A Letter from our ChAirmAnThe Harris County Hospital Foundation is proud to continue its legacy and essential role of support for the programs that improve the lives of those served by the Harris Health System. In 2013, the Foundation was focused more than ever on enhancing existing programs and making inroads with pilot programs that led to innovations in healthcare delivery and improvement in healthcare of our community.

The 2013 annual report highlights some of the Foundation’s recent accomplishments, recognizes our generous partners and supporters, and provides insight into the future of our organization.

We have goals and more than ever, the excitement to grow within our mission.

The Foundation’s success is in large part due to the incredible community support of the businesses, donors, and grants to the Foundation. These philanthropic partners have helped our Foundation grow and ultimately, assisted Harris Health System to meet the needs of our population.

Some of the 2013 event highlights include our Jubilee of Caring and the presentation of the Ben Taub Humanitarian Award to Sydney and Don Faust. Another fun event, Cinco de Bingo, produced by SCRUBS—the young professional group—grew in popularity again this past year and supported the New Beginnings Program at Harris Health’s hospitals. The 2013 Silver Tea provided funds to support Nursing Education and Development. “Day in the District” provided staff, board members and the community of new supporters an up-close unique day with Harris Health operations. The 12th Annual Texas Med Fun Run event supported nearly 7,000 immunizations for children and the elderly through the Troubleshooters Immunization Program.

Our Board of Trustees has been hard at work as well. They have provided many hours of volunteer time and expertise in several events and programs this year. We thank them for their continued dedication, hard work, and the leadership they provide to the Foundation.

The Foundation is committed to working with Harris Health System to achieve better health for the residents of Harris County, Texas. We continue, with Harris Health, to focus on supporting community health outreach programs, improve patient education, and strengthen the skills of health care providers within the Harris Health System through professional development.

Additionally, the Foundation will continue to work to educate stakeholders, ambassadors, community partners and the public about the needs and public health strategies in Harris County. Our success will continue to be measured by community support and funding and by the programs and services we support with Harris Health System for the uninsured and the underinsured of Harris County. Please consider donating time and funds to support our community—each of us can play an essential role.

IN MEMORIAM: H. BEN TAUB — LEADER AND PHILANTHROPISTThe HCHD Foundation lost a great leader and supporter in the passing of H. Ben Taub. He served as Chairman of the Board of the Harris County Hospital District Foundation for 12 years, among his service to many other Houston-based charitable organizations and institutions. Known as a great businessman, he led the Foundation with his generous spirit and his passion for causes that supported children and medicine. He will be greatly missed, yet his impact will live on in the hearts of those who knew him and the lives he helped to impact through his service to this organization and Harris Health System.

pubLiC-privAte synergy

Our essential role in increasing positive health outcomes is the result of a long-term relationship between private funders and public institutions. Loretta Hanser, the Harris Health Director of Imaging Services working side by side with the HCHD Foundation, brought together the infrastructure and expertise to make things happen.

Hanser has built strong and successful partnerships with the ACS, Komen, Pink Ribbons and Avon. Private and public grants funded projects and programs grew the delivery of screening and diagnostic mammograms from 5,000 in 2000 to over 40,000 in 2013.

At the same time the team has lowered patient appointment no-show rates from a high of 40% to less than 10%. Hanser and her team have developed some of the Best Practices in the delivery of patient centered services, increasing health literacy as well with compassion and professionalism.

People may forget what we tell them but they never forget how we make them feel. One such grateful patient sent a holiday card to Ester Martinez, Harris Health Breast Cancer Navigator: “deAr esther, LAst yeAr you Were kind enough to Assist me…i WouLd Like to thAnk you sinCereLy for your CompAssion And thoughtfuLness…in the 13 yeArs i hAve been Living in houston, i hAve never met A more Attentive And dediCAted person…i simpLy CAnnot express my grAtitude And AppreCiAtion for your efforts.”

Hanser’s team is now replicating their success with breast cancer outcomes for colorectal cancer screenings services. “We are very proud of what we have achieved so far but are only screening about 41% of the eligible Harris Health patient population. Increasing the number screened will require the assurance of follow-up diagnostic procedures and surgical services. Resources need to be expanded in many areas, not just radiology but surgical and oncology too,” said Hanser.

Diagnostic imaging services and stereotactic biopsy services are also provided at Smith Clinic and LBJ Hospital. All mammography imaging services in the Harris Health System are 100% digital. Imaging studies are available for viewing anywhere in the system within a few minutes after the completion of the study.

our essentiAL roLeWhile the primary focus of the Harris County Hospital District (HCHD) Foundation is to raise funds for Harris Health System, the Foundation also serves as a steward of a permanent independent endowment devoted to children’s health issues in Harris County. In 2002, HCHD Foundation began managing the Children’s Health Fund, which supports integrated health projects for children who are not eligible for other similar services. In partnership with Harris County and the Harris Health System, the HCHD Foundation awards grants from investment proceeds of the Children’s Health Fund. Some of these organizations include Legacy, Spring Branch, Good Neighbor, Vecino Health System, El Centro del Corazón, Asian American Health Coalition, and Central Care. The grants provided to these agencies assist in serving children in Harris County who are at the highest risk of not receiving even the most basic health care.

Since 2002, the Children’s Health Fund has given 80 grants for a total of $3,164,697 in grant awards; 40 of those grants have been given to collaborative projects between the Harris Health System and its affiliated medical providers—Baylor College of Medicine and UT Medical School, six grants have been awarded to Harris County public institutions, and 34 grants have been given to non-profit organizations in Harris County.

These grants have served in many ways: - A new vehicle for the Harris Health System outreach immunization program was purchased, - Harris County Protective Services engaged 300 parents with children with special needs and distributed 12,000 Natural Support

Resource Guides in the community, - Vecinos System-Denver Harbor Clinic served 24 families through an intensive Parent-Child Interactive Therapy Program, - ECHOS built a collaborative with 11 organizations to address the needs of 100 immigrant children living in non-parental care, - Family Services of Greater Houston provided counseling services to 183 families in the HISD South Region, - Healing Species worked with 15 children victims of human trafficking and engaged 705 HISD students in their violence prevention program, - Lamar High School provided case management services to 70 students with behavioral and mental health issues, - HACS delivered mental health services to 40 homeless youths living at Covenant House, - HGI partnered with two HISD elementary schools to provide counseling services to 321 children and parents, - 213 children and mothers living at Santa Maria Hostel participated in the Fitness and Nutrition Connection program.

fostering innovAtionThe HCHD Foundation plays an essential role in fostering innovation within the Harris Health System. Harris Health staff has a wealth of knowledge and ideas enhancing the quality of services to Harris County residents. The HCHD Foundation’s Small Grants Fund provides seed money for pilot projects benefiting patients or staff.

in 2013 thAnks to these smALL grAnts: • 109 nursing staff received training on the guidelines for communicating with unresponsive/non-verbal patients

and 72 patients benefited from this service enhancement. • 114 families in schools linked to our school-based clinics participated in project activities to learn how to make

healthy food choices while managing a limited budget. • 110 nurses and 75 patients attended the Cardiovascular Symposium. • 86 patients receiving treatment at the LBJ Hospital Infusion Center received a fanny pack to store

treatment supplies. • 4 LVNs received new certifications and 10 more are in the process of taking certification exams. • 129 patients being treated at the Dialysis Center received blood pressure machines. • Two Daisy Award ceremonies took place where 12 outstanding Harris Health nurses were recognized. • 673 patients being treated at Thomas St. Health Center received incentives for complying with wellness exams.

AdditionALLy ten neW smALL grAnts Were AWArded: • Emergency room nursing staff are receiving a specialized orientation. • A health literacy program is being implemented at selected school-based clinic sites. • Nursing staff at the Harris Health Ben Taub Hospital are receiving workshops on communication skills. • The Harris Health Community Outreach Services Department is implementing a health literacy program for

families. • Patients with asymptomatic hypertension treated at the Harris Health LBJ Hospital are receiving a follow up

card with further instructions to prevent recurrent ER visits. • The Psychiatric Unit at the Harris Health Ben Taub Hospital is implementing a support group for low-income

adolescents with a psychiatric diagnosis. • The Harris Health Pasadena Pediatric and Adolescent Health Center is delivering a weight management

project for adolescents to increase physical activity. • Harris Health Clinical Case Managers ongoing professional development was enhanced thanks to a new

interactive tool purchased with a small grant. • Services given at the Northwest Health Center: Nutrition class supplies for HIV positive pregnant patients

were enhanced. • Harris Health Administration/Think Innovation initiative also received a small grant to provide incentives to

Harris Health staff implementing projects to enhance the quality of services in the Harris Health System.

Investing in the nurses who serve Harris Health patients pays considerable dividends. Not only does it elevate the advanced level of care nurses are able to provide, but their professional development also contributes to the efficiency and effectiveness of care patients receive at Harris Health. A primary focus for Harris Health’s Nursing Leadership is to continue to create opportunities for advanced education to those nurses who serve Harris Health patients. The Harris County Hospital District Foundation assists in this endeavor through its annual Silver Tea special event. Funds raised each year help to provide stipends for books, fees, and time-off pay to full-time nursing service employees who are in the process of pursuing advance nursing degrees. Supporting nursing education will also help to advance Harris Health System in its goal to achieve Magnet status—an elite independent designation recognizing exceptional nursing and nursing leadership.

The Nursing Research Council is a key element to elevating nurse skill level and the quality of patient care at Harris Health. The Council supports nurse-led research that generates scientific evidence to provide high-quality health and healing services to all patients receiving care at Harris Health System. Nursing Leadership funding provided by the Harris County Hospital District Foundation has sponsored mentorship, education, and hands-on learning for Harris Health nurses to learn about evidence-based practice and nursing research in clinical practice. Nurses also received support to build their professional portfolios by conducting research projects and sharing that information through internal, local, and national presentations and publications. Looking forward, the Foundation will continue to its support for these educational initiatives that are of prime importance to the delivery of excellent patient care at Harris Health System.

heALthy hArvest progrAmOur essential role in providing these small investments have a domino effect that leads to improving system-wide efficiencies, as well as increased patient and staff satisfaction. Healthy Harvest, a program that coordinates farmers markets in healthcare centers to encourage patients to eat more fruits and vegetables, is one small investment that has had a far-reaching impact. The project was envisioned by Dr. Ann Smith-Barnes, Harris Health Medical Director of Weight Management Services and Disease Prevention.

HCHD Foundation seed money provided the support for Healthy Harvest to come to life and as a result, the program received the National Association of Public Hospitals and Health Systems (NAPH) Gage Award for improving Population Health. Healthy Harvest also received a $60,000 grant award that was instrumental in sustaining the program through 2013.

Another example lives within the walls of the Horticultural Therapy Garden at Quentin Mease Community Hospital. This innovative garden was built with seed monies procured by the HCHD Foundation and the overwhelming success of this project triggered a chain of events resulting in a grant award from the Stanford and Joan Alexander Foundation. Construction of a healing garden at the Ben Taub General Hospital was made possible with the funds received from the Alexander Foundation grant.

hArris heALth systemmobiLe heALthThe Trouble Shooters Mobile Immunization Program is another way that Harris Health reaches out to serve the community. Trouble Shooters operates by going to more than 400 local sites such as schools, non-profit and faith-based organization to provide immunizations to both children and the elderly in need. HCHD Foundation supports this program with funds raised at its annual Texas Med Run/Walk. Recently, the Foundation was able to provide replacement funds for a new Trouble Shooters mobile van when the existing vehicle was no longer operational. Trouble Shooters operates primarily at sites within the east corridor in areas experiencing the lowest health indicators in Harris County. These vaccines benefit both the people who receive them and the vulnerable, unvaccinated people around them by preventing the spread of infection. Immunizations reduce the number of deaths and disability from infections such as Whooping Cough, Influenza, and Chicken Pox.

Healthcare for the Homeless (HCH) of Harris Health System operates the Mobile Dental Program in partnership with Houston and Harris County homeless shelter providers. It provides dental care directly where homeless patients are located and where no permanent site Harris Health clinic dental services are available. HCH Mobile Dental Outreach Program overcomes access barriers bringing oral healthcare to men, women, and children who would otherwise go without. Mobile Dentist Dr. Matthew Plummer, Jr. found that adult patients who are served by the mobile dental unit often give up dental services for themselves so their children can receive care. Many, including pregnant women, have not seen a dentist for 20 years. Some adults have never seen a dentist in their lifetime. The HCH Mobile Dental Program provides oral health to 3,500 adults (annual average) at 12 homeless shelter locations annually. In 2014, HCHD Foundation began raising funds for future replacement of the more than 10-year-old HCH Mobile Dental vehicle.

MoBile Medical accoMplishMenTs

Trouble Shooters Program operates 11.5 months of the year, providing over 15,000 vaccines (average) to nearly 7,000 children (average) annually at 400 mobile operating sites to date.

In 2014, HCHD Foundation supported the “Shattered Dreams” program through generous funding awards obtained from the Fred and Mable Parks Foundation and the Houston Livestock Show and Rodeo. Traffic accidents are the leading cause of death and serious injury for youth 15 to 19 years of age. With hopes to educate young drivers, “Shattered Dreams” provides a live trauma injury prevention and educational dramatization of an alcohol-related automobile crash scene on or near a high school campus.

This effort is a collaboration between the HCHD Foundation, Harris Health System’s Trauma Services at Ben Taub General Hospital, local law enforcement, fire fighters, emergency medical services, and local funeral homes.

A painfully honest performance is presented to students, faculty and parents who experience the scene as it unfolds. Included in the live action scene are court hearings, family notifications, and arrest and booking of a drunk driver who has caused a fatal car accident. Harris Health’s “Shattered Dreams” program has been performed in front of live high school audiences since 2006. More than 40,000 students have participated in this interactive program. Educational videos produced from the live reenactment are also presented to youth in additional Harris Health System Trauma Services Outreach Programs at juvenile probation units, local churches and health fairs reaching thousands more teens and adults.

12th AnnuAL CirCLe of survivAL trAumA ConferenCeHarris Health System’s nationally renowned Ben Taub Hospital Ginni & Richard Mithoff Level One Trauma Center and LBJ Hospital Level Three Trauma Center provide education and training to the next generation of healthcare professionals as part of its ongoing mission and commitment to Harris County residents. Ben Taub Trauma Center delivers 89,000 emergency patient visits each year including 3,000 trauma cases. LBJ Trauma Center delivers 73,000 visits each year including 1,800 trauma cases.

The Circle of Survival Trauma Conference, now in its 12th year, provided additional access for over 300 attendees to cutting edge information on the management and treatment of trauma injuries in 2014. This successful collaboration partners Harris Health Trauma Services leadership at Ben Taub and LBJ hospitals with Harris County Hospital District Foundation administrative staff support.

The 2014 Trauma Conference provided Registered Nurses and other healthcare professionals with updates and new information in the areas of: trauma system development, management of shotgun wounds, treatment of snake bites, caring for trauma brain injuries and acute spinal cord injuries. Attendees will incorporate this wealth of knowledge in their own clinical practices ensuring the most appropriate and timely care for patients who suffer traumatic injuries.
