Personal Data Capture Project 2013

PREFACEThank you for reviewing this project.

I am looking to quickly create a simple and easy to use Personal Data Capture Project that results in saving the information to a Database for retrieval and reporting at a later time.

The data to be captured is personal data for the personal fitness industry.

It stands without question that the apps, concepts and ideas within this document or discussed fall under the assumed Non-Disclosure Agreement and its revisions (herein). Upon app completion Contractors will include the provision of the Source Code and its revisions. Agreement to proceed and produce the apps includes the final source code and 30-90 days support after the initial upload of the app to app store(s), or servers and roll out. All source code, concepts and assets are property of Patrick McGuire, Empowered Nutrition Products Inc., Empowered Apps Inc. and/or App-ilicious.com.

The app concepts outlined in this document are for assessment and quote only. Knowing pricing will also impact the order of production of this and future apps.

Each app may lend itself and build upon some of the features of the other and from our original project.

The goal is to build these rapidly under a tight budget and get approval for app networking, go live and promotion.

The use of this document is for assessment and quoting purposes. Once you provide a quote including fixed fee flat rate pricing and weeks in production to completion, I will list the project in oDesk and privately invite you to accept the job.

Please review the NDA and accept this document in order to be eligible for this project. Your active bid in this project represents your acceptance of the NDA. Should you win this project you may be required to sign the NDA for additional project details and opportunities.

APPS FOR PRODUCTION

Personal Data Capture Project

Project Overview:The goal of this project is to capture personal data and execute a simple task such as an onscreen calculation and trending chart.[BMI, Body Fat %, Lean Body Mass, AI (guys) or VI (girls)] I want a timeline trending chart to be on the top half of the RESULTS screen and on the bottom half a series of image or numeric dashboards that are easy to read and to the point direct. I have not provided an example screenshot, but can provide if needed.

This project needs to appear modern and fresh, appearing fun, engaging and with style that is customary of modern websites.

Project Coding Language/Systems:This project is to be built with Ruby on Rails and will utilize Heroku and as required MongoDB/Postgres.

User Experience – First Visit:1. Visit Site2. Register/Login3. Enter Goals – pick list or checkbox 4 goal options4. Enter personal stats5. Review Results Dashboard

User Experience – Second Visit:1. Visit Site2. Login3. Access Results Dashboard – Please add a nice pop up that notifies user to consider adding

weight or personal stats to update their dashboard, mainly for the trending line. Allow user to decline and allow them to do it at a later time.

4. Review Results Dashboard and access resources.

Web App Design Sample ScreenshotsThis section is for both the developer and if required a designer. Preferably as a good developer you will be able to code the screens so that I can add my own rotating image backgrounds.

DESIGNER: These are samples of how the web app phase one should look and feel and operate. Of course it is open to great suggestions. Essentially this design is the base. The top and bottom banners should allow for a slightly transparent colour (in this example transparent black) and the ability to brand the look for multiple brands depending on who is accessing the system.

The background image will scroll every 7 seconds and should be open to unique images for unique brands in accordance with the same banners color bars and logos or unique HTML/CSS based on the company.

The theme I have chosen to sample for you has come from a sample site www.jaybirdgear.com of course we will use our own background images, you can design with white or black for now if you like. Also this is a great example of a clean modern site http://projectsky.lakesidelab.com/

Page 7: Personal Data Capture Project .doc

Make the “NEXT” button stand out in green with white or black font

Phase 1 this will be a free membership site. Eventually it will be paid, maybe.

Also include the round social media buttons in the bottom corner and NOT the “LIVE HELP”

Please note the videos. These may be hosted or YouTube, currently undecided.

Note the boxes. These represent the sliding or fading background images as they rotate through

Page 8: Personal Data Capture Project .doc

All buttons or selection options should be easy to execute. Click or touch screen options.

These are personal data entry boxes. The user should be able to select Metric or imperial for data and switch back and forth.

Page 9: Personal Data Capture Project .doc

Personal Data Capture Project 2013

Results Dashboard is currently in discussion. However I am confident that I want to see a “trend” of weight and results. The primary goal is to show mainly total weight, Body Fat percentage and weight (decreasing) and Lean Body Mass % and weight (maintaining/increasing) as the primary dashboard with additional smaller dashboards to support the information as well as some added details such as AI/VI, VO2max, Max HR, etc… This page when completed will need links at the top, side or bottom to easily access information such as Edit Personal Settings, Resources, Free Stuff. Eventually we will in Phase 2,3,4,5 add functionality that will allow them to use a Nutrition Journal, Activity Journal and ultimately build personalized nutrition programs and workout programs and more.

This screen is irrelevant in Phase 1, but will be of use at a later date. It will however not have any content or copy on it. Just 5 green buttons across the middle of the screen. Please design for the blank green buttons 1, 2, 3, 4, 5. Thank you.

Page 10: Personal Data Capture Project .doc

Personal Data Capture Project 2013

DESIGNER: The results screen is the background screen with the top and bottom nav bars and will be a display of results from personal data calculations. The RESULTS screen should allow for a top half trending chart.

The middle of the screen should be a slider that can encompass many results in simple display boxes that would allow a user to slide or navigate from result to result for example

The bottom of the page will be a simple chart that shows the real data creating the trending line. It will display the personal results over 12 weeks and should allow for additional weeks to be added and scrolled across to display.

It has been recommended to use Google Charts for the Results Dashboards.

Trend Chart Covers whole screen width

Below the trend we need to show either numbers or charts of data driven from a calculation based on personal data entered on current date or historical data. These results should have a side to side sliding scale like many modern sites to access “more” results data. These include with formulas the following:

Body Fat +/- from previous week and or total from start Lean Body Mass +/- from previous week and or total from start Weekly or Monthly Fat Loss Weekly or Monthly Lean Body Mass Increase Weekly or Monthly Total Weight

Page 11: Personal Data Capture Project .doc

Personal Data Capture Project 2013

Weight to Goal (Count Down) +/- Weight to Weeks Goal (12 weeks to lose 12 kilograms = 1kg per week) show +/- tracking on this

one to speed up or stay on track.

This list can be exhaustive and growing… these are but some of the key results trackers to display

Empowered App User Story 1

FIRST VISITPatrick visits the app site for the very first time.

Patrick is prompted to register with his email or other social media account profile.

Patrick Registers with his Name and Email (maybe Facebook or other) and clicks NEXT

Patrick is then asked “What is Your Goal? Lose Fat. Build Muscle. Increase Endurance. Increase Speed.” Patrick can only select one option for the next 12 weeks. Patrick then clicks NEXT

Patrick is then asked to enter some personal stats. The first selection Patrick must make is a unit of measurement. Patrick can select either Metric (cm and kilos) or Imperial (inches and pounds).

Patrick selects Metric (METRIC highlights green) and all boxes default to “cm” in the grey shaded temporary content in the open fields boxes.

Patrick Enters all of his personal stats. The one stat that is not required is “Body Fat%” as this will be calculated by a measurement equation. IF Patrick knows his bodyfat content he can then enter his body fat as a percentage. This will be the default bodyfat% for tracking his statistics and showing on display on his Results Dashboard.

Patrick completes all fields and clicks NEXT

Patrick is then presented with his personal RESULTS DASHBOARD.

TRENDING CHART At the very top and center of the page is a trending chart as displayed by an area chart or line chart, showing his progress over the next 12 weeks. Clearly it is blank at this time as he only has his data today. One data point.Hardly a trend.

Below his weekly progress trend, Patrick can see a few unique pieces of information including his:

Body Fat Mass to Lean Body Mass show as a pie, which he would be able to drill down into for accurate data, represented by a portion of the pie being total fat mass and the other portion being total lean mass. Lean Body Mass is calculated by subtracting body fat weight from total body weight. The Lean body mass (LBM) has been described as an index superior to total body weight for prescribing proper levels of medications and for assessing metabolic disorders.

Page 12: Personal Data Capture Project .doc

Personal Data Capture Project 2013

Body Mass Index (BMI) as shown and calculated in detail on this page http://en.wikipedia.org/wiki/Body_Mass_Index

Max Heart Rate (MaxHR) as a Guage Chart, showing the Max at 220bpm as well as displaying within the gauge Patrick’s personal Max HR and the various training zones in relation to Patrick’s age see this chart for details and HR training zones by age here http://en.wikipedia.org/wiki/File:Exercise_zones.png

Below the smaller data sets and charts will appear the table that shows weekly measurements (every 7 days).

The chart tracks the following over 12+ weeks. If possible I would like a column 14 and 15 that would be ones measurement goals and the +/- to accomplish that goal. We have not yet discussed the GOALS for Personal Data but please plan for this as I will contract you for this immediately once we have this first phase of the project live. This is a table tracking each weeks data entry (measurements).

Date Height Weight Bodyfat% Neck Shoulders Chest Waist Hips Thighs (avg) Calves (avg) Inseam Clavicle Arms (avg) Forearm (avg)

Patrick can then see a few more options on the RESULTS page below the charts and across the top and side bars that will allow Patrick to update his profile, settings and goals or add new data manually. Along the bottom Patrick will be able to access inspirational videos, blogs and resources including success stories.

After reviewing his data and a few inspirational videos, Patrick logs out for another day or week.

If possible I want the ability to share on social media, Patrick’s results and a link to the app. Facebook, Google+ and Twitter. SMS and email options are great as well, but the focus will be on social media.

Second VisitA few days later Patrick returns to the site. Patrick logs in with his Facebook account, which was connected last time (the first time) he accessed the site and registered for his free membership.

Page 13: Personal Data Capture Project .doc

Personal Data Capture Project 2013

Patrick is provided a chance to update his weight and bodyfat% if he chooses to add more data points to the trending line. Patrick can either enter his Weight and/or Bodyfat or choose to SKIP. Patrick decides to enter his Weight and clicks NEXT (not SKIP).

Patrick is then greeted with his Personal RESULTS DASHBOARD. Patrick then reviews his trending line, watches a few of the videos and links to the private members blog site.

Patrick then logs out.

Third VisitPatrick comes back a few days later. Actually it is exactly one week from the first time he registered on the site. Patrick is brought to the Personal Stats Page for his weekly check in. Once Patrick enters all his stats to create new data points and clicks NEXT to access and review these new data points on his Personal RESULTS DATASHEET.

Patrick then is able to watch some more inspirational videos and visit the members only site.

Patrick Logs Out once again.

12 weeks later…

Many Visits - Patrick ReturnsThis time Patrick comes back and logs in with his facebook login button. He can then review his RESULTS DASHBOARD and see that he has accomplished his goals on his trending chart and the +/- results should all show that they are reading 0 or close to zero as he has accomplished his goals and maybe even exceeded his goals. It should be made obvious that Patrick can now SET NEW GOALS. This would be best accomplished with a button at the very top that pushes the whole dashboard down and says SET NEW GOALS.

Patrick then exports his data into an excel or csv file for future reporting and moves on to set a new goal, this time to add muscle over the next 12 weeks. Patrick can then export his data anytime he wishes either day 1 or day 365 anytime he wishes and resets his goals every quarter. IF required during the course of the year Patrick feels he needs to manually change his goals, but ideally will remain focused on his goals every 12 weeks.

NOTE: All of this data is kept and can be accessed for review by Empowered App Administrator. This will also allow us to build a case to capture data and create case studies and success stories. Eventually the ability to add photos by the user should also be added.

I am an entrepreneur with over 20 years’ fitness professional experience. At one time focusing my Personalized Nutrition Programs for fitness companies and supplying the industry with hundreds of diets per month to fitness companies on 5 continents.

Unfortunately this Personalized Nutrition Program was powerful, too powerful and about 10-12 years too early. Today it is outdated tech to say the least and the big picture is to replicate the success of my small entrepreneurial company in the new world of technology and marketing.

I am confident that the successful candidate will be invited to many of our projects as we rebuild our nutrition and fitness solutions to their former glory.

Phase 1 is what we are discussing for THIS PROJECT. It is a FREE personal data tracking with calculators.Phase 2 is personal nutrition journal with calorie tracking and planningPhase 3 is personal fitness journal with calorie tracking and planningPhase 4 is a Personalized Nutrition Program Software modulePhase 5 is going mobile and integration to other products like amiigo.com fitness bracelet, strava, etc...

Phase 2: Schedule and Ingredients Data To Be Captured

Lifestyle Data Active Lifestyle / Sedentary Lifestyle (select one) Love to Cook / Too Busy to Cook (select one) Schedule Data Monday – Tuesday – Wednesday – Thursday – Friday – Saturday – Sunday Wake Up Times (drop down 24 hours) Activity Times (drop down 24 hours) Activity Duration (drop down – 15, 30, 45, 60, 75, 120, 150, 180) Activity Name (drop down TBC, 5-10 activities) Activity Intensity (scale of 1-5, numbers, sliding scale or 5 flames)

Food Selections [this will be options to select to save to the database should this data collection project be successful]

Proteins Carbs Fats

Phase 3: Fitness Data to be Captured

Fitness Data xxxxxx (select one) xxxxxx (select one)

This is more of a fitness tracking log to start, ideally integrated with the Amiigo Fitness Bracelet and its associated open source software. In a simple format this would be to track activity, duration, intensity, etc… and track calorie burn for trending on the timeline.
