Conquering Every ObstacleLinda's Story

She arrived to work early. After all, it was her first day on her new job. It was also the first day of operations at the Health Center’s new location, so she wanted to set the right tone. Good thing she did. A short time later, just after figuring out how to use the phone and finding the key to the ladies room, she received a phone call from the State Health Department telling her the health center was operating without authorization at its new site and might have to shut down immediately.

That was Linda Muller’s first day of work as President and Chief Executive Officer of The Greater Hudson Valley Family Health Center, on July 1, 1991. Another dose of reality was soon to follow. Within a few months, she quickly realized that she was out of new space and that the more than 7,000 patients that were now receiving their care at her health center were just as cramped and crowded as they had been at the smaller, old site.

It took 20 years to nurture the growth of the health center, but Linda did it. Since assuming leadership in 1991, she has seen the health center grow from 4,000 patients to more than 18,000; from $1.8 million to more than $21 million in revenues and from 3,000 square feet of space to one that utilizes 5 different sites of care, totalling more than 80,000 square feet, crowned by a state-of-the-art health home named for the Kaplan Family, the guardian angels of our health center.

The real story, however, is not one of growth but of transformation. Today, this health center is leading our community in developing a patient-centered program of primary health care coordination that emphasizes comprehensiveness and continuity of care, is focused on increasing innovation in patient care, and results in continually improving patient outcomes.

Transformation has many meanings and comes in many shapes and sizes, but for Linda Muller, back then the goal of transformation was simple: to bring a health center critical to its community from a state of financial jeopardy and significant infrastructure challenge, to one of great programmatic success, and, most importantly, to a place where high- quality health care is the new normal.

“What’s next?” Linda says. “I’ll tell you. I won’t rest until our standard of quality health care is available to everyone in our community, and until every gap in service or accessibility I can eliminate has been eliminated. Look for more facilities, more programs that improve health, and new sites of service that meet the needs of those in other locations in our state who are, to this day, without affordable and accessible health care choices. I want to empower every patient to take charge of their health care and help them find their own health care voice. Patient by patient, block by block, community by community, we will create a truly healthy New York. Join us, help us; I can assure you, it is quite a ride.”

I believe the cornerstones of a strong and vibrant community are the people who take personal responsibility for that community’s quality of life. That is why I have chosen to give my time, talent, and contributions to The Greater Hudson Valley Family Health Center. It is an organization that has transformed itself into an economic and community leader in Newburgh, and in doing so has transformed the way health care is provided to and accessed by local residents.

When I first began my relationship with the Family Health Center in 2004, it was a small center struggling to meet the needs of its approximately 10,000 patients in severely limited space. Despite the challenges the center was facing, my business experience helped me recognize that it was an organization on the verge of dramatic growth and improvement, and that it had the right leadership to reach a far greater potential. I was fascinated by the way the health center’s outcome-driven clinical and business team would always strive to provide the best care by maximizing resources. Little did I realize then that this was just the beginning of what would become a personal mission for myself.

When plans were forming to build a new main facility, I began to volunteer more of my time and energy, which led me to join the Board of Directors and eventually become Chairman.

This gave me the opportunity to witness first-hand the growth and evolution of the health center into the 60,000 square-foot, state-of-the-art facility it is today. Now, The Greater Hudson Valley Family Health Center has national recognition as a Patient Centered Medical Home, which attests to the fact that our technology, services, and the care given by our providers is second-to-none.

My initial hunch about the Family Health Center was correct: this is an organization on the cutting edge of what it does. I am still intrigued by what the health center can become and I’m excited to chair the Board of Directors of an organization that is constantly reinventing itself as it leads the way in the ever-changing world of health care in the 21st Century. I’m not quite sure what health care delivery will look like in 20 years, but I do know that The Greater Hudson Valley Family Health Center will be at the forefront of defining the model.

� 99% of children who are patients at the Health Center have had at least 1 lead test before they are 2 years old. The statewide average is 78%.

� 91% of the Health Center’s female patients seen in our OB/GYN Department are referred for at least 1 mammogram every 2 years. The national average is 71% and the statewide average is 70%.

� 95% of the Health Center’s female patients seen in our OB/GYN Department have received at least 1 Pap smear in the past three years. The national average is 77% and the statewide average is 70%.

� 97% of the Health Center’s adult asthmatic patients are on preferred modern therapies.The national average is 81%.

� 60% of the Health Center’s adult patients with a confirmed diagnosis of HNT have blood pressure within the therapeutic range of less than 140/80. The national average is 56%.

� 91% of the Health Center’s Adolescents have a Well Child Visit annually. The national average is 43% and the statewide average is 59%.

� 97% of children at the Health Center with a diagnosis of strep throat receive recommended care. The national average is 78% and the statewide average is 87

THE PATIENT EXPERIENCE

ELECTRONIC MEDICAL RECORDSIn 2011, a major technological transformation took place at The Greater Hudson Valley Family Health Center. Starting in May, GHVFHC began implementing Electronic Health Records (EHR) throughout the organization.

EHRs offer many benefits to our patients. Important information in your medical history - such as blood type, chronic medical conditions, medications, and allergies - can be accessed with great ease. Ultimately, patients will be able to access their own medical records through a secure patient portal directly from their homes or mobile phones.

Launching EHRs has increased our technological capacity, making the delivery of health care services more accessible and convenient for patients.

The implementation of Centricity, our EHR system, and the cutting-edge technology it provides, helps our expert medical staff increase the quality, safety, and efficiency of all services provided to our patients and their families at all our locations throughout Orange County

Staying ahead of the curve with regard to the reform and evolution of our national, state and local systems of health care is quite a challenge. Change is coming at providers, institutions of health care, and government health and human service agencies on an almost daily basis. I know that many of our medical practices are working diligently to keep up. A few, like The Greater Hudson Valley Family Health Center, seem to be ahead of the curve.

How are they doing this?

•By being open to change and the possibility that there will be new and better ways of providing health care to the community.

•Developing a strong and positive emphasis on performance-based medicine; measuring data and outcomes, examining closely for shortcom-ings and gaps in care, and using the evidence available to make adjustments and corrections.

•Listening to patients and families. Including patients in the coordination of their care is a critical change in the culture of medical care.

•The Greater Hudson Valley Family Health Center is the medical home for more than 18,000 patients in Orange County. It is a distinct and dedicated Level III Patient Centered Medical Home for each of these patients.

•Knowing what is coming is key: Our health care system is under constant pressure to adjust and economize. The ability to stay ahead of the curve, understand how policy and funding is changing, and work collaboratively with others to develop innovative solutions is a key element of transforming the provision of primary health care. The Greater Hudson Valley Family Health Center has excelled in that regard.

Health Homes. Accountable Care Organizations.Regional Health Information Organizations. These and many more new structures for primary care provision are already on the horizon. The health care organization that is prepared for the future will find a smoother and easier path toward the transformation that is coming to our county and to our medical community. The Greater Hudson Valley Family Health Center seems well on its way to accomplishing this.

The National Commission on Quality Assurance’s (NCQA) Patient Centered Medical Home (PCMH) program recognizes physician practices that prioritize the strengthening of the physician‐patient relationship, coordinate care for patients across multiple settings, and engage in a team approach to improve patient care.

“The Patient–Centered Medical Home promises to improve health and health care” said GHVFHC President & CEO Linda S. Muller. “The active, ongoing relationship between a patient and a clinician in medical homes fosters an all‐too‐rare goal in care: staying healthy and preventing illnesses in the first place.

PCMH recognition shows that GHVFHC has the tools, systems, and resources to provide its patients with the right care at the right time.”

NCQA has designated our health center as a Level III Patient Centered Medical Home, the highest designation currently available.

With plans to open additional sites, GHVFHC was listed in Forbes Magazine as a key player in job growth in Orange and Dutchess Counties. on why the health center helps the economyI have watched The Greater Hudson Valley Family Health Center mature and develop for all of my years as President of the Orange County Chamber of Commerce. To be frank, there were times when I worried if they would make it. We all faced tough economies, constricting resources, seemingly insurmountable challenges to find the resources to improve facilities and equipment, and to strengthen staff and programs. I worried about whether the Health Center could hold its own, let alone expand and improve its services and the care it provides for its patients.

The change in the health center over the past five years has been remarkable. They have created a new home for themselves and their patients – one that is warm, welcoming and impressive. They did this through a successful capital campaign at a time when raising funds for non-profit endeavors was more difficult than ever. And they did it using almost 100% private funding. This alone is a remarkable accomplishment and one that speaks to the confidence that our

local business community has in the health center and its mission.

How are they doing this?

•The new facility has allowed GHVFHC to increase the hours of care they provide, offer new and needed medical services, improve connections with the community, and expand jobs. Most of these new positions have been filled by people who live locally. This health center has become one of our more successful economic engines, providing needed care, and reliable employment to a community that is in dire need of both.

•Old ways of doing business were discarded. New systems that focus on the “customer” were put in place. Waiting times for appointments and to see the doctor were dramatically reduced. Technology was put in place to eliminate paper records, and to give patients electronic access to their own medical records.

•That same technology is being used to

closely examine medical outcomes, to find areas in need of immediate improvement, and to allow the health center to respond to emerging community health care needs.

This truly is a health center for all people in the community, poised to be responsive to the rapidly emerging evolution of primary health care across America. I learned that it has accomplished this by being fearless about change, and throughout this process, by embracing transformation.

150 new jobs in 24 months, and I am told more to come! That is my idea of a strong and valuable member of the business community of Orange County. I am proud of what The Greater Hudson Valley Family Health Center does for our citizens, and that it is an enthusiastic member of our Chamber of Commerce.

I am pleased to take this opportunity to share my great esteem for The Greater Hudson Valley Family Health Center. As a long-time academic presence in Newburgh, Mount Saint Mary College has played a strong role in the development of leaders who will be able to motivate greater social equity, justice, and the honor of service throughout our community. Our Dedicated VolunteersRuth SmithRuth has been a cornerstone of the Volunteer Department at The Greater Hudson Valley Family Health Center Inc. for 4 years. She serves as the Volunteer Department Liaison, working with the Development Department, and external agencies, and individuals to help expand the volunteer corps at GHVFHC by recruiting new volunteers. She participates in monthly building tours of our new facility, planning fundraising events, and stewarding health center friends and donors. A familiar face at all of our locations, Ruth is always ready and willing to lend her time and talents to so many projects: Reach Out and Read, outreach activities at the health center, Encore special events, and our annual Pillars of the Community Gala.

Ruth’s efforts have helped transform The Family Health Center into the leading medical practice in Newburgh. She has been integral in coordinating the generous contributions of many donors and organizations. This influx of capital has proven invaluable in funding our growth and improvement over the last few years.

It is Ruth’s heartfelt belief that the staff of The Family Health Center truly welcomes everyone in the community and does everything it can to provide them with the best care possible. She feels this is the best place she’s ever worked – with the most caring and enthusiastic people dedicated to the common goal of accessible and affordable health care for all.

Lianne RittbergIn the winter before it opened, Lianne came for a visit to The Greater Hudson Valley Family Health Center’s new main facility. She met with Ruth Smith and other volunteer team members to learn about our health center and how it serves the Newburgh community. When The Kaplan Family Pavilion opened in April 2010, Lianne was our first volunteer at the front desk, greeting patients and visitors with a warm smile and assisting with the phones. She also learned Spanish in order to help our Latino clients make their way about the center. Lianne cheerfully steps up to assist with numerous special events, including our Annual Community Health Fair, Book Fair, Family Dollar Days, and many others.

As The Family Health Center’s first disabled volunteer, Lianne is helping to transform the perception of what we can do to serve the community. She came to the health center because she has always had an interest in a nursing profession, though she is unable to pursue it due to her inability to lift a patient. Volunteering here has given her an outlet for her seemingly endless energy, constantly advocating for people with disabili-ties. Thanks to her, The Family Health Center has grown in its understanding of the needs of its disabled patients.

Lianne is tremendously appreciated for her dedication and commitment to our organization and the patients we serve.
