Excellence is Our Standard! Welcome Students!

Fonville Students, It is my pleasure to welcome you to Fonville Middle School for the 2012-2013 school year! I am excited to see the number of new students coming to Fonville. I am also happy to see so many students returning to Fonville for another year. It is my goal to make sure everyone has the opportunity to be successful. I look forward to seeing all of you move on to high school, ready to succeed at whatever you want to accomplish! During your time here at Fonville Middle School, I expect you to “Be Respectful, Be Responsible and Be Ready!” for the challenges you will face in middle school and for the rest of your life. Remember that our motto is, “Excellence is Our Standard!” and I expect you to strive for excellence in everything you do. During the past summer, lots of construction and renovation work was completed at Fonville Middle School. You will be happy to see the new classrooms, restrooms, gym floor, bleachers, windows, air conditioners, white boards and newly painted walls. We also have new lockers and individual showers in the locker rooms. I want you to enjoy all of the new and renovated areas, but remember that I expect you to take excellent care of the school. We must not disrespect ourselves or others by damaging our beautiful school. I have high expectations of you while in class. Being ready for school means that you are following the dress code, you are on time and you have all of your supplies with you at school. You are also expected to wear your ID badge for safety and for eating lunch in the cafeteria. Parents, please make sure your students arrive to school on time and ready for success every day. The theme for 2012-2013 is: “Dive Deep into Instruction!” By working together and striving for excellence, we will succeed! This year will be an adventure and I am glad to have you as a student at Fonville Middle School. I have high expectations, but I am willing to support you in reaching your goals. Thanks again for being here and joining our journey to “Dive Deep into Instruction!” I challenge you to find a way to contribute and be active on campus, whether through band, sports, karate, after school activities or academic clubs. The Advocacy period is dedicated to the development of interpersonal and organizational skills. The advocacy teachers support the academic goals of the students and are the liaison between the students and their parents. During Advocacy period, all Fonville students are eligible for Breakfast in the classroom free of charge. Beginning at 7:30 a.m. all students must enter the building through the cafeteria. After school interventions and school wide detention, will be held from 3:30 p.m. to 6:00 p.m.

The faculty and staff at Fonville are not responsible for the supervision of students who arrive before 7:30 a.m. or stay after 3:30 p.m. unless the student is participating in an approved extracurricular program sponsored by an adult. Parents will be notified of all tutorial and detention assignments, including the assignment of Saturday school. Parents, please contact the main office at (713) 696-2825, if you have questions about these assignments. Tutorials and detention assignments are mandatory. Failure to attend an assigned detention or tutorial will result in disciplinary action and the consequences to be determined by the grade level administrator.

Promotion Standards In order to be promoted to the next grade, students must have:

a current course average requirement of 70 or above in Language Arts (average of reading and English) and Mathematics, and either Science or Social studies.

Meet the minimum attendance standards set by the Texas Education Agency.

Please see www.tea.state.tx.us for further information. Note: Students with 10 or more excused or unexcused absences will be retained and the student must attend summer. Student Recognitions and Award Ceremonies Students may only participate in recognition and award ceremonies if they have a current permission slip signed by parents on file. Recognition and awards may be presented to students for: Honor Roll, Post-High School Scores on Stanford, Perfect Attendance, History and Science Fair, participation in extracurricular activities, school programs, and other school functions and/or organizations. SUPPLEMENTARY PROGRAMS All students qualify to participate in Title I intervention programs during and after school, on weekends and during the summer. To meet the needs of accelerated students, the school provides enrichment activities that are more rigorous, interesting and challenging in a comprehensive Gifted and Talented program at all grades. Students identified as Gifted and Talented are served by using a curriculum, which has greater depth and complexity than the regular school programs. Services within the Special Education Program are available for students who qualify, including Self-Contained Life Skills, Multiple-Impaired, Behavior Adaptive Centers (BAC), Support classes. Qualifying students participate in English as Second Language (ESL) classes in beginning, intermediate, advanced and transitional levels. The 21st Century After-School Program offers a number of exciting after school programs for students. Participation in the 21st Century program is considered a privilege and requires the submission and approval of an application agreement. Students must follow all the rules in the student handbook and the district’s code of conduct. 21st Century Students must report directly to their assigned class. Students who leave campus are not allowed to return without parents’ supervision or the principal’s permission.

Clubs and Student Organizations A variety of student activities are available for students. A student must be in attendance at school on the day of the extra-curricular activity. A student is eligible to participate if he or she is passing all subjects on the six weeks report card. If a student fails any subject, he or she becomes ineligible for at least three weeks. Counseling Services HISD School Counselors and Social Workers are available to assist students in need of services. The counseling department may provide support services in the event of crisis or difficulties in academic, personal or family issues. (See Mrs. Barbara Cravin or Ms. Kelly Musick) Parent Seminars and Workshops Fonville Middle School coordinates seminars and workshops on various topics of interest to the parents. Coffee with the principal will be held on the 1st Thursday of each month during the school year at 8:30 a.m. Business and Community Involvement Fonville Middle School actively recruits business partners and the community at large to get involved in the educational process of its students. The parent volunteers, businesses and the community members all contribute time, talent, and resources to improve student achievement. ATTENDANCE Attendance Guidelines Instruction begins promptly at 8:00 a.m. Students who arrive after 8:00 a.m. are considered tardy, but remain in the classroom. Excessive tardies will result in disciplinary action. Official school attendance is taken daily at 9:45 a.m. Students must be in attendance at this time to be counted present for the day. Students may not have more than four unexcused absences per 18-week semester to receive class credit. TEC Sec. 25.085 states that “A child shall attend school each day for the entire period the program of instruction is provided.”

A school day shall be at least seven hours each day, including intermissions and recesses (TEC Sec. 25.082). A child shall not have unexcused absences of 10 or more days or parts of days within a six month period or 3 or more days or parts of days within a four-week period from school (Family Code Sec. 51.03). Failure to comply with these laws will result in legal prosecution. Absence from school When a student is absent from school, parents must submit a written excuse to the attendance clerk stating the reason for the absence within three days of the last absent day. After three days, the absence becomes unexcused. Excused absences are only allowed for the following:

Illness or death in the student’s immediate family,

Participation in legitimate school activities with the permission of the principal

Weather or road conditions making travel dangerous

Emergencies or extenuating circumstances as recognized by the principal or administrator.

Make-up work Students are responsible to make up all missed work during their absence(s) within three days. For extended absences from school, parents and teachers must make special arrangements for make-up work. The school can help in getting assignments from teachers. Parents should call a day in advance to make these arrangements. Tardiness to class A student is counted tardy if he/she is not in the classroom when the tardy bell rings. The teacher keeps an accurate record of tardiness. Tardiness may result in disciplinary action. Leaving School during the Day Parents or guardians that desire to pick up students during the school day must report to the school office, with photo identification, to sign out the student. No child will be released to any person other than the parent or guardian unless the school receives a written request from the parent, which can be verified, and the person picking up the child can provide proper identification. Parents are encouraged not to pick up students after 2:45 p.m.

Withdrawals Parents should inform the attendance office in advance of their plans to withdraw a tudent. Only a parent or legal guardian may complete the withdrawal process. All withdrawing students must submit an “Intent to Enroll” form to the attendance office upon withdrawal. Contact Information Parents must notify Fonville Middle School of any changes to their contact/emergency information. HEALTH SERVICES School personnel are available to address health or emergency issues. Students must adhere to the following procedures: A written permit to the grade level office from the teacher of the class from which a student is reporting is required. When admitted to the grade level office, each student signs in on the clinic log and then remains in the grade level office until he/she receives further instructions. Students who are ill can only be released to go home by the school personnel designated by the principal. If a student calls someone to pick them up without following procedure and going through the designated school personnel, the absence is unexcused. No child will be released to any person other than the parent or guardian unless the school receives a written request from the parent, which can be verified, and the person picking up the child can provide proper identification. The school personnel must approve requests for a temporary or long-term physical education activity restriction. Grade level offices have first-aid kits to tend to minor cuts and scrapes. If the grade level office is unavailable, students should report to the main office for instructions. The administration of medications to students at school is governed by district policy. Students may not bring over the counter medications to school. Students on prescription medication must have the “Physicians Request for Administration of Medicine” form completed and on file in the clinic. Medication must be brought to school in the correctly labeled prescription bottle. Pharmacies will issue duplicate bottles for school use. Medications must be administered in the grade level office under the supervision of designated school personnel, and a record is kept of each dose on the medication sheet. The only exception to this is self-administration of inhalers by asthmatic students. Parents must submit an authorization form to the school clinic.

Student immunizations must be up to date in order to be enrolled at Fonville Middle School. If a student is found not to have his/her immunizations up to date, they will be released from school pending updated immunizations. STUDENT TRANSPORTATION The district provides bus transportation for those students who live farther than two miles in Fonville Middle School’s zone. Physically challenged students are also eligible for bus transportation. Students are required to have an approved bus form on file every year before they are eligible for bus transportation. Eligible students are issued a list of regulations. Bus drivers report any violations of these regulations to the principal and repeated violations may lead to temporary or permanent ineligibility for bus transportation. Parents may arrange for private transportation services. Fonville Middle School is not responsible for any issues that arise between private transportation companies. Students using private transportation services are responsible to arrive to school on time everyday and to be picked up promptly after school, and are required to arrange transportation when attending after school or Saturday interventions. Private transportation companies may only pick up students from the bus ramp on the south side of the campus facing East Little York. Students are not permitted to drive to school. PHYSICAL EDUCATION CLASSES All students must participate in the physical education classes. Physical Education uniforms are required and are available through the P.E. Department. P.E. uniforms are available for rental for a daily fee. P.E. uniform shirts and shorts are 50 cents per article of clothing each day. (Total fee: $1.00) Students are not allowed to wear jewelry, chew gun, drink or eat in the gym, locker room or hall area. Physical Education teachers will assign lockers to their gym classes. Students will need a combination lock for gym lockers (no key locks). Students are not allowed to share lockers. The school is not responsible for lost or stolen items stored during gym classes.

BREAKFAST/LUNCH PROGRAM Meal Options Breakfast will be served during Advocacy period beginning at 8:00 a.m. There is no cost for the student breakfast meal. The cost of lunch is $1.85 for students and $3.00 for adults. Reduced price for lunch is $.40. Free and reduced lunch applications are available from the main office. Every student is required to submit an application regardless of qualification. (One application per family) All lunch applications must be approved before a student is eligible for free or reduced lunch. Cafeteria Rules Lunch periods are assigned. In order to maintain a safe, clean and pleasant environment in the school cafeteria, it is necessary to follow the following rules: Students are to report directly to the cafeteria after the previous class. No running is allowed when reporting to or leaving the cafeteria. While in the cafeteria students must remain seated unless waiting in line or disposing of trash. Students waiting in line for food or snacks purchases must wait in a single line, and no line cutting is tolerated. Students cannot make purchases for anyone else. Students will go through the serving line only once. Each class and/or student may have assigned seating per administrator or teacher discretion. Cafeteria monitors will dismiss students by rows or tables. When students have finished eating, they are to place all trash and uneaten food in the appropriate containers. Birthday or other holiday celebrations are NOT held at school. No cakes, cupcakes or other snacks are to be brought and served at school as part of a birthday celebration, and students cannot order food to be delivered to the school. Parents may eat lunch with their children and will have an assigned place in the cafeteria. A parent may only bring food for his/her own child. All other general conduct rules that apply throughout the campus apply to student behavior in the cafeteria.

TEXTBOOKS Students have the responsibility to take care of their textbooks, whether at school or at home. Parents may check-out textbooks for use at home during the school year. Textbooks must be returned in good condition. If a book is lost or damaged, the student is financially responsible for the full price of the book. If the bar code is damaged, the student is responsible for half the cost of the book. BEHAVIOR EXPECTATIONS Safety and Security

The safety and security of students, visitors, faculty and staff is the number one priority at Fonville Middle School. Collaboration between the school, parents, and community is essential to provide an orderly, safe and secure school.

Fonville Middle school uses a combination of surveillance cameras, metal detectors, a police officer and a security guard to improve the safety of students, teachers, and school administrators. All persons who are on campus are subject to be searched by an administrator or police officer. However, students must adhere to the following guidelines.

Coming Directly to School – All students are to report to the cafeteria as soon as they arrive at school. Walking to School – Students are urged to exercise caution when walking to school, particularly while crossing East Little York. Please use the crosswalks at all times when crossing the street. No Parking Areas – School’s patrons and parents who visit the school or who provide transportation to students must not stop to drop off students on East Little York. Parents are urged to observe the “NO PARKING” areas next to the building and along curbs. When waiting to pick up a student, please do not block the driveway. DOUBLE PARKING IS NOT ALLOWED. Drop Off and Pick Up – The student drop off and pick up zone has been designated to the east entrance on Van Ness Street. Parents can drop off and pick up students by entering through Van Ness Street and exiting East Little York.

Buses – Houston ISD and private buses are required to use the student drop off area located on East Little York. Leaving School – No student is to leave the school at any time after arrival during the school day unless excused through the office. Students arriving early, before school begins are not

permitted to leave the school grounds to go across the street to any store or for any other reason once he/she has arrived on campus. Staying After School – Parents will be notified when a student is required to stay after school. Teachers or administrators will assign detention before or after school for tardiness or other infractions according to the Discipline Management Plan. Students who are not participating in detention, interventions or any other school-sponsored activity are not permitted to remain in school grounds after dismissal. Clearing Building After School – After school, students must exit the building within five (5) minutes of the last bell unless detained by a teacher or staff member. Upon dismissal, car riders are to go to the front of the building. Students who ride buses are to report directly to the west driveway where they board their buses. Students who walk home should leave campus immediately after dismissal. Supervision of Students Before and After School Hours – Faculty and staff are not responsible for the supervision of students who arrive on campus before 7:30 a.m. or who remain after 3:35 p.m. unless they are involved in a specific activity under the direct supervision of a teacher or sponsor.

2012-2013 Fonville Middle School - Standardized Dress Code Policy for Students

Fonville Middle School is charged with the responsibility of maintaining an atmosphere conducive to learning. Dress should never distract from school activities or prove a hazard to the student’s safety or the safety of others. Since it is impractical to list every possibility of dress and grooming, final decisions concerning acceptable dress are left to a building administrator. ID Badges In order to help ensure a safe learning environment for students and staff, all students are required to wear ID badges. Students will adhere to the following rules: Students who lose their badge must pay $ 5.00 for a replacement badge and $ 1.00 for lanyards. Students who forget their badge at home must purchase a temporary badge for $1.00 Students in violation of the school ID Badge policy may face disciplinary action. ID badges will be used to purchase lunches and for entrance into special events. Electronic Devices Under Texas School Law Code 21.309, students at school or school related activities are prohibited from the use of communication devices. Students are allowed to have cellular phones / paging devices in their possession; however cellular phones/paging devices must be concealed and turned off while on campus. These devices may only be used by students in the event of an emergency (school will define emergency). Students may not bring electronic devices (MP3 players, IPods, Gameboys, etc) to school. All electronic devices are subject to confiscation. The school will not reimburse the student for items lost or misplaced. The unauthorized use of electronic devices will result in confiscation and a $15.00 fee will be assessed. All electronic devices not claimed by the end of the school year will be sent to HISD for disposal. Fonville Middle School will not conduct any investigations for lost or stolen electronic devices. Use of Office Telephones – Office telephones are restricted from student use during school hours; therefore, the students will not be allowed to use the telephone to arrange for transportation. Students and parents must make arrangements beforehand. If there is an emergency, school personnel will contact parents or give the student permission to use the office telephone. Arranging for transportation is not an emergency. Any student found tampering with the fire alarm or communication system in a classroom may be subject to appropriate punishment and/or prosecution. Signaling a false alarm is a crime and is punishable as such.

Student Conduct

We expect appropriate behavior at all times while students are on school property and during all school-sponsored activities. Our campus-level discipline management plan is in accordance with the HISD Code of Student Conduct.

HISD Board Policy also provides an action plan of procedures to deal with other major offenses and with chronic, persistent and habitual misconduct of any kind, which interfere with the educational process. Policies apply during school hours, before and after school, while on school property and at all school sponsored events. Students and parents should understand that in addition to taking disciplinary action at the school level, illegal acts must be reported to HISD Police and to the appropriate law enforcement authorities. Students and parents should refer to the HISD Student Code of Conduct for specific behavior and discipline guidelines.

Overview: Fonville Middle School - Discipline Management Plan

A Discipline Management Committee composed of parents, teachers and administrators, put together the Discipline Management Plan for Fonville Middle School. Parents and school authorities are working together to ensure that students receive the best education possible in a safe environment. Parents must ensure that students arrive to school on time and have all the required materials and textbooks. Students are responsible to get to all of their classes on time and perform to their best ability. Parents may be contacted by the teacher, administrator or other staff member if a student breaks school or classroom rules or policies. Students and their parents must review the Student Code of Conduct received at the beginning of the year. When rules or policies are broken, students should expect an appropriate consequence as determined by the Houston Independent School District. Misbehavior and disruptions to instruction will not be tolerated. Each teacher or staff member may implement particular rules and regulations for better management of each class or school related activity. The teacher or responsible staff member will inform students of his or her expectations for behavior, participation, and work ethics. In addition to individual teacher rules, all Fonville students are expected to come to class on time, comply with all teacher directives, complete class assignments and homework, and come prepared for class and bring all required textbooks, materials and supplies. Teachers or staff members may implement the following consequences to maintain classroom discipline: Oral reprimand or correction Assigned seating Restriction of privileges Student conference Teacher assigned detention Extra assignments or duties Parent conference Referral to an administrator

Pursuant a discipline referral, administrators will consider the offense, the circumstances in which it took place, previous incidents, and the seriousness of the incident. Disciplinary consequences may include the following: Parent conference Administrative detention Extra assignment or duties Parent in school for one or several days Out-of-school suspension Citation and fine Placement in an alternative school Expulsion Report to local law enforcement/Arrest Student Infractions for Detention, Suspension and Removal Detention will be held daily after school from 3:35 PM-6:00 PM. Students who do not attend detention may be suspended at the discretion of the administrator. Students will receive administrative detention for a variety of reasons including, but not limited to, the following infractions: dress code, failing to wear ID badges, tardy to class, failure to carry binder/supplies, disruption of class, etc. Administrators may assign students out of school suspension as established in HISD’s Student Code of Conduct. Some offenses are considered so serious that the student code of conduct requires that the student is placed in an alternative setting (such as C.E.P.) or expelled from HISD. Students and parents should review the Student Code of Conduct to be aware of the types of offenses that result in alternative placement or expulsion. Fonville’s Discipline Management Plan was designed with students in mind. By following these general discipline guidelines, our administrators, teachers and staff wish for all students to be safe and take full advantage of the wonderful educational and extracurricular programs at Fonville.
