I have to admit that I still find it very hard to talkabout “erectile dysfunction” but before you startimagining stuff that - trust me, readers - you reallydon’t want to get into, let me quickly add that all Imeant with that opening is that I still have a verytough time accepting the term “erectile dysfunc-tion” (generally defined as the inability to developor maintain a penile erection during sexual inter-course) instead of what I consider the much moreimaginative, more descriptive, and certainly morewidely-recognized lay term for the same problem, namely “impotence.”Despite my views and much to my chagrin,

however, the term “erectile dysfunction” has cer-tainly caught on and has now nearly completelyreplaced “impotence,” even in the public domain,a change that reflects a very interesting trend inthe use of medical language over the last couple ofdecades, a trend that stems from changing socialmores that have resulted in many previouslylargely “hidden” medical conditions coming topublic prominence (“out of the closet,” as it were). And as part of the process of helping us adjust to

those conditions (to accept them without shame, and just as important, to help us discuss these con-ditions in public), health experts (aided by drugcompanies) have developed new terminology formany old health “problems,” labels that the ex-perts feel are more publicly palatable (not to men-tion, way more flexible and hence more profitable, for both the drug companies and the experts) than

the old ones we’ve always relied on.So, for example, “social anxiety disorder,” a con-

dition that just sounds like it’s screaming to betreated and followed up for long periods is nowoften offered as a diagnosis instead of what we allused to know as “excessive shyness,” which wassimply considered to be a (sometimes unfortunate)variant of normal.Worse, battalions of “pre”-somethings are now

charging into the medical literature. For example,“pre-hypertension” is now commonly used as theterm-of-preference for what used to be called highnormal blood pressure. And since someone with pre-hypertension sounds like he’s standing on the edgeof an abyss, pre-hypertension clearly demands treat-ment, whereas high normal blood pressure was fre-quently accepted as just something that “we shouldkeep an eye on.”Anyway, as any T/-watcher can tell you, “erec-

tile dysfunction” (often just D) is now prettymuch the standard for what we all used to call im-potence, so I’ll live with it P I mean live with theterm, of course.“But what can you tell us, Art, about this incred-

ibly well-advertised problem that we don’t alreadyknow?” you may well ask, “besides, perhaps, ex-plaining why the people in D drug commercialsalways have those silly smiles on their faces whenthey’ve invariably forgotten to turn off the lawnmower or are disturbing everyone by taking theirseats late for the opera or why an amazing num-

*uestions, questions, questions: It seems like alla Pharmacist does during a workday is answer ques-tions. ven when we are not working, we are stillanswering questions. *uestions about: medica-tions, medical conditions, pets, costs, side effectsand more. The next question is: do we really mindall of these inquiries? The answer P luckily P is no,not at all. Helping people is what we do best. Peo-ple might feel a bit awkward about asking ques-tions that they believe are simple or trivial.Nonsense! The only dumb question is the onenever asked. Peoples Pharmacists love answeringquestions. It provides us with an opportunity toshare and help our patients. LetNs look at the mostcommon prescription questions that I get asked. What is the shelf life of prescription drugs?

Prepackaged, over the counter medications willhave an expiry date stamped on them. You reallyshouldnNt have any unused prescription drugsaround the house. Many of these will not have ex-piry dates because they were intended to be usedwithin three to six months. My general rule is thatif more than two years has passed since the date ofdispensing, the medication has passed its expirydate. If you have medication to be used at a laterdate, for travel or just-in-case, ask your PeoplesPharmacist to mark the expiry date on the bottle.Any medication that has pasted its expiry dateshould be disposed of properly. This nicely leads to my next question: How

should I dispose of old medications? You can bring

medications to ANY BC pharmacy for proper dis-posal. If a medication or over the counter productwas purchased in a pharmacy, chances are that itcan be returned there. The exceptions are cosmet-ics, sharps and household chemicals. Some phar-macies accept needles and other medical sharps fordisposal, but it is best to check with your local Peo-ples Pharmacist about this service before bringingany in. Another small bit of advice; always purchasesmall bottles of over the counter medications un-less you use it regularly. While it is true that largersizes can be more economical, these savings are lostwhen 9 of the bottle expires. I dispose of manylarge supplement bottles with only a few tabletsmissing. How should prescription medications be stored?

The first part of this answer is: always out of thereach of children and pets. They should also be keptin a dry place, away from heat, humidity and light.Perhaps the worst place to store medication is thebathroom. It is generally bright, moist, warm andcabinets are easily accessible by anyone. I think thebest place to store medications is in an upper cab-inet in the kitchen. They should also be stored intheir original containers. Do not combine differentmedications into the same bottle. The exception tothis is if you place them in a weekly pill box (dos-sette), which is a great idea. DonNt forget to askyour Peoples Pharmacist if there are any specialstorage requirement for your medications.I sometimes forget to take my medications, what

should I do? This is perhaps the most troublesomeand least asked question. While taking medicationsregularly might be necessary for maintaining yourhealth, it is not a natural act like eating or sleeping. The solution is to make taking your medication part ofyour daily routine. (ne trick to begin any new habitis to attach it to an already established habit. If you al-ways make coffee in the morning, place a sticky noteon the coffee machine to remind you to take yourmedications. If you are a hi-tech person, downloadone of the many free medication reminder apps foriPhone or Android. Do you always wear a watch?Place a bright red elastic band around it as a reminder.(ften it only takes a week to establish a new habit;both good or bad. If you take multiple medications, make a list of which ones should be taken at whichtime. Ask your Peoples Pharmacist for a medication re-view. This service is free and can be helpful to learnabout your medications and when they should betaken. Are generic medications just as good as the brand

name drugs? In a few words, the answer is: yes. InCanada, when a new drug is introduced to the mar-ket it has about years of patent protection. Thismeans that only the patent holder, the brand namedrug company who created it, can produce and sellit. After the patent expires, other generic drug man-ufacturers can apply to Health Canada to sell thesame drug. These generic companies must prove sci-entifically that their generic drugs are just as safe andeffective as the brand name product. BC Pharmacistsare allowed by legislation to substitute a genericdrug for the brand name drug; we usually ask youfirst. Most people welcome the idea of saving moneyby switching to a generic drug. What can you do if your medication tastes bad?

This can be a problem for little children. Little onesoften have more sensitive taste buds and things tendto taste stronger. Here are a few tips that everyonecan use to get bitter medicine down:• Sip or suck on something very cold before takingmedication. This cold temperature will help to dull your

Influenza, often called the flu, is an infec-tion of the nose, throat and lungs causedby an influenza virus. Many people use theterm “the flu” to refer to any illness causedby a virus, such as ‘stomach flu’ or the com-mon cold, which are different than in-fluenza.The flu is not always a harmless illness. It

can cause serious health risks, includingdeath. A person with influenza is also at riskof other infections. These include viral orbacterial pneumonia, an infection of thelungs.People at risk. The risk of complications is greater for

seniors 65 years and older, very young chil-dren, and people who have lung or heartdiseases, certain chronic health problems,or weakened immune systems. What are the symptoms?Influenza symptoms can include fever,

headache, muscle pain, runny nose, sorethroat, extreme tiredness, and cough. Chil-dren may also experience nausea, vomiting,or diarrhea.Symptoms can begin about one to four

days after a person is first exposed to theinfluenza virus. Fever and other symptomsmay last up to seven to ten days, with thecough and weakness lasting up to twomore weeks. How to fight a flu- Drink fluids: Increase your daily amount ofwater, juice and soup.- Get plenty of rest for seven to ten days- Take acetaminophen to reduce a fever. If your symptoms do not improve, see

your doctor; you may be suffering fromcomplications of the flu.

Wash your hands frequentlyTwenty seconds of hand washingwith warm water and soap helpsremove bacteria and viruses. Washbefore and after eating, after usingthe bathroom, after coughing orsneezing, and after touching sur-faces that may have been contam-inated by other people.Cover your coughs or sneezesUse a tissue, or raise your arm upto your face to cough or sneezeinto your sleeve. If you use a tissue, throw it away and wash your handsimmediately.Keep surface areas cleanDisinfect doorknobs, light switches, telephones, keyboards and othersurfaces regularly.Avoid touching your, mouth, nose and eyesGerms quickly spread this way!Maintain a healthy lifestyleMake sure you get plenty of rest, eat a proper diet, exercise, and taketime for relaxation.Stay home, If you get sick.Wait until you no longer have afever and your cough is improving.Get a flu shot.Your best way to protect yourselfagaint the seasonal flu is to get im-munized every year.

sense of taste. Popsicles or ice cubes are suitable. • Mix it or chase it down with something very sweet. This sweetness will distract your taste buds and makethe bitterness taste less. Use ice cream, honey, milkor fruit juices for this trick.• Take your medication with a big glass of water. This will ensure that it will get flushed out of yourmouth.• Brush your teeth afterwards.• Drink a glass of an acidic beverage after your med-ication. This acidity with help to mask the bitterflavour. Cola and orange juice tend to work best.

What is the best way to travel with medication? With a little planning, traveling with your medica-tions is relatively easy. Before you go, check withyour travel agency, or foreign embassies, to ensureyour medications are allowed in the country. Usu-ally this is only an issue with medications for pain,i.e. narcotics. Only take enough medication to lastfor your trip, plus a few days extra in case your flightis delayed. If you need a smaller vial, ask your Peo-

ples Pharmacist for one. Always, always, always keepyour medications in its original and properly labeledcontainer. Never combine two medications into onecontainer in order to save space. It might be helpfulto have a signed copy of your prescription. If you re-quire syringes to use your medication, ask yourPhysician to write you a letter stating this fact.

I'm sure there are a few question that I neglectedto answer. If you have any questions about yourmedications, feel free to ask your Peoples Pharma-cist. Your community Pharmacist is always accessi-ble and ready to help. All you have to do it ask. Theonly silly question is the one that goes unasked.

ber of people in those ads seem to have time forsex at times of the day that the rest of us reservefor other obligations.”To be honest, I have no way of explaining any of

that, but I can tell you a few things about ED thatare very important, starting with some of thecauses.Much like stage fright, temporary ED can clearly

occur for purely psychological reasons (“Yikes! AmI really up for this?”), and of course, the more aman focuses on his inability to perform, the lesslikely he is to be able to rise to the next occasion.Temporary ED can also be due to the use of recre-ational drugs (“Well, I certainly wouldn’t use theword groovy to describe what just happened,Mike.”), overuse of alcohol, fatigue, or even justlack of interest at the crucial moment (“Sorry, hon,it’s just that I still can’t stop thinking about whatthe Bruins did to the Canucks.”)More frequent instances of ED can be caused,

of course, by many of the same factors that pro-duce temporary ED but it can also be due to otherimportant causes such as the presence of co-exist-ing chronic illnesses, especially diabetes, neurolog-ical problems, and any condition that produceschronic pain, and as the side effect of many drugs, most frustratingly, many of the drugs that are usedto treat those chronic illnesses that I just men-tioned, such as for example, drugs for high bloodpressure and drugs to treat depression.ED is also strongly linked to unhealthy lifestylepractices, particularly smoking, being significantlyoverweight, and this one may surprise you, biking(“ Serves them right,” a female friend of mineonce said, “for wearing such ugly spandex out-fits”).ED is also associated with many benign prostate

problems, it’s a common complication of prostatesurgery, it can be caused by hormonal problems, and on and on. But most important, by far, I think, is ED’s strong

connection with co-existing heart disease becausein a huge number of men, ED is actually a strongsignal that their blood vessels are very unhealthy.

So taking all that you read in the precedingparagraphs into account, you should easily seethat a man with ED must think of his wiltingwillie as his own personal canary in his own per-sonal mine (I suppose I could have employed abetter image, but I think this one may stick withyou), that is, ED should invariably be taken assign of other potential problems, especially ofco-existing heart disease. All of which means that despite what you may

deduce from those TV ads about how effectiveCialis and Levitra and Viagra and the other drugs(which are known as PDE5 inhibitors) are incounter-acting ED, and they are generally quiteeffective, ED must never be treated as a simplesymptom to be swatted down (or up?) with asimple remedy.Rather rule number one about ED is this: a man

with ED must always get a full check-up to figureout what else may be wrong, which brings up avery important corollary of that rule: if you haveEd and you’re reluctant for whatever reason to seea doctor about it, never get around that reluc-tance by simply ordering those drugs on the net,not only because what you get often shipped isabout as close to resembling a real ED drug as Iam to Prince William, but also because you reallyneed to muster up the courage to see yourdoctor about your problem. When it comes to treating ED, many studies have

shown that the most important first-line therapy isto lose weight, do some exercise, and stop smoking.Once you’ve changed your lifestyle in a positive

direction, if the ED is still kicking in, then that’swhen you should consider getting one of thosedrugs for ED, but be warned that PDE5 inhibitorscome with several potential limitations, so as al-ways, a good source to tap for information is yourpharmacist, who is likely as familiar with ED drugsas your doctor is.Actually, I’m not sure that came out the way I

meant it to, but I think you know what I mean.Dr. Art Hister can be heard on CKNW and other Corus Radio Networkstations on House Calls on Saturdays at 10 AM, as well as seen onGlobal TV news on Saturday mornings at 9:20.

Now some of you may be wondering what the world’s secondhighest mountain has to do with calcium and I suppose it wouldonly be fair to say that if you are of a mind to climb mountainsyou may wish to read on. Calcium certainly is an essential nutri-ent that is needed to maintain the healthy bones and musclesthat will power you toward those lofty peaks you aspire to; but Iam not here to talk about tall mountains. The K2 I will be writ-ing about is a once overlooked vitamin that has landed a lead rolein the calcium show.

The fat-soluble vitamin K family consists of vitamin K1 (phyl-loquinone), vitamin K2 (menaquinone) and vitamin K3 (mena-dione) which is a synthetic form of the vitamin. Vitamin K1 iswidely known for its blood clotting effects and makes up thebulk of the vitamin K in our diet; we get K1 from leafy greenvegetables like kale, spinach and broccoli. Vitamin K2 is a rel-atively new kid on the block and is generating a buzz in recog-nition for the role it plays in calcium metabolism and hearthealth. The best source for K2 is found in a fermented soybeanproduct called Natto, which is not generally too appealing tothe Western palate; other sources are eggs, meat, and aged orfermented cheeses. Beneficial bacteria in our gut also producea small amount of Vitamin K.

For years now we have been fighting the battle of boneloss by throwing mega doses of calcium supplements intothe daily regime. And while some supplemental calciummay certainly be an asset in both the prevention and treat-ment of bone loss, too much of a good thing can create animbalance and lead to other problems. One of the ways inwhich the body deals with too much calcium is to deposit itin the soft tissues, joints and vital organs. These calcium de-posits can contribute to heart disease, cancers and painfulconditions such as kidney stones and inflamed joints. Someresearchers suggest that the dietary calcium we derive fromwhole food is best and calcium supplements should be usedin moderation and with a degree of caution.

But we have to remember that calcium is not the onlybone building nutrient; a good calcium supplement will con-tain other important nutrients involved in the process. The

major ingredients we need for healthy bones are calcium toprovide bone structure, vitamin D to help with absorption, phosphorus to aid in mineralization and magnesium to assistin the metabolization of calcium. Boron, zinc, fluoride, cop-per and many other trace nutrients are needed in lesseramounts but are no less vital. Quite recently, vitamin K2 wasdiscovered and has been upgraded in importance for its roleand relationship with calcium. This could be the best littlediscovery yet in the area of bone health.

One of the main wonders of Vitamin K2 is that it acts likekind of a calcium traffic cop. Simply put, vitamin K2 will di-rect calcium to the areas where it is supposed to be and pre-vent it from building up in areas where it does not belong. There are studies that link K2 to improved bone density andthe ability to break down calcium deposits in the arteries;there is also some evidence that it may help with certain cancers and slow the onset of dementia.

Vitamin K deficiency is more likely going to be a risk fac-tor for those who rely heavily on processed foods or thosewho may have chronic inflammation in the digestive tract, which can interfere with the optimal assimilation of nutri-ents. Long-term use of antibiotics and some prescriptiondrugs is also disruptive to the digestive ecosystem and can affect absorption of essential nutrients.

Remember that optimal health is all about balance. Megadoses of calcium may have therapeutic value over short term, but ideally we should strive toward getting most of our cal-cium from our diet and using supplements when a little extraboost is needed. Instead of upping your calcium dosage, perhaps the answer lies in taking vitamin K2 along with othersynergistic nutrients so the calcium we do ingest is well uti-lized. In a well-balanced supplement, this could mean thatless is actually more. Talk with your health care professionalabout the best supplement for your particular needs andkeep your eye on K2. This is one little superstar that I amcertainly going to be paying more attention to!
