New Methodist EMR Website:
A new website has been created for Methodist staff to get updates on Cerner information,
training materials, videos and important resources. The website requires computers to have an
updated version of the explorer search engine (9 or above) or Google Chrome. For quick and
easy access, a desktop icon was sent to most computers within the Methodist network which
automatically launches the website using Google Chrome.
Look for tips in this newsletter where to access information on the website. If you have any questions, please call
your Cerner Support team at 354-6990.
ICD-10 Implementation for Providers:
In preparation for the implementation of ICD 10 coding system that will go into effect on October 1, 2015,
Methodist Health System is requiring an ICD 10 diagnosis code at the time of scheduling for any procedure to
be done after October 1, 2015 (i.e. Surgical, Diagnostic, PT, OT, Lab).
To aid in proper code selection, the Cerner Diagnosis Assistant tool has been implemented in the EMR. This tool
will automatically launch when providers select a diagnosis to add to an order which is dated for completion
after 10-1-15. This tool will walk the provider through the various determinations needed to establish the proper
ICD-10 code, i.e. laterality, new encounter vs. subsequent, acute vs. chronic, etc. A training video is available
on the Methodist EMR Website for more information. Physicians are encouraged to view the training material
and familiarize themselves with this tool.
NOTE: Due to increased specificity in the new code set, many of the old codes will map to unspecified codes.
We have received communication from payers that use of unspecified codes may result in delays in payment,
increase in requests for documentation and denials. For that reason, the use of unspecified codes is strongly
discouraged.
The American Academy of Professional Coders (AAPC) has produced specialty specific online training modules
that will be required for physicians, midlevel providers and some staff. These modules take approximately 3
hours and address the needed changes in documentation as well as proper code selection. It is imperative
that along with the proper diagnosis code selection, the documentation support the code description. In some
codes, this is simplified. For example, diabetes is coded by type, no longer requires controlled or uncontrolled
and one code may represent the diabetes and the complication. Unfortunately, that doesn’t hold true in other
areas, i.e. A-Fib (427.31 in ICD-9) now has 5 options in ICD-10. The complete narrative of the diagnosis code will
be necessary in the documentation to support code selection.

IMPORTANT: MPC Scanning Outside Documents Reminders
It is very important that documents are scanned to the correct document type, the correct encounter for your
clinic/department, and to the correct date. Separating out all the specific documents and scanning them
individually may seem time consuming, however for the care of the patient, it is vital that this information is
entered into the patient chart as carefully and appropriately as possible. Over the last year there have been a
number of changes with document types because of Health Maintenance, HealtheRegistries and Meaningful
Use requirements.
It is extremely important that outside documentation be scanned to the appropriate specific document types
as much as possible and not all grouped under ‘Outside Record’ like they once were. Some document types
fall under different ‘Review’ tabs, help satisfy Health Maintenance, Meaningful Use, or other necessary criteria
within the patient’s medical record.
Below are some examples of document types that help our providers and Health Coaches monitor patient
care. There are a number of other document types available (including specialty consults). For a complete list,
please reference the MPC-EMR Document Scanning List on the Methodist EMR Website.
Bronchoscopy Bronchoscopy results, photos and related pathology - regardless of source (scan in color)
Cardiac Event Monitor Cardiac event monitor reports/results (e.g. ACT monitors, King of Hearts) - regardless of source (NOTE: scan Holter monitors to 'Holter Monitor' document type)

NOTE: Because of the number of changes that occur with document types, we strongly recommend that staff
no longer set up ‘Personal Document Lists’ in Clinical Notes, as this can lead to missed document types and
scanning to incorrect document type locations. The MPC-EMR Document List is updated as these changes
occur so please watch for new versions periodically on the Methodist EMR Website.

Coding Corner
How to Fix MAR Documentation
This guide will show you how to edit the MAR in cases where it was filled out incorrectly. Filled out incorrectly
could mean missing one of the key elements needed to bill or one of the selections marked with the wrong
information.
Editing the MAR: It is very important that you get the MAR window EVERY TIME you make changes on
med/vaccine documentation. To edit the MAR you need to Right-Click the dose box
and select Modify. Do not use the other boxes.
IMPORTANT: We absolutely need all of the boxes filled out to bill the
med/vaccine successfully. A lot of what is entered in these fields drop
directly on the claim. It takes special access to edit the MAR which only
clinical staff have, any fixes HAVE to come back to the clinic.
DOSE: This drops a CPT code directly
to coding queues for the coders.
**For vaccines you will need to choose
the correct VFC or NON VFC dose
based on the patient’s age and
insurance.
**If the specific dose isn’t available
choose the dose right above what
you gave and you can comment as
such.
Admin: Fill in how the product was
given.
NDC: This populates the claim with the
NDC number for billing.
NDC Quantity: Think of this as how
many times you are using the NDC
number. ALWAYS 1.
NDC Units: This is the unit of measure.

Waste: Document any waste you

General Training information for MPC Clinic Managers:
When a request for training is received, a “Training Request form” will be sent to the manager. This form will contain the information needed by MPC Support to set up the user for Powerchart. Once a training date and time are arranged, a Microsoft Outlook meeting request will be e-mailed to you and each trainee, indicating the training time, location, training information, etc. The trainer assigned may be contacting you to obtain necessary information for training set up & materials.
Please request Cerner & Network sign-ons for new employees in advance. NOTE: It is extremely important that you do not give the Trainee this information prior to him/her being properly trained on how to navigate our patients’ medical records. Karen Rau will request sign ons for all Providers, Midlevels, Residents and students. Clinic managers will request sign ons for other staff.
Reminder: Include the Pools and Proxy information under the appropriate sections of the Sign On request form so they have access to what they need in message center.
TRANSFER Employees: To make sure employees who transfer from one department to another are set up and/or trained properly be sure to fill out the ‘User Information Section’ and mark this area appropriately.
Resident/Student Training: Send request for training to Karen Rau. She will request sign-ons. When they are received, manager will be emailed a link to the video, the user’s network sign-on and a Training Checklist with Confidentiality Agreement. When the user has watched the video and signed the Checklist, return the signed checklist to MPC Support. At that point the Cerner Login information will be sent.
