Phone/Verbal Orders – Nursing Verbal/Phone Order by Doctor, will be documented and signed by two RNs or one RN and one EEN for verification / administration Note: It is important that the ordering nurse and the nurse witness are present at the phone conversation with the doctor giving the order instruction from the commencement of the order to the final signature. 1. Go to Orders and select Add 2. Click on Add, and search for the medication/order 3. Select the order sentence most appropriate to the prescription required. Ensure you select the frequency of ‘ONCE only’ within the order sentence for phone/verbal orders (see example below). 4. Enter Physicians name and choose Communication type “Phone/Verbal (co-sign)” and select OK. Note: The dosage calculator may populate for order sentences. Ensure the appropriate weight is set in the actual weight section and the physician is aware of the final dose. 5. The doctor will confirm the medication dose against the patient weight and any other relevant detail 6. When the doctor has confirmed the dose to both nurses click Apply Dose 7. Click Done 8. Review the order detail in the Order Entry Field (OEF) 9. Pay particular attention to the *Frequency field ensure this is a Once Only dose 10. Change the First Dose Priority from Routine to NOW 11. Complete mandatory Indication Field and Nurse Witness 12. Add into Special Instruction ‘Phone Order’ or ‘Verbal Order’ 13. Once the Medical Officer has confirmed the dose and both nurses have heard and viewed the order, select Sign 14. Enter Password and OK 15. Refresh the screen 16. Navigate to the MAR and click Refresh Note: The order will now be viewable on the MAR and ready to administer via the MAW. QGEA-PUBLIC

1. Go to Orders and select Add

2. Click on Add, and search for the medication/order

What to do when there isn’t an appropriate order

sentence for the drug

When searching for a particular medication and you are unable to locate the appropriate prescription these are the additional steps you would take.

1. Select the generic order and press OK

2. Step through the process of identifying the Ordering

Physician

3. After sign off the OEF is populated as before however, there are no prepopulated details. Enter the details as instructed by the Medical Officer on the phone.

4. Select sign and continue to follow steps 9 to 16.

Note: Follow steps 1 and 2.

Note: Please ensure DOSE and ROUTE OF ADMINISTRATION are updated. Change the FIRST DOSE PRIORITY from Routine to NOW. Set FREQUENCY to ONCE ONLY. Add indication appropriate for prescription ie AGITATION. Add in SPECIAL INSTRUCTION, ‘Phone Order’ or ‘Verbal Order.’
