Online-‐Application Instructions to TFT-‐5th Year @ KSOE Let’s get started! Just a few things to keep in mind:
• Before you click “submit”, you may save and return to the application to continue working. • Remember to click the “save and continue” button as you complete each section. • If you need to make a change, press the “reset” button. • Save your PIN and password. • When returning to your in-‐progress application, click “edit application” and then access each
section via the navigation bar (on the left). • After you click “submit”, you cannot change answers but you can continue to upload documents
(i.e. TB test)
What you’ll need now to get started: • Social security number • Grade point average • Unofficial transcript from GaelXpress (pdf)
What you’ll need later to complete the application: (refer to Application Checklist and FAQs for more information on each requirement):
• Personal statement • TB test report (test after 6/2015 is valid) • Verification of the following requirements:
o Basic Skills Requirement (i.e. CBEST and other options) o Subject Matter Competence Requirement (i.e. CSETs) o Certificate of Clearance Requirement (LiveScan Fingerprinting and CTC)
What’s not required:
• Letters of recommendation • Resume • GRE (Graduate Record Exam)
Step 1: Create Account
1. First check if you already have an account (we may have created one for you). Go to KSOE home page and click “Apply Now”
2. Click “Forgot your User Name or Password?”
3. Enter your SMC email address and click “search”
4. If your account is found, a notification message will be sent from [email protected] (check
Spam folder if you don’t see it in your inbox). Follow instructions to reset password. Return to KSOE home page, click “Apply Now”, click “User Login” tab and log-‐in with your username and password.
5. If your account is not found, create an account. Return to KSOE home page, click “Apply Now”. Click on “Create Account” tab; enter profile information and scroll down to create log-‐in. Click “Create Account” button and return to KSOE home page to log-‐in.
Step 2: Start Application 1. Click “Start Application”, this brings you to the Welcome page. You will complete the general
Application (for all graduate students) and also the Graduate Education Supplement sections. When you return to work on your application, you can access sections through the navigation links on the
left side of screen. Click “Start Application”.
2. Program Selection: select the options in the drop-‐down menu according to the screenshot below.
Click “save and continue”.
3. Personal Information: Answer questions and click “save and continue” 4. Educational Background:
a. Saint Mary’s College code is “386” b. Approx. number of semester units is “36”
c. Major is “JCL TFT” d. Click “Upload Document” to upload pdf of your unofficial transcript from GaelXpress. Official
transcript is not required until your BA posts.
5. Employment and Resume: This section is not required. Click “save and continue”. 6. Personal Statement: Click “Upload Document” and upload your statement. Click on “Graduate
Education Programs” for statement instructions.
7. Financial Aid Information: Complete the section. Click “save and continue”.
8. You have completed the general application. Now you will complete the supplement.
Graduate Education Supplement – Program Selection: Under “Credential Programs”, select “Multiple Subject with Master of Arts in Teaching” or “Ed. Specialist Mild/Moderate with Master of Arts in Teaching”. Click “save and continue”. Do not complete “Degree Program” or “Doctoral Program” areas.
9. Terms and Required Documents: Complete all sections except for “Credential Information” and
“Experience Verification” – you can skip these 2 sections.
Concentration: This section will auto-‐populate with your program and start term.
Required Tests – Subject Matter Verification (1): Click “Upload Document” to upload your CSET passing score report or proof of your CSET registration. Skip #2 Subject Matter Program Preparation (SMPP) – this does not apply. Required Tests – Basic Skills Requirement: Click “Upload Document” to upload your CBEST passing score report or proof of your CBEST registration. If you are meeting the Basic Skills Requirement with a different exam option (SAT, ACT,AP exam) upload verification under “different exam option”. Credential Information: Skip this section. Student Health and Accident Insurance: Complete sections A and B. Health insurance is not available to graduate students. TB Test: Click “Upload Document” with your test report (taken 6/2015 or later). Convictions: Complete this section. Refer to information in California Clearance Requirement below if you answered “yes”. California Clearance Requirement (Certificate of Clearance or CoC): If you have already gone through the fingerprinting process, but you do not have a CTC Certificate of Clearance then you are required to do this again. The CTC requires the results of your
fingerprinting to be reported directly to them. First complete the Form 41-‐LS (Fill out Section 3 only. All other sections are already completed for you) for the fingerprint background check . Double-‐check your information to ensure it is accurate and matches your CTC record (especially SSN and DOB). Bring the form and valid ID to a Live Scan location. Then go online to CTC and apply and pay the fee for your Certificate of Clearance. For detailed instructions, refer to How to apply for the CoC or CTC FAQs. Upload proof of your CoC by:
• Logging into your profile at the CTC (enter SSN and DOB). • Click “Next”, which brings you to a list of your CTC documents. • Click on the Document Number for your CoC to view details.
Take a screenshot of this screen and upload the file to application. Your CoC must be granted by the CTC to be eligible for Supervised Teaching. If you need to answer “yes” to one or more of the “moral fitness” questions (expunged arrest, community service, paid a fine, etc.), please contact Colleen Keirn, SMC Assessment Coordinator. Colleen can help you through the process of documenting this history. Failure to properly document this history will result in a delay of your CoC and may also delay your supervised teaching placement. Experience Verification: Skip this section. Click “Save and Continue”. This will bring you to the Recommendations page. 10. Recommendations are not required. However, you must complete the section with the information
below. No other information or fields are required. a. Click button “recommendation provider list” b. Click “+ add a provider” c. Enter the information below for your 3 recommendation providers. Click “add Provider”

11. When done, click on “additional information” in the Graduate Education Supplement in the navigation bar (on the left side of screen).
12. Additional Information: Complete section. Click “save and continue”.
