Patients must be at least 18 years of age and not homebound OR if the patient is under the age of 18 or homebound, have a caregiver register on the patient’s behalf (see reverse for caregiver criteria). Caregivers are required to undergo a criminal background check. Visit medicalmarijuana.pa.gov for a list of physicians registered with the Department of Health to recommend medical marijuana. Patient must suffer from one of 17 qualified serious medical conditions as certified by a health care professional (see reverse). If the physician recommends medical marijuana for the serious condition, the patient will be issued a physician certification. Patients pay a $50 processing fee for the ID card. Individuals participating in Medicaid, PACE/PACENET, CHIP, SNAP, and WIC may qualify for a discounted processing fee. Patients or caregivers are sent their medical marijuana ID card from the PA Department of Health. Patients or caregivers take their medical marijuana ID card to a licensed PA Department of Health-approved medical marijuana dispensary location in Pennsylvania and obtain their medical marijuana. Patients or caregivers must renew their patient certifications after one year. Caregivers: How to Get Medical MarijuanaIf the patient is over 18 years old and homebound, the caregiver must be:

• At least 21 years old and designated by the patient

If the patient is under 18 years of age, the caregiver must be one of the following:

• An individual that is a parent or legal guardian of the patient• An individual designated by a parent or legal guardian• An appropriate individual approved by the Department of Health if no parent or guardian

is appropriate or available

A registered caregiver may be designated by up to five patients and a patient can designate up to two caregivers.

Patients and caregivers can obtain medical marijuana from Solevo Wellness.

Solevo will have its first location at 5600 Forward Avenue, Pittsburgh, PA 15217.Future dispensaries will be opening in Butler County and Washington County.

In Pennsylvania, a patient is able to get medical marijuana if they’ve been diagnosed with one of the following 17 serious medical conditions:

• Amyotrophic Lateral Sclerosis• Autism• Cancer• Crohn’s Disease• Damage to the nervous tissue of the spinal

cord with objective neurological indication of intractable spasticity

• Epilepsy• Glaucoma• HIV/AIDS• Huntington’s Disease• Inflammatory Bowel Disease• Intractable Seizures• Multiple Sclerosis• Neuropathies• Parkinson’s Disease• Post-traumatic Stress Disorder• Severe chronic or intractable pain of neuropathic

origin or severe chronic or intractable pain in which conventional therapeutic intervention and opiate therapy is contraindicated or ineffective

• Sickle Cell Anemia
