Message from the Chief Medical Officer I knew it was coming. That day when I would sound like my parents, saying “I can’t believe the New Year is here already!” But I am saying that now and feeling as if time is passing way faster than it used to. How do you harness time when it flies by so quickly?

Those much smarter than I am have said that it helps to be organized, to write lists, and set goals of things to accomplish. Oh, yeah—and you have to review those lists and goals periodically to make sure you haven’t forgotten to do the things you promised to do when the year began.

Chances are, I am not the only one who needs to follow that good advice. If you are like me, take out your pen, find a pad, and write this down:

» See the doctor for a physical – it is covered by the health plan, as is the annual Wellness visit.

» Get your flu shot (if you skipped it in the fall) – it’s not too late, trust me.

» Ask the doctor what health screenings you need this year – some might be new for you.

» Laugh more, worry less, and take that walk in the park whenever you can!

The importance of visiting with a Primary Care Provider (PCP) Start the New Year off right – schedule a Wellness Exam! Visiting with a doctor at least once a year is essential. Researchers at the Health Affairs Journal found that patients with a Primary Care Provider (PCP) have better management of chronic diseases, lower overall health care costs, and a higher level of satisfaction with their care.

Your PCP will get to know you and, over a period of time, begin to recognize what is and isn’t normal for you. They can track and notate changes and, if needed, work with other specialists to ensure you receive the right care, at the right time, and at the right place. Besides having a trusted partner when it comes to your care, your health records are in one central location and your medication can be better supervised.

Your PCP also gets to know your values, family situation, and health goals, which provide valuable information as he or she assesses your health. By preventing health issues before they become serious, you can save yourself from more challenging health conditions and the costs associated with them.

Hesitation to schedule an appointment with a PCP is understandable because of the potential wait. Luckily, there are alternatives if you need to make an appointment outside of your scheduled, routine check-up — you may be able to see a Physician Assistant (PA) or Nurse Practitioner in your PCP’s office more quickly, reducing the time it takes to obtain an appointment.

To help your PCP provide you the best care, make a list of questions to ask before your appointment and be sure to:

» Notify your PCP within 30 days of any emergency room or urgent care visit

» Indicate office visits or evaluations by a specialist, including lab work and procedures performed (be sure to request your records and share them with your PCP)

» Indicate changes in medication, including how much you take, new medications, or any time you stop taking a medication

» Share information about diabetic eye exams

» Include behavioral health visits, as well as any admission or discharge from a hospital, rehabilitation facility, or nursing home.

The BCBSAZ Advantage Partnership You should always be able to get the care you need when you need it, so BCBSAZ Advantage continually partners with doctors and hospitals to ensure you receive medical care in a timely manner. We require our providers to offer access to medical and behavioral health services without excessive scheduling delays, as well as communicate standards for timeliness of appointments and office waits with our provider network. If you feel your physician’s office is not scheduling you in a timely manner, please contact Member Services for assistance.

Changing a PCP Did you know you may change your PCP for any reason at any time? It’s also possible your PCP might leave our plan’s network of providers and you would need to select a new provider. To find a new in-network PCP in your area, or to identify if the doctor you selected is in-network, follow these easy steps:

» Log onto the BCBSAZ Advantage website at www.azbluemedicare.com

» At the top of the page, locate and click on “Find a Doctor”

» Click on the link “BCBSAZ Advantage’s Network Providers” to find a PCP

Once you select a PCP, it’s important that you call Member Services prior to making your first appointment. Member Services will confirm if the physician you chose is accepting new patients, update your membership record to show the name of your new PCP, and submit a request for a new plan membership ID card to be mailed to you with the name of your new PCP. (You will receive your new plan membership ID card in 10-14 business days.) All new PCP assignments will be effective the 1st day of the following month. For example, if you call on July 14, your change will be effective August 1.

Quick tips for a smooth transition to a new PCP include:

» Ensuring your new provider practices at an in-network facility

» Signing up for an online patient portal if a new PCP offers it, which can help maintain a central point of coordination for your care.

» Transferring medical records

» Providing your prescription refills coverage

» Indicating if any orders for Durable Medical Equipment (DME) supplies have changed or if any services require an ongoing prior authorization.

» The time is now to access primary care services. Regularly visiting a PCP helps you to look out for your health and well-being not just for today, but for your future.

Important 2018 Pharmacy Benefit Information If you were a member with us in 2017, we want to thank you for allowing us to be your healthcare plan again in 2018. We also want to take this opportunity to highlight some of the major changes in the pharmacy benefit in case these changes might affect you so you can plan accordingly.

Deductible applied to certain drugs – mainly brand name drugs If you joined the Plus plan in Maricopa County, you have a $100 yearly deductible. (The Classic plan in Pima County has a yearly deductible of $195.) The deductible applies to only those drugs on tiers 3, 4, and 5 which are almost all brand name drugs. After your deductible has been met, whether on one or multiple prescriptions, you will move into the Initial Coverage Stage and pay the standard copay for that drug. Members with Low Income Cost Sharing (LICS) subsidy levels 1, 2, and 3 do not pay the deductible while those with LIS level 4 pay an $83 deductible.

COPD inhalers and Insulin no longer covered in the Gap phase Preferred COPD inhalers and insulin (tier 3) no longer have copay coverage through the gap, meaning those drugs will be subject to the standard gap brand coinsurance of 35% of the total cost when you’re in the Coverage Gap Stage. If you normally enter the coverage gap, it’s beneficial to ask your prescriber for samples early in the year or about the less expensive ReliOn™ insulin brand available at Wal-Mart. Please ask your local pharmacist or health care provider for more ways to be cost-conscious.

Brand diabetic drug no longer covered – but the generic is Nesina (alogliptin), Kazano (alogliptin/metformin) and Oseni (alogliptin/pioglitazone) have been removed from the formulary and replaced with

their generics on tier 1. This change will save you money because the deductible does not apply to these drugs and will control your blood sugar just like the brand name drugs. If you’re using Januvia or Janumet, you may want to ask your prescriber about switching to alogliptin so you pay a tier 1 copay instead of a tier 3 copay and also avoid the deductible.

Preferred COPD inhalers no longer have a generic copay Preferred inhalers for COPD are now on our preferred brand tier 3 and non-preferred COPD inhalers are on tier 4. If you’re using one of the non-preferred inhalers on tier 4 and are interested in using a preferred agent, please ask your prescriber if an alternative on tier 3 is appropriate for you. Refer to your 2018 formulary for a list of covered COPD inhalers.

Plan ahead and use your EOB and Pharmacist to help To estimate what your costs will be should you enter the coverage gap use your past Part D Explanation of Benefits (EOBs) that you receive every month. First find the drug you’re interested in researching then add the amount you pay + plan costs to get the total cost of each Rx. Multiply the total cost by 0.44 (44%) for generics or 0.35 (35%) for brand name drugs and you’ll have an idea of what to expect when you transition into the Coverage Gap Stage. Keep in mind drug prices fluctuate often, much like groceries or gasoline, so these costs are not set in stone.

Please ask your local pharmacist or health care provider for medication alternatives, samples or potential changes to your drug therapy.

Prevention Corner A New Year – A Healthy New You! Preventive exams are an integral part of any wellness program where early detection provides an opportunity to uncover health issues or risk factors you may not be aware of and could allow for more treatment options. Diseases where early detection is important include glaucoma, breast cancer, and kidney cancer.

Eye Exams JANUARY is Glaucoma Awareness Month

Many eye and vision problems have no obvious signs or symptoms, so annual comprehensive eye and vision examinations are an important part of preventive health care. Early diagnosis and treatment of eye and vision problems can help prevent vision loss, especially with age-related diseases. Each patient’s signs and symptoms, along with your optometrist’s professional judgment, will determine what tests your optometrist conducts, which may include, but not limited to, patient history, visual acuity (how clearly each eye is seeing), preliminary tests (visual function and eye health) including glaucoma and cataracts, keratometry (measures the curvature of the cornea), refraction (lens power necessary for nearsightedness, farsightedness, or astigmatism), and how well your eyes focus and move and work in unison.

Mammograms FEBRUARY is Cancer Prevention Month

A mammogram is a specific type of breast exam used to aid in the early detection and diagnosis of breast diseases. Early detection of breast cancer

with screening mammography means that treatment can be started earlier in the course of the disease, possibly before it has spread. Mammograms can show changes in the breast up to two years before a patient or physician can feel them, and may also prevent the need for extensive treatment for advanced cancers, and improve chances of breast conservation.

Kidney Exams MARCH is Kidney Cancer Awareness Month

Kidneys filter 200 liters of a blood a day, help regulate blood pressure (and the body’s salt, potassium, and acid content), direct red blood cell production, and balance the body’s fluids. But they are also prone to disease—1 in 3 Americans is at risk for kidney disease due to diabetes, high blood pressure, or a family history of kidney failure. More than 30 million Americans already have kidney disease and most don’t know it because there are often no symptoms until the disease has progressed. Early detection can make a difference in preventing kidney disease which is the 9th leading cause of death in the country.

Your Emotional Health Now is a perfect time to start working on those New Year’s resolutions and there is no better place to start than your emotional health! Make some goals for the New Year – Do you want to lose weight? Maybe you want to become stronger or healthier? What areas do you want to improve on?

What is Emotional Health? Emotional Health is a state of being that exemplifies self-confidence, happiness, and emotional security. It’s also about being resilient to change—people who are emotionally healthy are able to cope with life’s challenges and are able to share their feelings with others in a constructive manner. Emotional health is a reflection of our emotional well-being. According to author, Jess Scott, “When our emotional health is in a bad state, so is our level of self-esteem. We have to slow down and deal with what is troubling us, so that we can enjoy the simple joy of being happy and at peace with ourselves.”

Tips for Improved Emotional Health Emotional health comprises of five (5) components where it’s important to take care of all five parts to remain healthy:

Purpose. Find a meaningful reason to wake up each day and get out of bed. A way to feel needed can improve your emotional heath— volunteer at an animal shelter, work with a hospital program to visit terminally ill or sick patients, become a companion for a senior person, or volunteer at a local library. An activity that makes you feel good about yourself will improve your emotional health.

Sleep. Quality sleep is very important for emotional health. Skipping even a few hours of sleep can have a lasting impact on your emotional health throughout the following day. Decreased sleep can lead to stress and poor focus. It is best to avoid stimulation before bed such as

television, caffeine, computers, and electronic devices. Utilize relaxation techniques like a warm bath, meditation, journaling, and reading.

Diet. Be aware of what you eat affects your mood and the way you think or feel. A diet rich in fats and low in sugar can improve sleep and energy. Always check with your doctor first before you begin any type of diet.

Stress. There are many ways to reduce stress. Be socially active—visit friends, attend a support group, and become physically active. Engage your senses such as taste, smell, touch, visual and hearing. Listening to music, cooking, journaling, and reading are just a few ways to reduce stress.

Physical Health. When you improve your physical health, you improve your emotional health. Exercise releases endorphins which are the body’s natural way to improve your mood. Physical exercise can relieve stress, improve memory and help you sleep better. Always check with your doctor before you begin an exercise program.

Fraud, Waste, And Abuse (FWA) What is an Explanation of Benefits Statement? After your provider visits and filling of prescriptions, BCBSAZ Advantage sends its members an Explanation of Benefits statement (EOB) each month for all services provided and payments made on their behalf under its Medicare Advantage (Part C) and Medicare Prescription Drug Benefit (Part D) plans. Please note that an EOB is not a bill.

Your EOB will provide you with information regarding: » What services your health care provider

billed for

» The amount approved by BCBSAZ Advantage for payment

» How much BCBSAZ Advantage paid; and

» What, if anything, you as the member may be billed for.

Tips for Detecting Errors and FWA Reviewing and understanding your EOB is one of the key tools for both you and BCBSAZ Advantage to detect errors, as well as potential fraud, waste, and abuse.

» Keep a calendar, journal or other record of medical visits, tests, receipts for services, and equipment received.

» Review your or your loved one’s EOB as soon as it arrives in the mail to ensure all of the services listed were actually rendered.

» Compare your EOB to your receipts, calendar, journal and other records and if you notice any mistakes, or have questions, call your provider.

» If you still have questions or need further help, contact Member Services at 1-800-446-8331 (TTY: 711).

How to Report Errors and FWA If you think a charge is incorrect or you don’t understand something, and you know the

doctor, supplier, or other healthcare provider that performed the service(s), your first step is to call their office and ask about it. The person you speak with may be able to help you better understand the services or supplies you received, or may realize that an error was made and offer to correct it.

If you’ve contacted the doctor, supplier, or other healthcare provider and still have questions, or if you need additional assistance, please contact Member Services toll-free at 1-800-446-8331 (TTY: 711). Hours are 8:00 a.m. to 8:00 p.m., Monday - Friday from February 15 to September 30; and 7 days a week from October 1 to February 14. Member Services also has free language interpreter services available for non-English speakers.

If you suspect that BCBSAZ Advantage is being charged for a service or supply you didn’t get, or you don’t know the doctor, supplier, or other healthcare provider on the EOB, or you believe that fraud, waste, or abuse has occurred, please call the toll-free ComplyLine at 1-855­729-9719 and report your concerns. TRENDING Member FAQs Happy 2018 – It’s Welcome Season! It’s January 1 and you are newly enrolled in a BCBSAZ Advantage plan or are a returning member – Welcome! At the beginning of each year and through December 31, our Member Services department is pleased to be a resource to help answer your questions so you can be the expert in managing your health care. Member Services can be reached at 1-800-446-8331 (TTY: 711).

Health plans contain a lot of information so you’re not alone in having some questions! In addition to perhaps identifying a new in-network Primary Care Provider (PCP) (see article on page 4), typical member questions during the months of January through March include:

Member ID Cards Be sure to bring your BCBSAZ Advantage ID Card with you when you receive medical services or pick up prescriptions (you don’t need your Medicare card). You should have received your new 2018 ID card in December 2017. If you haven’t received your ID card by January 1, you can call Member Services (1-800-446-8331) to request one.

Finding a Contracted Provider Because BCBSAZ Advantage is a Health Maintenance Organization (HMO), you need to use providers who are part of our “network” in order for services to be covered. You can find contracted providers at www.azbluemedicare.com, using the Find a Provider link. You can also find contracted providers in the 2018 Provider Directory; you can call Member Services (1-800-446-8331) to request one.

Healthy Recipe March is Nutrition Month! Looking for a new healthy dinner recipe to try with friends or family? The Arizona Nutrition Network (www.eatwellbewell.org) offers several categories of recipes (breakfast, lunch, dinner, snacks) where you’re sure to find a new favorite!

MANGO CHICKEN STIR-FRY Ingredients 1 pound boneless, skinless chicken breasts, cut into bit-size chunks ¼ cup pineapple juice 3 Tablespoons low-sodium soy sauce ¼ teaspoon ground ginger 1 red bell pepper, cut into bit-size strips 2 mangos, pitted and cut into bite-size strips ¼ cup toasted, slivered almonds Ground black pepper to taste 2 cups cooked brown rice

Directions 1. Wash hands with warm water and soap. Wash

fresh fruits and vegetables before preparing. 2. Spray a large wok or skillet with nonstick

cooking spray.3. Sauté chicken over medium-high heat until

cooked through, about 10 minutes.4. In a small bowl, stir together pineapple juice,

soy sauce, and ginger. Add sauce and bellpepper to the skillet.

5. Cook and stir for about 5 minutes untilpeppers are crisp-tender.

6. Add the mango and almonds to the wok orskillet and cook until hot. Season with groundblack pepper to taste.

7. Serve each cup of stir-fry over ½ cup brownrice. (Makes 4 servings, 1-1/2 cups each)

CORRECTION In our 4Q 2017 member newsletter discussing flu shots being free of charge, we inadvertently indicated “both pneumonias and HepB vaccines are also no charge.” For clarification, HepB vaccines are free only if the patient is deemed high or intermediate risk by the PCP. If you are not in this category, a tier 3 HepB vaccine will have a $45 copay for Classic; $40 for Plus. Please refer to Chapter 4 in the EOC (page 66, Immunizations) for more information.

The BCBSAZ Advantage Care Management Department appreciated the opportunity to share their preliminary ideas – and receive MAC feedback – on a new packet of information to benefit those who serve as caregivers. As the number and need for caregivers is growing, the packet will include information and tips on caring for a loved one; national, state, and local resources; and a personal health record to help keep track of medical information in one spot. Jon Desjardins, Medicare Advantage Program Director of Imaginarium at Banner Health, has the same vested interest in Medicare Advantage members and introduced the program to the MAC. He is leading a member council similar to ours and will be sharing surveys with our MAC members to elicit their opinions and insights and will be attending future West Valley council meetings.

New Benefit Refresher Classes for current Maricopa County members began in mid-November and are scheduled through January.

These educational sessions are designed to help you better understand your health plan benefits for 2018 and will help you get the most out of your plan. We look forward to seeing you at one of these informative sessions.

Maricopa County MAC Member Profile: Michael Feist

Where are you from Mike? Originally a native of Philadelphia, I’ve been anArizona resident since 2013.

What did you do in your former life? I got really lucky in the 70s. I had a boss who asked me if I wanted to learn about computers so they sent me to school to learn about computers. I’ve worked as an IT consultant for many years with a company that built nuclear power plants. I also worked for S & H Green Stamps in Elmhurst, IL and Gold Bond Stamps based out of Minnesota where I converted COBOL systems to Unix.

How do you occupy yourself now Mike? Presently, I work for Meals on Wheels. And, since I had a serious illness in 2010, I take very good care of myself – I swim 15 laps a day and enjoy the sauna; I also have the Silver&Fit exercise DVDs. I also play cards and shoot pool several times a week.

Why did you want to join MAC? It all has to do with purpose. I joined MAC because I enjoy being involved and knowing what’s going on with my insurance company and the whole health industry. MAC talks about interesting things and we are able to move forward with some of the MAC committee suggestions like the website enhancements.

LOCAL FOCUS: TEMPETempe, a city with a population of about 190,000 residents, is part of Dementia Friendly America, the biggest initiative to change people’s perceptions of dementia – it aims to transform the way the community thinks, talks, and acts about the disease. There are currently about 1,500 people over age 65 in Tempe who have dementia.

From education classes, to safety programs and support groups, Dementia Friendly Tempe offers extensive resources and trained Champions who are available to help neighbors, business associates, church members, and other community members become Dementia Friends to make Tempe a livable city for people living with dementia.
