing pain patterns. The therapist chooses movements to combine to either increase or decrease symptoms. It is useful to combine movements that have a similar biomechanical effect on the motion segment and to observe whether the symptoms are increased or decreased by such movements. With the knowledge of known biomechanics of the spinal joint movement it is then possible to describe patterns of physiological ‘mechanical coupling’ of movements. Using this simple approach the clinician will be able to develop a sense of direction and thereby a rational choice of treatment technique, rather than a collection of techniques that are difficult to

progress as the patient’s condition changes for better to worse. This theory can be used more as a provocative procedure to increase the stress on the spine to ‘chase the symptoms’.

Combined movements will vary the mechanical stresses placed on the innervated structures of the spine (which include the facet joint, disc, spinal ligaments, muscles, dura, nerve root etc., ) more than single [lane movements. Hence it is possible to vary the patients symptoms both in intensity (strong pain- mild pain) and in distribution (local pain-referred pain). For patients with severe pain and with disorders of a more serious nature it is possible to minimise the stress on the painful structures by the use of combined movement theory. Even patients with more severe pain can be safely examined and treated using this approach.

It is apparent in clinical practice that end range mobilisations are more effective than early range movements. Combined movement theory permits the use of a end range treatment techniques throughout a treatment program. Progression is always by way of position rather than grade.

This theory attempts to incorporate the biomechanics of the intervertebral motion segment in examination and treatment. It is, therefore, important to have a thorough understanding of spinal biomechanics when using this approach.

PHYSIO PULSE 2015

A THEME BASED CELEBRATION This year annual physio week celebrated between 2nd to 4th March 2015. We adopted the theme ‘Each One Teach One’ which aimed to motivate the educated class to understand their moral and social responsibility towards educating the under privileged in some or the other way which can mark a big difference in the society.

Miss Lajja Gauswami, an Indian woman shooter and Dr. Dinesh Shah, the director of center for surface science and engineering at University of Florida have graced the grand finale on 4th March 2015 and released the souvenir. A club “Vidhya- Each One Teach One” was inaugurated on this day to educate and empower the underprivileged children. Activities during the Physio Pulse included various cultural and sports events by the students and faculty of KMPIP. Students of ‘Aandh Apangjan Vikas Mandal Sanchalit Vidhyalaya- A school for blind’ and Children from an NGO ‘Bachpan’ participated in cultural events on the eve of grand fenale. Gokalpura village was adopted by the institute to provide education to the underprivileged children.

PATIENT CARE

Special OPD at cancer centre

Screening and Rehabilitation Program at our cancer centre was initiated from the month of February 2015 where a special OPD is run by the physiotherapy faculty. Total of 52 patients suffering due to various problems due to cancer have benefited till April -2015.

COMMUNITY EXTESNION PROGRAMMES

Rural outreach programs/Physiotherapy camps:

• The institute conducted rural outreach programs at Boriya and Padgol village duirng Jan and Feb 2015. Total of 51 patients with various musculoskeletal, neurological, cardio-respiratory and life style disorders were benefited.

• Rehabilitation on wheels- A mobile Physiotherapy Unit

• The institute has applied for funding to the embassy of Japan in November 2014 to expand physiotherapy services in the community. The embassy staff visited the institute during January 2015 and we are awaiting final approval.

EXETRANL CONSULATNCY

The institute has provided sports physiotherapy services to the national players of football and volleyball team during following camps:

1. State level summer coaching camp for the volleyball players organized by the sports authority of Gujarat during 28/4/2015 to 17/5/2015 at Nadiad

2. Summer coaching camp organized by sports authority of Gujarat for the national football players of Khelmahakumbh during 27/4/2015 to 18/5/2015 at Shastri Ground, Anand

Ranger’s football academy

The institute has recently (Jan 2015) tied up with Rangers Football Academy, Anand for the fitness screening of children (2nd to 10th std.) registered with the Academy. The institute will look after the following activities during this screening program:

l Provide understanding about proper exercises to the children and their parents l Delivering lectures on maintaining physically healthy and strong body l Screening the child for physical fitness and/or any abnormalities l Immediate attention to sports injuries

INTEARNATIONAL WOMEN’S DAY

Women’s Empowerment cell of K M Patel Institute of Physiotherapy arranged an awareness programme as a part of International Women’s Day celebration on 13th March, 2015 at primary school of Gokalpura village, Karamsad. A street play about the issues that women face in their life like dowry, domestic violence, foeticide, child marriage, etc. was performed by the students. A special talk on menstrual hygiene and family planning was arranged for adolescent and rural women.

Farewell function and Oath Taking Ceremony for the out-going interns was organized on 26th April 2015 for 12th batch of interns. During the ceremony all the interns were awarded with Provisional Degree certificates. They were also conferred the course completion and internship completion certificates along with the awards for best project and seminar presentation. The best out-going students were awarded with gold medals.

n This year the interns attended the 53rd IAP conference at Shimla in April 2015.

n An educational visit to Indian Spinal Injury Centre, New Delhi during April 2015 was organized by the institute to provide an idea about complete rehabiliatation of patinets with spinal cord injury.

n Final year student Ms. Kalgi Desai has been selected in the badminton team of Sardar Patel University and participated in the west zone badminton competition.
