The State Employee Injury Compensation Trust Fund - SEICTF - was created by the Alabama Legislature to be effective October 1, 1994. Its purpose is to provide indemnity and medical benefits for injuries incurred on the job. In this section we will discuss the procedures you are to follow if involved in an on-the-job injury.2Safety First!In a severe medical emergency, seek treatment immediately. Proceed to the nearest emergency room and let them know that you are a state employee and the medical emergency is an On The Job Injury. After any emergency room treatment you must follow-up the next business day with a network gatekeeper provider.Non-life threatening injuries will be treated by a SEICTF network gatekeeper provider.

But, first things first. Your safety and well-being is primary! (read verbatim) After emergency room treatment you must follow-up the next business day with a network gatekeeper.3Injury Incurred While Driving Seek treatment, if needed, immediately.Notify your immediate supervisor.Obtain a copy of the police report as soon as possible to be submitted to your supervisor.A copy of the report must be faxed to SEICTF and to the ABC Personnel office.

If an accident occurs while you are driving on state business you should (read verbatim)4When injuries happen

Sometimes when injuries occur on the job employees are hesitant to make a report thinking that the injury is minor. However, later on the injury may develop in severity resulting in the need for medical care. All injuries, regardless of how minor you may think the injury is, should be reported to your immediate supervisor. 5If you are injured at work

Report any injury immediately to your supervisor no matter how minor you may think the injury is!Your supervisor will provide you with an injury packet.If the injury is serious, dont delay treatment. Go to the Emergency Room!

When an injury occurs, a First Report of Injury form must be completed by your supervisor. You must complete an Accident Report which is your statement of how the injury occurred and what injuries were sustained. You must provide these forms to your immediate supervisor.7Authorization for Initial TreatmentPharmacy

You must also complete the Authorization for Initial Treatment. Your supervisor will also have a section to complete on this form. You will take this form with you for treatment by your medical care provider who will complete the designated section on the form. Once this form has been completed the employee will submit the form back to the supervisor. The gatekeeper will also provide the employee with a work slip following any follow-up visit for treatment that must be returned to the supervisor as well. The supervisor will review any activity restrictions to determine if accommodations can be made. This form includes a section of instructions for any pharmacy where you may have prescriptions filled relating to your injury. If any problems occur at the pharmacy in filling prescriptions you must call SEICTF directly for help.8Exposure to blood or body fluidsIf you are exposed to blood, body fluid or other potentially infectious materials do the following:Clean the site immediately;If you still feel uncomfortable and want treatment, go to the nearest SEICTF network gatekeeper for immediate treatment;Complete the Blood/Body Fluid Exposure Report as soon as possible.

Your job involves contact with people on a daily basis. Being aware of your customers, and observant of whats happening around you is important. In the event an unusual occurrence of you being exposed to blood or other body fluids happens, you should do the following: (read verbatim)9Complete Necessary Forms

The Blood/Body Fluid Exposure Report must be completed. This report is in two parts. Part 1 must be completed by your supervisor and Part 2 must be completed by you. You must also complete a First Report of Injury form. The supervisor will be responsible for faxing both completed forms to SEICTF and to the ABC Personnel Office.10GatekeepersEmployees who are injured on the job receive medical care from an approved network health care provider (physician or hospital). SEICTF is responsible for payment to the medical care provider. The employee will not be charged for co-payments or deductibles. And, employees participating in the SEICTF program are not required to provide an ABC medical validation form in order to return to work.

Election for Lost Time Benefits...If your on-the-job injury results in you missing more than 3 days of work, your options for lost time benefits are:For the first 3 days you should:Use available annual/sick leave, orTake unpaid days.After 3 days you should:Take SEICTF benefit of two-thirds pay with no deductions, orTake available annual/sick leave.

Private Healthcare ProvidersInjured employees may choose not to participate in the SEICTF program and may choose to be treated by their own private healthcare provider. Employee will be required to pay all co-pays and deductibles.Employee will not be allowed to return to work until such time that the private healthcare provider validates on an ABC medical validation form that the employee can perform all essential functions of the job.

Whos responsible?It is your responsibility to make sure the necessary forms are completed and returned to your supervisor.Your supervisor will be responsible for communicating with SEICTF and the ABC Personnel Office and faxing all necessary forms.

Who is responsible?In order to receive the benefits provided through SEICTF in the event that you are injured on the job, you and your supervisor MUST follow the procedures as outlined in this presentation. Failure to follow these procedures may jeopardize your benefits and may lead to disciplinary action for you and your supervisor.
