East Valley
Magellan Health Services of Arizona and Community Bridges, Inc. invite you to an Open House to learn more about the new East Valley Access Point & Transition Point facility located in Mesa, AZ.
Learn more about two new levels of behavioral health services available for adults, take a tour and meet our team!

About the East Valley Access Point & Transition Point Facility in Mesa, Arizona
Access Point24/7 intake and needs assessment.A front door to the behavioral health system offering assessment for ongoing services, brief intervention, and coordination of care through contracted network providers with Magellan Health Services of Arizona.
Transition PointLevel III time-limited residential (3-5 days).16 beds for individuals (18 and older) who need targeted overnight support or planning for transition back into the community.
Facility is located three blocks south of the US 60 (Exit 180) on the NW corner of Javelina and Mesa Drive.
