Introducing the eCr 2010 Congress PresidentBy Mélisande Rouger

ECRT: How does ECR compare to other radiological congresses?MST: The majority of radiological congresses are focused on one radiological discipline or even a spe-cific narrow subject. ECR is a unique forum addressing and accommodating all radiologists. There are sessions presenting the cutting edge of science in our own field of activity and at the same time there are opportunities to attend sessions presenting progress outside of our own discipline. This is a great advantage of a multidisciplinary meeting, which provides possibilities to keep abreast of developments in other radiological fields.I find it extremely important for every radiologist to be aware of the progress in various fields of our specialty. This knowledge may help to further develop our own field and to choose the modalities best suited to diagnosing our patients.

ECRT: What are your ambitions and plans for ECR?MST: Like every president of ECR, I would like to shape a very attractive scientific, educational and social programme. Working together with the Programme Planning Committee we have been trying to create sessions choosing topics that should meet the expectations of all participants. It is well known that every year ECR introduces some innovations. In 2010 we will launch a new type of course that will be organ-oriented. In the coming year, it will be ‘Liver from A to Z’. Experts will show the best, most appropriate imaging methods used in diagnosing a wide range of pathologies of the liver in four sessions.The second innovation is the introduction of a multidisciplinary symposium, which we will start with ‘Managing patients with cancer’. The symposium consists of 4 sessions dedicated to prostate, ovary, colorectal and lung cancer. In each session there will be presentations and discussions between a surgeon, an oncologist and a radiologist. By introducing multidisciplinary symposia we would like to bring closer and enhance exchanges between clinicians and radiologists. Nowadays, if radiologists want to remain the key players in clinical discussions, they should understand the clinical questions and stay updated with advances in disease management. I hope this type of session will emphasise the importance of clinical training in the radiological curriculum.My ambition is that we provide an excellent congress for the participants, the faculty and exhibitors, and that it will contribute to the further growth and development of our specialty.

In an interview with ECR Today back in March, Professor Małgorzata Szczerbo-Trojanowska from Lublin, Poland, discussed her agenda for next year’s congress. She revealed that ECR 2010 will shed light on imaging and interventions in oncology and clinical emergencies, among other topics, and the congress’s tradition of innovation will be upheld with the introduction of a multidisciplinary symposium and organ-oriented courses.

ECRT: What will be the main focus of ECR and what will be the scientific highlights regarding new technologies presented?MST: ECR 2010 will focus on a number of subjects. Many sessions will be dedicated to imaging and interventions in oncology. We will also focus on imaging and interventions in clinical emergencies, which will be extensively explored during the congress. There will be a Categorical Course on abdomi-nal emergencies and two Special Focus Sessions: on imaging in polytrauma patients and on paediatric non-traumatic emergencies. I would like to draw attention to the Professional Challenges Sessions ‘The radiologist of the future: Challenges and chances’ and ‘Radiology and Law’. The dramatic evolution of our discipline raises many burning questions which will be presented and discussed by experts.There will be a number of scientific highlights of ECR 2010. Almost every year there is a significant development deserving to be a highlight of a meeting in the majority of fields of our rapidly evolving discipline. As has already been said, one of ECR’s charms is that it covers all aspects of the specialty. Among the scientific highlights in the field of new technologies there will be New Horizons sessions on MRI in lung diseases, hybrid imaging, and functional imaging in CT as well as sessions dedicated to new developments in US imaging.

ECRT: What do you think of the initiative ‘ESR meets partner disciplines’? Will there be another specialty invited to ECR?MST: I find it a very attractive and useful initiative. Those sessions bring radiologists closer to clini-cians and help them to better understand the information clinicians expect from imaging. On the other hand we can clarify what information is possible with contemporary imaging, and which modality is best suited to provide it. The session should enable radiologists to exchange opinions and to improve communication with the clinicians. At ECR 2010, neurology will be a partner discipline.

‘ESR meets’: another virtuoso programme at eCr 2010

Saudi Arabia will offer a general presentation based on its radiological routine, and explain how imaging can help assess peculiar musculoskeletal infections and infestations, as well as Brucellosis infection. The Saudi delegation will also stress the role of imaging in the discovery and characterisation of genetic diseases, and the role of nuclear medicine and functional imaging in Non-Hodgkin lymphomas (NHL), a diverse group of haematologic cancers.

ECR 2010, under the theme ‘Virtuosity in Radiology’, promises to be yet another landmark in the history of the congress. To ensure this success, ‘ESR meets’, one of ECR’s most popular programmes, will invite three countries from three different continents to present their expertise in radiology. Poland, Saudi Arabia and Argentina will deliver their latest scientific achievements of international relevance. Neurology will also be a guest, as part of ECR’s efforts to promote multidisciplinary dialogue.

Inviting a partner discipline to take part in the debates has become a necessity and ECR recognised this two years ago by extending its ‘ESR meets’ programme to specialities beyond radiology. At ECR 2010, neurologists will, together with radiologists, focus on the diagnosis and treatment of stroke, the number two cause of death worldwide. From clinical requirements and expectations of diagnostic imaging to diagnostic imaging of ischemic stroke, experts will talk of their experience in order to improve both their cooperation and medical outcome. They will also deal with the management of acute stroke patients and the endovascular treatment of patients with ischemic strokes.

Argentina will be the first Latin American country to take part in the ESR meets initiative, and expects an increased attendance of Argentinean and Latin American radiologists as a result. The delegates will focus on pneumo-oesophageal MDCT in cancer presurgical characterisation and staging, radio-anatomy of the lower arterial anastomoses, and high-resolution MRI in rectal cancer. They will also deliver a presentation on cystic echino-coccosis, based on local experience in Patagonia.

Poland, home country to ECR 2010 President Prof. Małgorzata Szczerbo-Trojanowska, a world famous interventional radio-logist, will present a session on Emergency Radiology. Polish ‘rising stars’ in radiology will present their achievements in modern emergency stroke imaging, cardiac emergency imaging, and in the diagnosis and management of aortic or end-organ complications in the dissection of descending aorta. They will also provide examples of their use of CT pulmonary angiography in staging acute pulmonary embolism.

Lymph nodes come under renewed scrutinyBy Paula Gould

The T-word – tumour – usually dominates discussions on cancer imaging. Can you see a suspect mass? Are you sure it’s malignant? Is it responding to therapy or continuing to grow?

Nodal metastases may be mentioned, but only in passing. Yet lymph node involvement is a key piece in the diagnostic jigsaw. Cancerous spread to nearby lymph nodes can have important implications for a patient’s prognosis and the recommended course of treatment.

Prof. Jelle Barentsz, professor of radiology at the University Hospital Nijmegen, the Netherlands, will be championing the cause of this often overlooked aspect of oncological imaging at ECR 2010. A self-styled ‘musketeer’, he will be leading a new state of the art course covering the best of current and future techniques for lymph node evaluation.

“Ask any clinician. They will say that the recognition of small metastases is vitally important to the patient. There are not too many imaging techniques that can do this at the moment. In this session, radiologists will be shown how a variety of novel techniques can solve this challenging problem,” he said.

In prostate cancer, for example, detection of just one or two positive lymph nodes rules out curative therapy. Patients with no nodal involvement may have a prostatectomy, whereas those whose cancer has spread to nearby lymph nodes will be treated with hormone therapy or aggressive radiotherapy instead, Barentsz said. Treatment choices also change for patients with breast cancer and head/neck cancer if lymph node metastases are found.

PET/CT with the glucose analogue fluorine-18 FDG is regarded as a good way of evaluating lymph node

involvement, particularly in lung cancer, melanoma, and lymphoma. Pathological lymph nodes will stand out clearly on FDG-PET images as bright or dark dots. The downside is that this strategy works poorly in cancers with low metabolic activity, such as pelvic cancer and prostate cancer.

Advances in technology have also made it possible to see lymph nodes on CT and MRI. Here, nodal enlargement is typically taken to indicate the presence of metastatic disease. The sensitivity of conventional CT and MRI for the detection of lymph node metastases is only around 30%, though. As a result, surgical lymph node dissection is sometimes used to check whether a cancer has spread.

Size is not the only factor that should be considered when assessing the status of lymph nodes on conventional CT and MRI, according to Dr. Dow-Mu Koh, a radiologist at the Royal Marsden Hospital in Sutton, U.K. The shape and contour of nodes can also help to indicate whether they have been invaded by cancer cells, as can certain internal characteristics. Radiologists looking at abdominal and pelvic nodes could also be looking for calcifications, heterogeneity, or signs of necrosis, in addition to measuring nodal diameters, said Koh.

He will be posing the following question to ECR 2010 delegates: is morphological assessment using CT and MRI useful?

“We are very reliant on size,” he said. “Size is not always a good way of working out whether a node is involved. In fact for some cancers, size is a very poor discriminator.”

Koh will stress that lymph node imaging findings should not be interpreted in isolation. Pathological features of the primary tumour must also be taken into account.

Detection of nodal metastases can make a big difference to the prognosis and management of cancer patients, yet this aspect of oncological imaging seldom gets the attention it deserves. This explains the introduction at ECR 2010 of a new state of the art course on imaging-based lymph node evaluation.

“You have to think about the behaviour of the tumour, how this tumour would normally spread, and the pathway the spread is likely to take through the lymph nodes,” he said.

Contrast-enhanced MRI with a lymph node-specific agent could improve the sensitivity and specificity further, according to Barentsz. At ECR 2010, he will use his own presentation to outline the future potential of MR imaging with ferumoxtran-10, a lymphotropic, ultrasmall superparamagnetic iron oxide (USPIO) agent.

Lymph node imaging with the USPIO agent, known as Combidex in Europe and Sinerem in the U.S., has yielded promising results. In particular, the technique’s high negative predictive value means that surgery can be ruled out if lymph node involvement is not seen on imaging. ‘Just in case’ lymph node dissections should no longer be required, saving time and money and sparing patients unnecessary anxiety and discomfort. Regulatory approval for lymph node imaging with ferumoxtran-10 has yet to be granted.

Contrast-enhanced ultrasound is another up-and-coming strategy. This works best when visualising lymph nodes that are close to ultrasound transducer, Barentsz said. Patterns of enhancement are being used to identify small metastases in small lymph nodes, the ‘holy grail’ of lymph node imaging.

“I want this course to show that lymph node imaging is back, thanks to developments in ultrasound, PET, contrast agent development, and MRI,” he noted. “I would like to revitalise radiologists’ interest in lymph nodes by showing them not just state-of-the-art techniques but the state of the future art, too.”

emergency room experts lay out options for polytrauma imagingBy Paula Gould

Road traffic accidents in Europe lead to around 40,000 deaths and well over 1.5 million injuries every year. Many of these accident victims will be admitted to hospital with multiple internal and external injuries. Rapid diagnosis of the most serious traumatic damage and prompt treatment may be a matter of life or death.

The critical role that radiologists can play in the management of polytrauma patients will be the subject of a Special Focus session at ECR 2010. Given the time-critical nature of the problem, a strong emphasis will be placed on integration with clinical colleagues and teamwork. Good polytrauma imaging involves more than giving an accurate description of specific injuries, and requires highly efficient logistics, diagnostic algorithms, and examination protocols, according to speakers at the session.

“We have to find the optimal way of moving the diagnostic front line as close to the actual whereabouts of the patient as possible, and then we have to create a diagnostic pathway for the patient that is as short as possible,” said Prof. András Palkό, professor of radiology at the University of Szeged, Hungary, who will be chairing the session.

The diagnostic work-up of polytrauma patients can vary considerably from hospital to hospital, country to country. Speakers will outline the ways that ultrasound, multidetector CT (MDCT), and interventional radiology can each play a role, though without imposing a prescriptive formula. Local circumstances will always influence protocols, Palkό said.

Palkό hopes the session will ignite discussion on the competing merits of ultrasound and CT in polytrauma

patient management. An ultrasound scan can be completed quickly at a patient’s bedside, or even at the site of an accident, providing important information about abdominal injuries. If the patient has other injuries, however, then why not go directly to CT?

“Many people, including myself, say that a very fast CT survey of the critical regions of the body might be better for the patient than just a simple x-ray/ultrasound tandem algorithm. This has to be discussed further,” he said.

FAST scanning (focussed assessment with sonography for trauma) is very good at detecting bleeding in the abdomen or thorax, said Dr. Ulrich Linsenmaier, associate professor of radiology at Ludwig-Maximilians-University, Munich, Germany, and one of the session’s invited speakers. All polytrauma patients admitted to the Ludwig-Maximilians-University hospital, which is a level I trauma centre, undergo a FAST scan as soon as they arrive, which includes the pleural space and the pericardium. If severe internal bleeding is detected, they will progress straight to the operating room.

CT is the next step in the polytrauma imaging pathway for patients who do not have bleeds requiring immediate surgery. Almost all diagnostic questions that conventional radiography used to answer can now be answered by MDCT, Linsenmaier said. As a result, the role of radiography in this setting has steadily decreased, while the use of whole-body MDCT has become more common.

department. Transport to the CT suite, patient positioning, imaging, image reconstruction and preparation of a preliminary report for the clinician must then be completed in the next half hour.

“We reach this goal in 75% of patients,” he said. “This allows us to make a comprehensive diagnosis of all the patient’s major injuries.”

The MDCT examination covers the head, cervical spine, thorax, and abdomen in a single step. This ‘head to symphysis’ protocol has never failed to spot a major injury, and no patient has been sent home with an undiagnosed injury or has received incorrect treatment owing to under-diagnosis, Linsenmaier said.

Efficient data handling is a key part of the process given that the MDCT study may generate up to 2,500 images. The technologist charged with image reconstruction must be experienced at handling complex CT scans. Radiologists then review the reconstructed images on a workstation adjacent to the scanner rather than a PACS console, a practice known as volume image reading.

Linsenmaier endorses the need for cooperation between radiologists and clinicians in trauma teams. He also advocates further subspecialisation of emergency radiology in Europe. The European Society of Radiology intends to form a working group on emergency radiology as a step in this direction, he said.

Preparing for tomorrow today By Mélisande Rouger

From training to management, numerous challenges await radiologists in the future. And in the face of constant evolution in medicine, the role radiologists will play in innovative multidisciplinary fields such as molecular imaging needs to be discussed now. This is what a professional challenges session will offer to do at ECR 2010.

To prepare future radiologists, it is necessary to update the curriculum for training in radiology, bearing in mind alarming demographic projections. The next ten years will indeed see a significant decrease in the number of radiologists in Europe, which will have adverse effects on the quality of healthcare. In France, restricted access to medical universities through admission examinations has already led to a reduction in the workforce. Combined with the looming retirement of the last baby boomers, this will leave radiology departments short of professionals. “This is going to have a huge impact, maybe less so in other countries, but in France this is a real problem. We need to be aware of that today to prepare ourselves for what’s coming,” said Prof. Philippe Grenier, who will chair the session.

Constant progress in medical imaging and the development of new therapeutic and diagnostic strategies mean that some subspecialties need to be better represented in the curriculum. Cardiac radiology is one of them, but interventional radiology and molecular imaging must gain importance as well.

Subspecialisation in radiology necessitates more in-depth clinical knowledge, whether it is in gastroenterology for a digestive radiologist or orthopaedics for an articular radiologist for instance. But the risk of radiologists specialising too far is that radiology as a discipline may disintegrate. “We need to maintain a common trunk in radiology, for economic reasons but also to guarantee efficiency, quality control, and ensure 24/7 healthcare. We need a single house for radiology,” explained Grenier, who is Chairman of the Department of Radiology and head of the medical board at the Hôpital La Pitié-Salpêtrière in Paris, France. Together with ESR Past President Prof. Ian McCall, who will present this topic, he updated the curriculum in

2005, increasing the time allocated to subspecialisation to two years while decreasing general training to three years.

Radiologists now also need to address challenges brought by the imaging technique of the future: molecular imaging. One difficulty they must face is finding their place in a multidisciplinary environment. First, the control of PET scanning, a widely used modality in molecular imaging, sparks off turf battles with nuclear medicine in most European countries. Then, radiologists are operating within multidisciplinary teams together with physicians, biologists, physicists and chemists, but their role in this research work is not necessarily essential. They have been involved in the early steps thanks to pioneers such as Prof. Gabriel Krestin, founder of the European Institute for Biomedical Imaging Research (EIBIR), who will present the chapter on molecular imaging at ECR 2010. However, radiologists could play a key role in transferring research work to the test phase, Grenier believes. But for this, they must absolutely secure their place in research teams as of today.

Finally, radiology could defend itself better in turf battles and to the authorities if it communicated more effectively and efficiently. “I think radiologists in general do not yet realise how important it is to show the efficiency and quality of their services,” said Grenier. Participants will be informed on these issues by Mathias Goyen, Doctor at the University Medical Center Hamburg-Eppendorf in Hamburg, Germany, and CEO of UKE Consult und Management GmbH, a company specialising in the consulting and development of healthcare projects. Communicating their standards, taking the necessary managerial steps, but also providing more information to patients, on radiation for instance, is how radiologists could improve their disciplinary strategy.

Numerous challenges face imaging, but there are reasons to be optimistic, Grenier believes. For instance, teleradiology could help to reduce the effects of the impending lack of radiologists. This, and other solutions will be debated during the session, and participants are extremely welcome to present their ideas.

New ways to image lung diseases must be introduced in clinical routineBy Mélisande Rouger

Lung imaging has changed over the past few years as new techniques have been introduced, but not all thoracic radiologists are aware of this evolution, which could significantly improve patient’s management. A dedicated categorical course at ECR 2010 will present the latest developments in this field.

Divided into six chapters, the course will first focus on chronic obstructive pulmonary disease (COPD) in smokers. COPD was long thought to affect only the bronchial tree and lungs, but it is now established that COPD also affects the cardiovascular system and particularly the heart. When performing a CT scan of a smoker’s lungs, it is thus important for radiologists to consider whether they should image the heart as well, explained Prof. Martine Rémy-Jardin, who will coordinate the sessions. New protocols in morphological and functional CT will be presented, as well as recent research in MRI.

Other respiratory diseases may also have an effect on the heart, especially if they worsen. Acute pulmonary embolism and many other acute and chronic thoracic diseases may alter cardiac function. A cardiothoracic CT examination is now possible in only one acquisition, and radiologists need to know this to adapt their protocols.

New horizons are also visible in the management of pulmonary tumours. One new therapeutic approach consists of destroying neovessels that develop in a tumour to dry it out. Researchers have worked a lot on angiogenic treatments for the past few years and radiologists should make themselves aware of these developments, said Rémy-Jardin, who is head of the Department of Radiology and Chairman of the Department of Thoracic Imaging at the Hôpital Calmette of the University Center of Lille, France. Chemotherapy and radiotherapy have changed, and these new treatments change the way radiologists manage patients by creating new ways to perform examinations, she added.

For instance, one innovation developed by Rémy-Jardin’s team enables depiction of the number of neovessels in a tumour using CT. Dr. Nunzia Tacelli, who worked for two years at the Hôpital Calmette, will tell participants how to do so with CT tumoral angiogenesis analysis.

“We used to image the tumour to measure it and see if it had grown, but now our focus has changed. Imaging can offer very precise answers to these new, targeted treatments,” Rémy-Jardin said.

Scintigraphy has long been regarded as the only way to perform lung perfusion imaging. However, CT and MRI have garnered researchers’ attention over the past three years, as both techniques offer more precise results. CT’s biggest advantage over MRI is that it enables perfusion and diagnosis to be done during the same examination. But research is also progressing in MRI, and the course will emphasise this point through various clinical cases.

New technologies lead radiologists to use more and more volumetric CT, which allows one to scan the whole thorax but also exposes patients to high doses of radiation. By comparison, imaging with sequential cuts is ten times less irradiating, and might be more pertinent in some cases. Alternatives to CT are particularly relevant in the follow-up of pulmonary diseases, since one can focus on a precise location instead of screening the whole thorax again.

Participants are invited to take part in discussions on how to optimise volumetric protocols. “We use a slightly provocative approach to start the debates by asking ‘Is volumetric CT the only way of scanning lung diseases? If yes, in which cases? What are the alternatives to it, and when should we use them?’ It is easy to put in place, but we need to reach a consensus, so this is why we need to discuss it,” Rémy-Jardin explained.

Finally, the course will focus on the interaction between endoscopists and radiologists, to inform the latter of new developments in endoscopy and improve cooperation between both disciplines.

The Radiology Trainees Forum (RTF) was established in 1991 as an organisation representing the views of radiology residents (trainees). It is coordinated by its board, maintains close relationships with the European Society of Radiology (ESR) and invites all ESR member countries to appoint a resident representative. The workshop starts with a 90-minute lecture programme focusing on how to interpret stroke CT and stroke MRI and datasets.

Clinical experts will provide an overview of the anatomical and pathophysiological prerequisites for the interpretation of perfusion CT and MRI in ischemic stroke and the basic concepts and new developments in post-processing of stroke MRI. The potential and limitations of perfusion CT and perfusion MRI will be discussed separately. After attending these lectures, participants will have learned which key imaging findings should be considered for a comprehensive stroke CT or stroke MRI report. The second part of the workshop provides hands-on experience of post-processing CT or MRI perfusion data of stroke patients using workstations of different vendors. Each vendor supplies six workstations (each vendor 3 CT and 3 MRI workstations). Each type of workstation is mounted with clinical cases (12 CT, 12 MRI) including either non-enhanced CT, CT-angiography and CT-perfusion, or diffusion, perfusion, FLAIR, T2*w MRI and MRA. During the workshops, clinical stroke case data will be post-processed and interpreted and implications for treatment decisions will be discussed.

Non-experienced users are advised to follow the introductory lectures before entering the hands-on sessions. The user will need to select either CT or MRI post-processing for each vendor.

Clinical experts will provide an overview of the anatomy of the heart, diagnostic accuracy and limitations of CT coronary angiography, and tips and tricks in the evaluation of cardiac CT examinations. After attending these lectures, participants will have learned which key imaging findings should be reported in a comprehensive cardiac CT report.

The second part of the workshop provides hands-on experience of cardiac CT post-processing using dedicated workstations of different vendors. Each workstation is loaded with the same clinical cases, including calcium scoring, coronary angiography, and left ventricular function CT datasets. A comprehensive report of the case will be presented including conventional coronary angiography correlation after each session. This format allows the participants to evaluate the performance of three different workstations using the same CT datasets.
