DATA COLLECTION PROCESS

• All data will be submitted electronically

• A paper tracking document will be provided

• Data must be submitted at least weekly for H/M/L and

encourage you enter weekly for the other categories

• Time in 5-minute increments, zero if no time spent

• Data will be reviewed weekly by DOC
