OFFICIAL 2018 READ & RIDE REPORT FORM Read 3 books, earn 3 carnival rides FREE! It’s as easy as 1-2-3!

1. Read three (3) books approved by your teacher/guardian as appropriate for your age and reading level.

2. Complete brief summaries of each book below. An adult must sign this form to verify you completed the books.

3. Bring the completed form to the Guest Services booth during the AZ State Fair. The AZ State Fair is openWednesdays through Sundays, October 5- October 28, 2018. Refer to the official program for location of the GuestServices booth.

Who Can Participate? Any Arizona student age 5-14 years can participate. Children age 5-6 may draw pictures on the back of this form instead of writing summaries. Student MUST be present to redeem form for ride passes.

Restrictions: This form may not be exchanged for cash rides. Forms will NOT be provided at the Fairgrounds. A maximum of three (3) rides are earned by reading three (3) books. Only one (1) form accepted per child per day.
