2015 has been an exciting and productive year for the Central Coast VNA & Hospice. The Monterey Chamber of Commerce awarded the VNA with the Business Excellence Award in Health and we achieved the Home Health Quality of Patient Care Star rating of 4.5 which places us in the top 7% of Home Health agencies nationwide. The VNA home health and the hospice divisions are accepting more patients on service than ever before because of specialized training for the admission team, implementation of a Transition Care Manager, and a

Clinical Program Manager, to focus on care programs that utilize continuous patient centric care.

The VNA & Hospice started the Regional Palliative Care program by establishing a clinically based advisory group in Monterey County. Over 30 local physicians attended and are now working in three different sub-groups to continue the work in this long term and on-going project. With the assistance of some seed money from the Hospice Giving Foundation, VNA is creating a physician driven educational program that will provide more compassionate care to patients and their families experiencing long term critical illness. This program will allow all providers of care to be prepared to provide comprehensive integrated services in a collaborative and coordinated fashion to patients receiving standard medical care that need compassionate and dignified symptom relief support as well.

We have received an unqualified audit report for 2014, ratified a new union contract through 2017, and implemented the new IDC10 codes for home health and hospice, as per government regulations.

I am looking forward to seeing our regional palliative care program develop and expand to all clinicians in the Monterey County area for the benefit of our aging population and strengthening our collaborative commitment to the health industry. We are continuing our community Wellness program through the generosity of our donors by providing wellness programs within strategic housing complexes that traditionally do not seek out wellness.

The Central Coast VNA & Hospice will be turning 65 years old in 2016, and I am honored to lead the oldest not-for-profit home health and hospice that is a preferred provider within the Central Coast area. Thank you to all our staff, volunteers and especially our donors that have made our organization what it is today.

Planned Giving1. An unrestricted gift or bequest is of special value because of the freedom to use it wherever and however needed. An unrestricted gift or bequest intended for the general purposes of the VNA & Hospice, one which the VNA & Hospice is free to use either to establish an endowment fund or to expend in whole or in part as it deems best, might read simply:

“I hereby give (give, devise and bequeath) to The Central Coast VNA & Hospice, a California non-profit 501(c)3 public benefit corporation, Tax id 94-1205572 (. . .percent of my estate or percentage of my residuary estate) for the general purposes of the VNA & Hospice as the VNA & Hospice may determine.”

2. If the donor or testator wishes to assure that the principal will be used to establish an endowment fund and remain as such permanently with the earnings only to be expended, the will might read as follows:

*The CCVNA Foundation will accept donor named endowments that equal $25,000 or more to create permanent endowments for the Central Coast VNA & Hospice programs.

First, let me introduce myself. My name is Desiree and I am a VNA primary care nurse. That means I am the lead clinician or field clinician on all my patients. I see approximately five patients a day and they are on care for approximately 60 days and then are either renewed on care, graduate to wellness, or move on to another part of the care continuum. I started in southern California as a hospital nurse in the chemo therapy ward. I love being a home health care nurse for the VNA because I’m out in the world, making visits to people’s homes so they can understand and care for their health issues. I am directed by the patient’s primary physician orders of care and guided by my supervisory team. With today’s electronics, I can synch my laptop in the morning while I’m still at home and will get all the latest information on my patients. I will be driving from Castroville to as far away as Carmel for my patients. 1st patient of the day 7:45AM: Sally has two cute pug dogs that greet me at the door. Sally has just returned from the hospital with cardiac issues and her husband Bill is her caregiver. Sally appreciates the fact that VNA tries to keep the same nurses on each case so there is some continuity of care and a bond can be created between the nurse and the patient/family. Bill is worried but he trusts the VNA and Sally’s doctor to instruct him on how to care for Sally properly and get her back on her feet. Sally fell while Bill was in the shower last night letting the dogs out the sliding glass door and has a scrape on her knee. After I do her vitals and review her skills (standing, walking across the room) and checking for any weight gain on the scale we provide to cardiac patients, we discuss the need for a physical therapist to re-educate and reiterate fall/safety and encourage Sally not to do tasks unsupervised that would be a potential fall risk. I review strengthening exercises with Bill and Sally. Both can benefit if they do them together. I talk to Bill about letting Sally do some tasks that are safe with him around rather than him trying to be husband of the century. He won’t be exhausted and she will get stronger and feel better about herself. They want me to have tea and banana bread but I need to move on to my next patient. I move to my car, make a call to our physical therapist to have him schedule a time to see Sally, chart my notes and know that even though Sally just came out of the hospital, she will get better with Bill watching over her and me working on her medical needs at home.

2nd patient 9:30AM: Alice and Jane are twin sisters, in their late 80’s. When you work for a home health organization, you see situations that no one would ever know about otherwise. As I ring the doorbell I hear Alice call out to just come in. All patients are called the day before and told that I will be coming by with an approximate time so we don’t conflict with doctor’s appointments. Otherwise, the patient is required to be home bound to receive home health care. This makes sense. If you can leave the house and drive around, you can go to your doctor for care or see the therapist at his office. Some patients don’t see it that way, until they realize that Medicare is going to deny their claim. It is not the VNA who stops service. Back to Alice and Jane. Alice had pneumonia and COPD from years of smoking. Jane still smokes and even though I tell her she needs to go outside when she has a cigarette, she sometimes forgets. I know I have compliance issues but if I keep on educating them about the problem eventually it will sink in and Jane might stop smoking all together. I look for a place for my medical bag and realize there is fecal material and urine on the floor. Since both ladies are advanced in age, they don’t see as well as they should and forget they need to let the Chihuahua out to the back yard. I know I have to talk to them about getting someone in to clean everything up and keep it up. They don’t like strangers but when I say it needs to be done to help Alice get well then Jane gets on the phone and she orders the cleaning. I’m hoping when I come back next week, it really has happened. I open the windows and let fresh air circulate and let the dog outside. Alice says she can breathe better already. Sometimes you have to be more than a nurse to help patients get better. We have her inhalation treatment, check her oxygen levels and warn her about lying around on the couch all day watching reruns of Rifleman. Jane has to get Alice up and walking around the house. I wash my hands in the bathroom and finish my charting. The condition of the home becomes part of the medical record

and the VNA will try to help with donated funds from their “Quality of Life” program to get the ladies back on track. The VNA is here to seek out the best solutions to keep Alice from re-hospitalization and at home where she is happiest.

3rd patient 11:15AM: George’s caregiver Joe, is a big guy from a domestic care agency. George has Joe for 4 days a week, 24 hours a day and then another woman comes in for the rest of the week. George has wound issues since he is wheelchair bound and we have been working on them with a wound vac for a while. I’m pleased to see there is progress and talk to Joe about repositioning George when he is in bed so he doesn’t get pressure sores. I put George through his paces – How is his ability to transfer from the bed to the commode or his wheelchair; is he bathing appropriately, how is his pain management – do we need to make any medication adjustment? (I’ll have to call the primary physician to adjust his medications) Does he understand when and how to take his medications. VNA provides a weekly pill box to every patient and reviews the medications in that box each time we visit the patient to verify the medications are distributed in the box appropriately and the patient is taking the medications at the right times. Medication management is the hardest to get the patient to pay attention to and since most patients’ take anywhere from 5 to 10 medications this can be disastrous if it isn’t monitored correctly. I work with George each week on filling his med-box, not his caregiver since Joe is an employee. Back on the road.

12:30PM: Lunch in the car and a few calls to check on my mother who lives in the Midwest and my husband to remind him that our son has a baseball game this afternoon and pick him up at the field.

4th patient 1:45PM: Rosa lives with her daughter and is on palliative care (comfort care for those patients continuing medical treatments). With palliative care we bring in a whole team of clinicians and volunteers to support the family as well as the patient. We have spiritual care, medical social workers; a physician specialized in palliative care oversight, therapists (physical and occupational) as well as me as the primary nurse. Rosa has a history of Alzheimer’s, dementia falls, and depression along with her end stage pancreatic cancer. Anna, her daughter has sought respite so she could just rest which was provided through a respite grant award VNA has obtained. Rosa is always sweet with me and compliant but she is becoming more and more noncompliant with Anna and the other clinicians. Rosa will wait for Anna to go to sleep and then slip out of bed and ultimately fall on the tile floor. She refuses all attempts to increase functional abilities. We have a life line and whistle for her to use. Within days of my visit, Rosa fell again and broke her hip. She ended up being admitted to an Assisted Living facility and re-admitted to VNA care until her death. The VNA provides bereavement to Anna and will be there for her for up to a year or more depending on her needs.

5th patient 3PM: Benny is always happy to see me walk through the door. He ordered flowers and had them sent to the office to thank me last week. He is graduating today and going off service. He has met all his goals and is returning to his normal life. His wife Connie can’t say enough about VNA care. Benny had a hip operation, so he did a lot of physical therapy and will now continue therapy in an outpatient setting. He also is a diabetic but I’ve been educating Connie on easy ways to help his diet so his blood sugar stays level and he finally decided to comply with his medication regime. He feels great and looks it too. Connie and Benny thank me for always being so patient, explaining and working on his exercises each time. They say I always asked so many questions and wanted so much detail. (That’s my job!) I give each a quick hug, close my notes off and get back in my car to head to the office.

At the office, I will meet with the supervisory team to review issues on some of the patients and review patient charts. I make my physician calls and my calls to tomorrows patients. Pack my medical bag with needed supplies. I am happy that most supplies now are shipped directly to the patient’s home. It makes it easier to not have a huge bin in the trunk of my car. I have an assessment first thing in the morning, that is when a patient is coming on service and we have a lot of documents to go over and questions to ask. That will take me probably 1 ½ to 2 hours.

There… a day in my life. I wanted to end by thanking you. Without the community’s support, the VNA couldn’t do the great work we do. Your donations provide the extra items we have to make our job easier and help to make the patient’s life more comfortable. You understand that we are a free standing community based organization. There is no big brother for us, no conglomerate watching over us. You help me take care of Alice, Benny, George, Sally and Rosa. So thank you for all that you do!

VNA Relationships Can Go Beyond The Required Plan Of Care

Dear Loyal Donors:

To each of you thank you from our staff, our volunteers, and especially from our patients who have directly benefited from your loyalty and generosity. I am reflective of all the wonderful people I have encountered and made long term relationships with, in the philanthropic world over the last 18 years in fundraising. This is my 10th anniversary at the VNA & Hospice and I can honestly say it has been a wonderful ten years serving all of you in the community. Across the nation, the 65 and older group will account for 19 percent of the population in 2030, an over 50% increase since 2010. This demographic trend and the focus on reducing health care costs will continue to rapidly increase demand for home care. Monterey County statistics put our senior citizens at 12%. That is over 52,000 people in Monterey County who will potentially avail themselves of the health care industry at some time before they pass.

The VNA & Hospice recognizes this tremendous responsibility and is constantly looking for new ways to ease the burden of illness or end of life issues for our population in Monterey and San Benito Counties. We are the preferred provider of home health and hospice and are the leaders in wellness[at]work programs in businesses as well as our community wellness programs for low income families. Our Adult Day Center allows caregivers, primarily elderly spouses, respite from the 24/7 duty of caring for a frail and/or dementia partner.

I want to thank those who have come before me that established a fund development department so we could provide expanded services and cover sponsored care to our community. I also want to thank my small but mighty staff, Julia Basaldua, our Donor Relations Coordinator for always having my back and keeping the best donor files anyone could ask for in this industry. I look forward to serving you in the future and being a strong steward of your donations. Thank you again.

Katya Kuska, a Medical Social Worker for the VNA Hospice arranged a meeting between our patient, Mr. Noel Van Bibber and naval personnel from the Naval Post Graduate School to witness his testimony of his years of service in the Navy and present him with some gifts of appreciation. Mr. Van Bibber had a wonderful time recounting his military service and at the end of the interview stated, “I am so happy I could cry”. Mr. Van Bibber also talked about the “cap” he was wearing and that it was made by our VNA RN, Anna Linden. He thanked VNA for making this connection for him. The VNA provides compassionate care to all that it can within our financial means as provided by medical reimbursement and charitable donations like yours. To spur charitable donations, the VNA conducts small events executed by its “Fund Development Committee”. These volunteers work hard to put on fun events that you might like to attend to raise monies for care that are not funded any other way. 2015’s events were more successful than 2014 and raised money for the VNA & Hospice projects or programs. Anyone wishing to participate in any of these events, please contact Tina or Julia at 831-648-3734. You can volunteer to help with the event or raise the deeply appreciated funds. The VNA All Star Team at the Walk to End Alzheimer’s. For the second year in a row, the VNA staff, volunteers, and client families from the VNA Adult Day Center ( a day facility in Monterey that cares for frail seniors experiencing dementia issues) was the #1 team out of 96 teams in raising donations from you our donors, friends and relatives . The team raised over $26,000 which was split between the Alzheimer’s Association and the VNA Adult Day Center. Lead by Sharon Gota, (whose mother is an Adult Day Center client) the VNA Adult Day Center received $16,914.60. The monies are used for scholarships for Veteran clients and those families, whose financial circumstances wouldn’t allow for such care. It also provides art supplies, music and poetry classes.

VNA Tea at Parklane. Another year of exquisite vistas, wonderful guests, great hats and beautiful designer tables. The VNA Tea had the best food as well, tea sandwiches from Jeffrey’s Catering, dainty tea cookies from Whole Foods; assorted fresh berries from Driscoll’s; and petit fours from Gail’s Bakery, tea, coffee, and some sparkling cranberry drink to start off the festivities. It was everything you dream about as a little girl. I need to thank all the table designers who made every table magical: Ann Bartoletti (The Steinbeck Best Cellar), Hilary Brewer, Jackie Craghead, Tina Del Piero, Lynda Maguet, Merrilyn Mancini, Darline Nelson, Park Lane, and Ruthie Watts. Thank you to our Tea Sponsors: Visionary Health Care Services, Salinas Valley Memorial Healthcare System, Alliant Insurance, Rabobank, Central Coast Senior Services, Hutchinson & Bloodgood, Louise Oppenheim - Morgan Stanley, Williams Roofing, and Aladin Properties. The fundraiser made $11,888 gross with only $1,599 in expenses for a net profit of $10,289. Again, an increase from 2014. The monies of this tea provided palliative care for our home health patients experiencing advanced disease. All on VNA care are provided compassionate and dignified care because of the donations you provide to your community.

VNA “Dine Around the Peninsula” again was a success making $9,882 gross, $664 in expenses for a net profit of $9,218. We had 12 dinner certificates from local restaurants for first place, 6 dinner certificates for second and a special third place of a stay in a Carmel cottage with dinner in Carmel. How could you go wrong for only a $10 donation for a chance to win one of these great prizes. Operating support for both the home health and the hospice is where we see these monies at work.

Behind the Scenes: Jane Russo

If you’re a local physician, you probably recognize that smile from ear to ear and warm southern hello. Jane Russo is the VNA Hospice Administrator and the Director of Business Development. She has over 15 years of health care experience with a focus on coordination of care to individuals, families and the health care community, emphasizing home health and hospice educational programs.

Jane has a quick wit and advocates for compassion and choice. Among the highlights Jane is passionate about the advancement of palliative and end of life care to the community and is the administrator for the Palliative Care Regional Program working closely with all the providers. She also enjoys a partnership with the United Veteran’s Council to attend to the veterans needs and concerns about their health.

In addition, Jane is active in the community as a Program Chair for the Alzheimer’s Association, dedicated volunteer for the Junior Golf Association, past board member for Meals on Wheels of Salinas Valley and past member of the Monterey Peninsula Junior League.

Jane and her husband, Justin, reside in Monterey with their four-year-old son Justin Jr. and Labrador Retriever, Bella. Her true desire when not working at VNA, is spending time with her family and friends outdoors whether playing golf, bike riding, hiking or on the beach.

Extra’sGary’s Garage Ferrari. Guiding Ferrari’s into parking positions is a hard task but someone has to do it. The members of this local Ferrari club are not only a fun group of individuals with a passion for their cars but extremely generous as well. The club donated $1,400 with no expenses. Again, thank you to the Eisenberg Family, (Gary’s wife, Mona and their son, Paul) and all the club members.
