The Patient Log

Instructions on this page are self-explanatory. Please do not register twice. If you are unable to get into the log after you have registered, DO NOT REGISTER A SECOND TIME. Contact Dr. Alvita Nathaniel at anathaniel@hsc.wvu.edu.

Complete your user profile using upper case and lower case letters. You may change or update your profile at any time in the future.

You do not need to change your password when you fill out the profile.

For purposes of your course, you really only have two choices:

1. To create a clinical log

2. To view the reports generated from your past clinical logs

The major function of the log is to maintain a record of your clinical experiences. When you are ready to record your clinical time, click “Clinical Log.”

1. Indicate the date of your clinical experience.

3. Choose the site of your clinical experience.

2. Choose your preceptor from the pull-down box

4. Indicate the number of hours you spent at this site with this preceptor.

5. Click “Add Log”

Important: You add more than one log on a single day, but either your site or preceptor must be different. So, you must include all hours that you spent in one site with the same preceptor in only one log. If you changed sites or changed preceptors in the same day, you can make more than one entry.

If you choose a preceptor and site and receive a message like this…….Contact anathaniel@hsc.wvu.edu. For the log to function properly, each preceptor must be connected within the database to the correct site. It is quick and easy to make these connections.

To begin the log, click “Activities”

Type your reflection in this box.

Type a short title or subject in this box

Choose either Advanced Practice or Organizational Aggregate focus from the pull-down box.

Click “Add Activity’

Choose an essential from the drop-down box that is appropriate for the activity, then click “Add Essential”

You may add as many essentials as are appropriate.

IMPORTANT: After you’ve added the essentials, if you decide that you need to write more in the Reflection box, you must click Update or your work will be lost when you move to another page or choose another essential

Once the activity is recorded, if you click “Activities” again……… (see next slide)

You now have an opportunity to add another activity to the same day with the same preceptor. You can add as many activities as needed. Each activity is recorded with its corresponding focus and essentials

For a report of all of your clinical experiences, click Log Reporting.

“Log reporting” is the site that faculty will use to monitor your clinical experiences.

Your log report will look like this, though it will probably make more sense than this one.

To see your report, click “Run Log Report”

To see a summary report, click “View Summary Report”

This is a mock-up summary report of three different clinical days with three different preceptors. You can see that this student’s experiences touched on several essentials, some more than others. You can also see that the student completed a total of 35 hours. This report is cumulative. At the end of your program, you will see all of the hours you’ve completed using this log system.

The administrator made a mistake and deleted the clinical site. We hope this won’t happen in real life!

This form becomes a permanent part of your student record.

When you have completed the program, you should print this page and save it forever (in addition to all of your course syllabi).
