The administrator may type in information in the Comments, Recommended Actions for

Improvement, and Resources Needed to Complete these Actions sections. The administrator may

also check the appropriate boxes in the Evidence or Documentation to Support Rating section or

add additional items in the open text boxes.

After the administrator has completed the Summary Rating Form, the Allow the teacher to view this

summary rating form box in Summary Status may be checked so the teacher can view the form.

An additional file may be added prior to the principal/evaluator signature by clicking on Choose File to

find your file and then Add File. After the form is complete, click on the box in the

Principal/Evaluator Signature column and follow the electronic signature steps previously

described.

evaluation. Observations are not

evaluations…they are single points in time.

• The evaluation is the Summary Rating Form

completed at the end of the evaluation

process.
