Goal: Integrate actionable data into the provider’s EHR systems and workflow. New Measures: Readmission Rates & Medication Reconciliation Provider Feedback & Demand: Visibility when their patients utilize the hospital or the emergency room. Connectivity: Near real-time ADT messages from 46 of the 90 hospitals in Alabama, representing approximately 73% of the short-term acute care hospital beds. Member/PCP Attribution: Established processes to identify the appropriate provider-to-member healthcare relationship. Actionable Message

ALERT: A patient attributed to you was discharged from the hospital. Remember to contact this patient within 2 days of discharge to schedule a follow-up appointment and to reconcile medication no later than 30 days post discharge. Once discharge medications are reconciled with the patient’s current medication list, document it in your electronic medical record system and submit CPT Category II code 1111F on a claim to confirm that medications have been documented and reconciled.
