PREPARING TO GO HOME Nurse will give you the “Heart Attack and Back” book. Health Care Provider will talk to you about: • the reason you were admitted to hospital. • the care for the next day and how long you may stay in the hospital. • why you need to report chest discomfort. • the importance of not straining during bowel movement. • how to use nitroglycerin. • whether you are currently smoking. • concerns you and your family have. • if you have family to help when you are at home. • professionals who can help you and your family. Other health care professionals may be involved in your care: • Social Worker • Spiritual Care • Home Care • Psychologist May transfer to ward. Health Care Provider will talk about: • your risk factors for heart disease. • for smokers, the need to stop smoking and, if you agree, faxing a Smokers’ Helpline referral. • if you are working and review you need to have a doctor’s letter or medical certificate for benefits while you recover. Social Work may be asked to see you if you have concerns about finances or benefits. Physiotherapist will talk to you about: • why it is important to report angina. • warning signs of a heart attack. • the difference between angina and heart attack. • when and how to go to Emergency. • reinforce how to use nitroglycerin. May transfer to ward. Dietitian will talk to you about: • foods to eat and avoid and where to obtain further diet information. Physiotherapist will talk to you about: • how to use nitroglycerin. • the Home Exercise Program. • warning signs of what to watch for when you increase your activity. • gradual return to daily activities at home. • return to work. • sexual activity. • driving restrictions. • leisure activities. • Stress management Nurse will talk to you about: • emotional health. • show the video “After Your Heart Attack, How Are You Doing?”. The nursing staff will: • give you medication prescriptions. • review tests you are to have. • tell you when to see your doctor. Pharmacist or Nurse will talk to you about: • how to take your medications. • what to do if you miss a medication dose. Physiotherapist will talk to you about: • how you manage going up and down the stairs. • the role of the Cardiac Rehab Program in getting further information to stay heart healthy. • the Next Steps Heart Fair.
