IntroductionWherewelivematterstoourhealth.Thehealthofacommunitydependsonmanydifferentfactors,includingtheenvironment,educationandjobs,accesstoandqualityofhealthcare,andindividualbehaviors.Wecanimproveacommunityshealthbyimplementingeffectivepoliciesandprograms.Forexample,people
wholive
in
communities
with
smoke
free
laws
arelesslikelytosmokeortobeexposedtosecondhandsmoke,whichreduceslungcancerrisk.Inaddition,peoplewholiveincommunitieswithsafeandaccessibleparkandrecreationspacearemorelikelytoexercise,whichreducesheartdiseaserisk.
However,healthvariesgreatlyacrosscommunities,withsomeplacesbeingmuchhealthierthanothers.And,untilnow,therehasbeennostandardmethodtoillustratewhatweknowaboutwhatmakespeoplesickorhealthyor
acentral
resource
to
identify
what
we
can
do
tocreatehealthierplacestolive,learn,workandplay.
Weknowthatmuchofwhatinfluencesourhealthhappensoutsideofthedoctorsofficeinourschools,workplacesandneighborhoods.TheCountyHealthRankings&Roadmapsprogramprovidesinformationontheoverallhealthofyourcommunityandprovidesthetoolsnecessarytocreatecommunitybased,evidenceinformedsolutions.Rankingthe
healthof
nearly
every
county
across
the
nation,
theCountyHealthRankingsillustratewhatweknowwhenitcomestowhatismakingcommunitiessickorhealthy.TheCountyHealthRoadmapsshowwhatwecandotocreatehealthierplacestolive,learn,workandplay.TheRobertWoodJohnsonFoundationcollaborateswiththeUniversityofWisconsin
PopulationHealthInstitutetobringthisgroundbreakingprogramtocountiesandstatesacrossthenation.
TheCountyHealthRankings&RoadmapsprogramincludestheCountyHealthRankingsproject,launchedin2010,andthenewer
Roadmapsproject
that
mobilizes
local
communities,nationalpartnersandleadersacrossallsectorstoimprovehealth.Theprogramisbasedonthismodelofpopulationhealthimprovement:
Inthismodel,healthoutcomesaremeasuresthatdescribethecurrenthealthstatusofacounty.Thesehealthoutcomesareinfluencedbyasetofhealthfactors.Countiescanimprovehealthoutcomesbyaddressingallhealth
factorswith
effective,
evidence
informed
policiesandprograms.

TheCountyHealthRoadmapsmobilizeslocalcommunities,
national
partners
and
leaders
acrossallsectorstoimprovehealth.TheCountyHealthRoadmapsshowwhatwecandotocreatehealthierplacestolive,learn,workandplay.TheRobertWoodJohnsonFoundation(RWJF)collaborateswiththeUniversityofWisconsinPopulationHealthInstitute(UWPHI)tobringthisgroundbreakingprojecttocities,countiesandstatesacrossthenation.
TheRoadmapsprojectincludesgrantstolocalcoalitionsandpartnershipsamong
policymakers,business,
education,
public
health,healthcare,andcommunityorganizations;grantstonationalorganizationsworkingtoimprovehealth;recognitionofcommunitieswhosepromisingeffortshaveledtobetterhealth;andcustomizedguidanceonstrategiestoimprovehealth.
RoadmapstoHealthCommunityGrantsTheRoadmapstoHealthCommunityGrantsprovidefundingfor2yearstothirtystateandlocaleffortsamongpolicymakers,business,education,healthcare,publichealthand
communityorganizations
working
to
create
positivepolicyorsystemschangesthataddressthesocialandeconomicfactorsthatinfluencethehealthofpeopleintheircommunity.
RoadmapstoHealthPartnerGrantsRWJFisawardingRoadmapstoHealthPartnerGrantstonationalorganizationsthatareexperiencedatengaginglocalpartnersandleadersandareabletodeliverhighqualitytrainingandtechnicalassistance,andcommittedtomakingcommunitieshealthierplacestolive,learn,workandplay.Partner
granteesincrease
awareness
about
the
County
HealthRankings&Roadmapstotheirmembers,affiliatesandallies. AsofFebruary2013,RWJFhasawardedpartnergrantstoUnitedWayWorldwide,NationalBusinessCoalitiononHealth,andNationalAssociationofCounties.
RWJFRoadmapstoHealthPrizeInFebruary2013,RWJFawardedthefirstRWJFRoadmapstoHealthPrizesof$25,000tosixcommunitiesthatareworkingtobecomehealthierplacestolive,learn,workandplay.TheRWJFRoadmapstoHealthPrizeisintendednotonlytohonorsuccessfulefforts,butalsotoinspireandstimulatesimilaractivitiesinother
U.S.communities.
RoadmapstoHealthActionCenterTheRoadmapstoHealthActionCenter,basedat
UWPHI,
provides
tools
and
guidance
to
help
groupsworkingtomaketheircommunitieshealthierplaces.TheActionCenterwebsiteprovidesguidanceondevelopingstrategiesandadvocacyeffortstoadvanceprohealthpolicies,opportunitiesforongoinglearning,andasearchabledatabaseofevidenceinformedpoliciesandprogramsfocusedonhealthimprovement:WhatWorksforHealth.ActionCenterstaffprovidecustomizedconsultationviaemailandtelephonetothoseseekingmoreinformationabouthowtoimprovehealth.Coaching,includingpossibleonsitevisits,is
alsoavailable
for
communities
who
have
demonstratedthewillingnessandcapacitytoaddressfactorsthatweknowinfluencehowhealthyapersonis,suchaseducation,incomeandfamilyconnectedness.
