    1. PURPOSE

    This procedure defines the methods of working at height on Saudi Arabian Saipem drilling sites and toassure specific induction and orientation training to all personnel working at height. This procedure covers inshort terms, the key requirements with regard to preventative measures, equipment used, inspectionrecords, and working conditions. The provisions of this procedure apply to all work at height where there is arisk of a fall  likely to cause personal injury. The procedure places duties and responsibilities on Saudi Arabian Saipem Ltd. Drilling Rigs and other job categories.

    The present procedure applies to all personnel involved in operations require working at height.

     ABSTRACT: The purpose of this procedure is to eliminate and to mitigate the specific risks factors whichappear at work at height.

    This procedure contains provisions and health & safety settlements to prevent specific accidents for work at

    height. It intends to define the Rules for any type of work performed at heights more than six (6) feet aboveground and/or floor elevations.

    OBJECTIVES: The main objectives of this procedure shall be, but not limited to the following:

    •  To ensure the hazards related to working at height on SAS drilling rig sites are well identified.

    •  To provide necessary guidelines to all personnel involved, for the safe working at height, erectionand use of scaffolding on drilling site;

    •  To ensure that drilling crew personnel and other workforce involved, in the drilling and subsequencesworks are aware of and consider the hazards involved;

    •  To ensure that drilling crew personnel and other workforce involved, take necessary precautionarymeasures and adhere to the relevant Safety Rules and Regulations;

      To eliminate causes, prevent accidents and incidents resulted due to incorrect handling tubular ortools and or use of scaffolds.

    DEFINITIONS: Work at Height means working in a place (except a staircase in a permanent workplace)where a person could be injured by falling from it, even if it is at or below ground level. Access to and from aplace of work can also be classed as Work at Height.

    Examples of work activities that are classified as Work at Height:

    •  Working on a Flat Roof;

    •  Working on a Ladder or Stepladder;

    •  Working at ground level adjacent to an excavation;

    •  Working on formwork within an excavation;

    •  Working near or adjacent to fragile materials;•  Rigging up;

    •  Assembly of the Rig:

      Setting up the Substructure;  Setting up Rig Floor and Mast or Derrick;  Installing Handrails, Guardrails, Stairs, Walkways, and Ladders;  Installing Power Systems;  Rigging up the Circulating System;  Installing the Auxiliary Equipment;

    •  Inspecting the Rig;

    •  Rigging Down;

    •  Derrick or Mast Maintenance and repairs activities.

    3. RESPONSIBILITIES

    PROJECT M ANAGER 

    The Project Manager shall be responsible for the implementation of this procedure and to monitor that allpersonnel unfamiliar with the Drilling Site Elevated Areas undergo the specific induction training, as specifiedwithin this procedure. He shall ensure the implementation of this procedure.

    DRILLING SUPERVISOR

    The Drilling Supervisor (or his delegated person) shall be responsible for the implementation of thisProcedure, within his areas of responsibilities in the Project. He shall ensure that the Toolpushers and otherpersonnel involved in working at height and scaffolding works are aware of the hazards and they arequalified, trained and well experienced.

    TOOLPUSHER 

    The Toolpusher is responsible for performing the induction HSE briefing to all the drilling personnel beforethey commence to work in the drilling related areas.

    He is also responsible for:

    •  He shall be fully responsible to apply safe procedure by the drilling personnel for working at height;

    •  He shall be accountable for ensuring the Safety of his workers and familiar with all the precautionaryrequirements;

    •  Ensuring the Work Permit System (where applicable), is strictly adhered to (Issuing, Validating,Renewal of Permit, etc.) for all works performed at heights and of all scaffolding activities on the site;

    •  Ensuring that his Drilling Crew is properly using suitable Personal Protective Equipment (PPE)whenever and as required.

    DRILLER 

    The Driller is having full responsibility and authority in performing activities in a safe manner during his shift.

    For work at height activities related to drilling rig he must be informed by the personnel appointed to performthe task and must ensure that personnel is properly prepared to perform the task, dressed with correct PPEand instructed accordingly.

    HSE COORDINATOR 

    The HSE Coordinator shall be responsible to the following;

    •  To advise Line Management to comply with Procedure while working at height;

    •  To advise Management to provide required PPE and Safety Devices to the workforce;

    •  To arrange monitoring of drilling and subsequences activities.

    •  To verify compliance to the present procedure by means of site inspections and internal audits

    S AFETY OFFICER 

    •  The Safety Officer  shall be responsible for ensuring the new personnel on the drilling site are madeaware of the details containing this procedure.

    •  He must ensure that new arrival personnel on rig site are familiar with equipment installed and withprovision of the present procedure.

    •  He shall monitor all the drilling activities on site (e.g. Drilling Crew to wear the proper PPE whencarrying out works at height).

     ALL PERSONNEL 

    •  All Personnel are responsible to comply with the requirements of the present procedure.

    4. WORKING AT HEIGHT PROCEDURE

    F ALL FROM HEIGHT 

    There are many different factors that may contribute to falls from heights such as:

    •  Failure to identify where employees may be at risk;

    •  Failure to provide safe access to and from the place of work;

    •  Failure to provide safe systems of work;

    •  Failure to ensure that safe systems of work are followed;

    •  Failure to provide adequate information, instruction, training or supervision;

    •  Failure to provide adequate or specific medical fitness examinations;

    •  Failure to use appropriate equipment;

    •  Failure to provide safe plant / equipment

    GENERAL REQUIREMENTS 

    SAS will implement appropriate measures to mitigate the risk of working at height. This procedureestablishes simple rules for managing Work at Height:

    •  Avoid work at height where this is reasonably practicable;

    •  Use work equipment or other measures to prevent falls where you cannot avoid working at height;

    •  Where you cannot eliminate the risk of fall, use work equipment or other measures to minimize thedistance and consequences of a fall;

    •  All work at height will be properly planned, organized, supervised and carried out safely;

    •  The place (drilling areas) where work at height is done is safe;

    •  All Work at Height take account of weather conditions;

    •  Those involved in Work at Height are instructed and trained;

    •  Equipment for work at height is appropriately inspected;

    •  The specific risks from unstable and fragile surfaces are properly controlled and assessed;•  Injury from falling objects is prevented;

    •  Safety Harness shall be used by everyone who is required to work at height, to perform drilling ormaintenance operations / activities;

    •  In order to minimize the exposure of workforce to work related risk, the minimum practical number ofpersonnel shall be assigned to the job

    •  Good Safety Supervision will be assured by Operation Personnel;

    •  The personnel working on the rig responsible to stop the work whenever the provisions of the presentprocedure are not followed.

    •  The operations above ground level shall be carried out in such a way that non-involved personnel, in thevicinity of the activity, are not at risk;

    •  Suitable access and platform to be provided for working at height;

    •  For all the operations will be used a Proper and Safe Route – Appropriate Access Way or Ladder;

    •  No loading is allowed on derrick/mast subassembly with materials or tools;

    •  Approved type man working basket to be used for lifting the personnel to the derrick/mast subassembly,to complete maintenance operations;

      All work areas and access ways, when night work will be carried out, shall be provided with adequatelighting, which shall be properly maintained during all the drilling operations;

    •  On the bad weather conditions with possible thunderstorms, work at height not allowed due to the risk oflighting;

    •  No part of any operational equipment (Drilling or Escape Line) in a drilling process shall be used as asupport, or tie point for any scaffolds or platforms;

    •  The Rig Line Management will ensure that an Emergency Escape Device is installed on the Drilling Rig;

    •  SAS will develop, implement, and enforce a written comprehensive Safety Programes, which will be inaccordance with other Policies or Procedures (e.g. Dropped Objects Policy);

    •  Rig up derrick in a proper manner;

    •  History of Rig will be recorded and updated.

    Falls from derrick or mast systems, ladders and roofs still account for the majority of falls. Identifying fall

    hazards and deciding how to protect workers is the first step in reducing or eliminating fall hazards. Thereare a number of ways to protect workers from falls including conventional systems such as guardrailsystems, safety net systems and personal fall protection systems (fall arrest systems, positioning systemsand travel restraint systems) as well as through the use of safe work practices and training. The use ofwarning lines, designated areas, control zones and similar systems are permitted in some situations and canprovide protection by limiting the number of workers exposed and instituting safe work methods andprocedures. These alternative systems may be more appropriate than conventional fall protection systemswhen performing certain activities. Whether conducting a hazard assessment  or developing acomprehensive fall protection plan, thinking about fall hazards before the work begins will help to managefall hazards and focus attention on prevention efforts. If personal fall protection systems are used, particularattention should be given to identifying attachment points and to ensuring that employees know how toproperly done and inspect the equipment.

    PLANNING, ORGANIZATION AND RISK ASSESSMENT 

    The subjects to be covered by the Toolpusher and Safety Officer will, in the main, consist of:

    •  Ensure that work is not carried out at a height where it is reasonably practicable to do it in another way;

    •  Ensure that work is properly planned, appropriately supervised, and carried out in as safe a way as isreasonably practicable.

    •  Carry out a workplace Risk Assessment and take account of this in planning the work.

    Risk Assessment of Derrick / Mast work at height will be done (e.g. In order to eliminate the Hazards relatedto work on A-Frame during installation of Bridle Lines – it is recommended to use a Mobile Crane SuspendedManbasket as per to provide more safe conditions for the employees who work at height). JSA will beprepared for critical jobs during Rigging-Up, Installation Services and Rig Down / Move, as per to ensure thatall Hazards are identified and Risks are mitigated. Toolbox Meetings will be held before any Pre-Mobilization/ Mobilization Critical Operations in order to discuss the relevant JSA’s for Routine or Non-RoutineOperations on Rig site.

    Periodical safety training Course “Fall Protection” will be provided for the rig personnel.

     Additional Fall Protection Systems for the Rig Move (Horizontal Lifelines Systems) will be recommended andinstalled on Rig.

    Rig Management will ensure that the place where work is carried out at height (including the means ofaccess) is an existing place of work or means of access that meets the Safety requirements, unless thiswould mean that it is not reasonably practicable for the worker to carry out the work safely.

    RISK ASSESSMENT FOR WORK AT HEIGHT 

    The Work at Height Plans or Programs will be based on a Risk Assessment Approach. When consideringWork at Height, a Risk Assessment should be carried out in order to identify what the hazard is and thedegree (value) of risk present.

     A Risk Assessment is a careful examination of what could cause harm to people as a result of a workactivity. The value of it allows to take the precautions and measures to prevent and mitigate the harmoccurring

    In the Risk Assessment Process, SAS will need to:

    •  Look at the hazards;

    •  Decide who might be harmed and how;

    •  Evaluate the risks and decide whether the existing precautions are adequate or whether more should bedone;

    •  Record the findings;

    •  Review the assessment.

    Regarding the Process, SAS will consider the following topics:

    •  The work activity;

    •  The equipment to be used;

    •  The duration of the work;

    •  The location of the work activity i.e. presence of hazards such as lifting operations, derrick / mastmaintenance activities, run the pipe, open excavation, underground services overhead power lines, etc;

    •  The working environment, e.g. weather conditions, lighting;

    •  Condition and stability of existing work surfaces;

    •  Derrick / Mast stability;

    •  Physical capabilities of the workers (e.g. Vertigo suffers)

    PLANNING OF WORK AT HEIGHT  Any Work at Height (even specific routine drilling operations) needs to planned in advance of the workactivity, giving careful consideration to the selection and use of the work equipment so that a safe system ofwork is adopted. This safe system of work needs to take account of:

    •  Any supervision of workers that may be necessary, e.g. work equipment selected lower down thehierarchy of control, such as Fall Arrest Equipment, will require a high level of supervision;

    •  Any weather conditions that workers may be exposed to, e.g. carrying out work on a sloping Rig Structurein icy or rainy conditions;

    •  Any EMERGENCY or RESCUE Procedures that may be required, e.g. if a person falls while using a Fall Arrest System. It is not acceptable to rely on the Emergency services. It needs to be covered in the Risk Assessment and planned prior to the start of the work activity. The user of Fall Arrest Equipment and asufficient numbers of persons must have received adequate Training in the use of the equipment and thisincludes Immediate Rescue Procedures. Examples:

    -  Rescue of person after falling in a fall arrest harness;-  Rescue of unconscious or injured person;

    -  Use of a Man rider Air winch with stretcher Basket to undertake the rescue;

    -  Medical Examinations for Emergency Response Team members

    WEATHER CONDITIONS 

    The Toolpusher must ensure that the work is not carried out when weather conditions endanger Safety orHealth. Employers are required to ensure that Work at Height is carried out only when weather conditions donot jeopardize the Safety or Health of employees.It is totally forbidden to Work at Height (on Rig Structure or on the roof of Highly Hydrocarbons Tanks) in thefollowing conditions;

    •  Especially Meteorological Conditions:-  Strong Wind direction;

    -  Heavy (Torrential) Rains;

    -  Thunderstorm Lightning;-  Frog (Poor Visibility);-  Desert Sand Storm;-  Strongly Frost-  Very Hot Barren Climate (above 55 degree Celsius).

    •  Slippery Rig Structure (dirty derrick systems with oil, grease or other substances; frozen or snow whiteelements)

    •  During the Night Shift in case that are not assured the minimal lightning working environment conditions

    Protective Gloves, Sun Block or Non-slip Footwear may help, but the Risk Assessment should considerwider aspects for example:

    •  Bulky Clothing can get caught in machinery or make access more dangerous;

    •  Build up of Mud on Ladders will make them less safe as would placing them on a surface that mightbecome slippery;

    •  Extreme Heat can make people exhausted more quickly and may increase human error;

    •  Wind may reduce the stability of Working Platforms and as such the Manufacturer’s Specifications forSafe Operating conditions should always be adhered to;

    •  Wind can blow materials such as Sheet Insulation, Decking and Timber off edges and such materialshould be secured to prevent it being blown away by the wind.

    The Risk Assessment and Planning arrangements should take into account the effects that the weatherconditions can have on outdoor Work at Height

    INDUCTION,  COMPULSORY TRAINING,  PRACTICE AND USE OF COLLECTIVE AND PERSONAL PROTECTIVEEQUIPMENT (PPE), AND SUPERVISION 

    The rig tool pusher will ensure that everyone involved in the work has received adequate training andinstruction to prevent them being injured in a fall.Where the precautions do not entirely eliminate the risk of a fall occurring, you must (as far as it isreasonably practicable to do so) train and instruct those who will be working at height. The HSE training andinduction program will cover the following topics:

    •  How to avoid falling;

    •  Use of equipment safely;

    •  Carry out any necessary checks;

    •  Avoid or minimize injury to themselves should they fall;

    •  Rescue others or assist in their rescue, where necessary;•  What to do if they find defective or inadequate equipment.

    TRAINING REQUIREMENTS 

     All workers need the appropriate knowledge, skills and experience to work safely, or be under the

    supervision of someone else who has it. They need to be able to recognize the risks, understand theappropriate systems of work and be competent in the skills to carry them out such as:

    •  Installing and wearing Harness Systems;

    •  Installing life line system;

    •  Operating a man riding winch.

    Training will usually be required to achieve these competencies. It is not sufficient to hope that workers will“pick up safety on the job”.

    RISKY SURFACE AND D ANGER AREAS 

    The rig tool pusher must ensure that no employee goes near a risky surface unless that is the onlyreasonably practicable way for the worker to carry out the work safely.Where anyone does work on or near a fragile surface rig tool pusher must

    •  Ensure (as far as it is reasonably practicable to do so) that suitable platforms, covers, guard rails, orequivalent are provided and used to minimize the risk;

    •  Minimize the distance and effect of a potential fall, where any risk of a fall remains.

    If an employee may go near a fragile surface or danger area, Rig tool pusher must ensure that the employeeis aware of the danger, preferably by using prominent warning notices fixed at the approaches to the dangerzone and that the area is equipped to prevent unauthorized employees from entering the area.Rig Management must provide adequate supervision for the employees to ensure that the work is carried outsafely

    The different types of Fall Protection are to be used while working at height.

     Access and Egress

    If workers are required to work at the rig floor, mud tanks and super sack mixing systems or in areas wherethere is the risk of falling, a safe method for workers to get to and from, and move around that work areashall be provided. Personnel who intend to work at derrick must record on derrick access log book scope ofwork and tools carried on derrick.

    This should take into account the tools and equipments they may be required to carry to and from, andaround the work area.

    In providing safe access, aggress and movement around a work area, consideration should be given to:

    •  The installation of fixed working platforms, walkways and stairways;

    •  The use of temporary work platforms such as:

    •  Single man Work basket;

    •  Two man basket;

    •  BOP working platform;

    •  Scaffolds;

    •  Ladders;

    •  All access ways are clear of obstructions and readily accessible.

    Guardrails

    Standard guardrails shall be installed on the outer perimeter of all working platform and walkways that areover 1.2 Meters above the ground level.Guardrail requirements:

    •  A top rail 1.1 m in height.

    •  An intermediate rail located an even distance between the top rail and the floor.

    •  A toe board mounted flush with the floor and a 10.2 cm

    When it is not possible, during a temporary condition to provide adequate railing all personnel workingwithin 1.8 meters (6 feet) of the edge must be tied off with safety harness.When a chain or wire rope is used as a temporary substitute for a guard rail on mud tank walk walkways, thechain or wire rope shall be adequately secured and kept taut.

    Floors All holes openings in floors should be protected and covered with material of adequate strength to prevent byobjects or persons and be securely fixed. Any holes or openings covered with wire mesh should not be usedas a work platform. A sturdy guard rail shall be provided at the V-door. It shall be in place at all times except the use of chains onwide spans, such as V-doors, is not allowed. Care must be taken when operations are being carried out onthe area.

    Grid Mesh and Checker Plate Flooring

    Grid mesh and checker plate flooring used for walkways, access ways and working platforms should besecurely fashioned.

    Where grid mesh or checker plate flooring is used it should be securely fixed and assembled. During theinstillation, this flooring should be secured by tack welding, panel grips or other means to prevent movementbefore being permanently fixed.

    The fall protection system for the stabbing consists with:

    •  Standard guard rail, intermediate rail, and toe board protection on each side and back of the

    platform.•  The fall protection is required to be insulted for workers working on

    •  A non-skid floor surface

    •  A secondary safety system to support the carriage if the winch or winch cable fails

    •  A full body harness for the stabber on the platform

    •  The Stabbing Board Frame shall be fastened to the derrick or mast with a wire rope at least 12.7millimetres (1/2 inch) in diameter

    •  A fall Arrestor shall be attached to the mast above the stabbing board

    The stabber also has to accomplish physically demanding task, such as positioning, opening and closingelevator on the casing. The stabber performs this job while tethered by a harness and standing on a stabbingon a stabbing board, some 30 to 40 feet above the rig floor.

    •  Two safety tail ropes; one end attached to the anchor railing of the monkey board, another end to thederrickman’s wais strap D-rings. This devise is part of the Restraint System, which prevents thederrickman to reach a fall hazard area on the end of the diving board and allows him free use of both

    ends.•  The Fall Arrestor (Self Retracting Lanyard) shall be suspended above the Derrick man working on

    the monkey board. This designed to suspend the derrickman in case of accidental fall.

    There are two ways to setup the Fall Arrestors:

    Two fall Arrestors shall be installed above the monkey board, one for derrick man and another on for hisassistant.

    The swing falls can occur when the fall arrestor is not anchored directly above the Derrick man. The force ofstriking an object in a pendular motion can cause serious injury. Always minimize swing falls by working asdirectly below the fall arrestor (anchorage point) as possible.

    Swing fall hazard must be minimized by anchoring directly above the Derrickman work space.

    BOP working platform fall protection

    The BOP working platform is one of the hazardous work areas on the rig. An inadequate fall protection could

    cause severe accidents to personnel. The major hazards related to the work are:

    •  Work At height

    •  Hazard of Falling Objects

    The BOP ladders shall be securely fixed to the anchor beams with safety pin, in order to avoid accidentalfalls.The BOP working platform shall be adequately set on the BOP ladders.The four fall arrestors shall be installed above the BOP working Platform in order to allow workers freemovement around the BOP.

    Man riding and Man basket operations

    Man riding operations should only be seen a last resort for carrying out work at heights. It is alwayspreferable for person to work from stable work platform.The use of air hoists for man riding operations should be undertaken in a safe and controlled manner due tothe inherent risks (being pulled into the equipment, falling out of the harness, equipment failure).Were air hoists are used for man riding operations, the following points should be considered.

    •  All man riding operations to be planed assessed and Work permit to be opened.

    •  Only certified man rider winch to be used for man riding operations

    •  Only full body riding belt or where possible, use certified single man work basket as an alternative toriding belt.

    •  Visual check shall be carried out for the man riding belt

    •  The clear instruction must be given to the rider for the task to be done

    •  Top drive must be remain stationary while man riding is in progress

    •  Tag lines shall be used to control and prevent accidental swings of the rider.

    •  All movement of the drill pipes and blocks to be stopped

    •  Air winch operator shall keep visual contact with rider. If this not possible a third person shall beused.

    •  Recommended to have radio communication between operator and rider

    •  Secondary fall protection such as fall arrestor shall be used•  Secondary fall protection is required for all man riding operations, the lifting line is the working line

    and not considered secondary fall protection, nor can it be part of the fall protection system.

    •  BOP Ladders securely fixedto the anchor beams.

    •  Four fall arrestors to beinstalled above the BOP

    MOBILE ELEVATING WORK PLATFORMS (MEWPs)

    MEWPs may provide a safe means of working at height if used properly in accordance with their Instructions.Saudi Arabia Saipem Ltd. and others responsible for the use MEWPs will assess the risks of users fallingfrom or being thrown from the Basket and take precautions to eliminate or control those risks.The precautions for safe work from MEWPs include:

    •  A guard rail and a mid rail round the edge of the Basket to stop the user falling;

    •  A slip resistant floor;

    •  Toe-boards round the edge of the Platform;

    •  Dead-man controls clearly marked to show their method of operation;

    •  Use of stability devices, e.g. outriggers, provided to make the machine stable, which are interlockedsuch that the MEWP will not operate unless they are fully extended;

    •  Locking out controls (other than those in the Basket) to prevent inadvertent operation.

     A Safe System of Work will be in place and includes:

    •  Making sure that the MEWP selected is suitable for the task;

    •  Consideration of access to and exit from where the work is being carried out;

    •  Planning the job to address the risks from overhead hazards and passing traffic, includingprecautions to prevent collision;

    •  Use of trained / Experienced Operator(s);

    •  Use of Harnesses;•  Instructions to the workers about Safety issues;

    •  Instructions in Emergency Procedures, such as Evacuation, should power be lost.

    The MEWP’s will have an examination at least every SIX months. Regular Maintenance will be performed inaccordance with the Manufacturer’s Manual and Instructions and the advice of the Competent Person

    INSPECTION 

    Rig Management must ensure that for all equipment will apply:

    •  Each individual place at which work is to be done at height (including parapets, permanent rails andfall protection measures) is checked visually on every occasion before that place is used;

    •  Equipment is inspected before being used after Rig / Structure installation or assembly, where itssafety depends on how it is assembled or installed;

    •  Equipment exposed to conditions causing deterioration is inspected at suitable intervals to ensureSafety (please refer to SAIPEM HSE Procedure on “Work Site Inspections”);

    •  Equipment exposed to exceptional circumstances liable to cause danger is inspected as soon as

    practicable;•  Equipment must not be used in a workplace unless it is accompanied by evidence of the last

    required Inspection;

    •  Where specified by Site requirements, a formal “Work at Height” equipment and ScaffoldingInspection System, e.g. ”Critical Equipment”, will be established, used and correctly documented;

    •  Inspections must be carried out by the Competent Person.

    These requirements will apply to: Guard-Rails, Toe-Boards, Barriers, Working Platforms, or other CollectiveSafeguards, Personal Fall Protection Systems, Work Positioning Systems, Rope Access and Positioning,Fall Arrest Systems, Work Restraint Systems and Ladders.

     All the Fall Protection measures of every place of Work at Height will be “checked” prior to use and at thecommencement of each shift (e.g. for the Derrick man). This requires visual check to ensure the appropriatesafeguards are in place. These checks do not have to be recorded.

    Saudi Arabian Saipem Ltd. will ensure that the results of an inspection are recorded. The record of theinspection must kept available for inspection by a body authority member for FIVE years from the date ofinspection Records may be kept electronically if they are secure and capable of being printed out.

    W ARNING SIGNS 

    When a “Work at Height” equipment or scaffolding is in use, a Warning Sign with the text “WORKOVERHEAD” or “CAUTION” will be mounted where the work is carried out and / or people are passing underthe working areas. Red and White Tape Barriers may also be used.

    F ALL ING OBJECTS 

    Where it is necessary to prevent injury to an employee, Rig Management must:

    •  Do all that is reasonably practicable to prevent any object or material from falling;

    •  Where is not reasonably practicable to prevent objects from falling you must ensure that no one isstruck and injured by anything falling;

    •  Ensure that nothing is thrown or tripped from height if it is likely to cause injury;

    •  Ensure that material is stored so that the risk of injury from collapse, overturning or movement isprevented.

    5. EMERGENCY PROCEDURES

    CONSIDERING SPECIFIC EMERGENCY PROCEDURES 

    Emergency Procedures need to be considered for reasonably foreseeable circumstances, such as stuckaccess equipment or deployed Fall arrest equipment, in order that a person or persons can be rescued.

    The method selected needs to be proportionate to the risk and there should not be reliance on theemergency services. The method of rescue may be simple, such as putting a Ladder up and allowing thefallen person to descend, or pulling a worker hanging on a deployed Lanyard onto the surface below.

    In other situations, consideration may need to be given to the use of other work equipment, such as MEWPsor Man basket with Air winch. Whatever the situation presented, Saudi Arabian Saipem Ltd will be able todemonstrate that if someone falls, that there are arrangements in place that are capable of rescuing thatperson and that those involved are trained in the Procedures and the equipment required is available and

    well maintained.

    •  In an Emergency, the Derrickman can quickly reach the ground by an Escape Line  often calledGeronimo.

     AUXILIARY ESCAPE FOR DRILLING OPERATIONS 

    The Derrickman is one of the Drill Rig Crew members who get his name from the fact that during trips heworks on a Platform (called Monkeyboard) attached to the Derrick or Mast, typically 85 ft (26 m) above theDrill Rig Floor. According to derrickman duties and Tasks – align and manipulate sections of pipe or drill stem from aPlatform on the Rig Derrick during the removal and replacement of Strings of Pipe, or Drill Stem and DrillBits, on all SAS Rigs, the Derrick or Mast will have an auxiliary means of escape installed prior to personnelworking in the Derrick. The Auxiliary Escape route will use a specially rigged and securely Anchored EscapeLine attached to the Derrick or Mast so as to provide a ready and convenient means of escape from theDerickman’s Working Platform. The Escape Line route will be kept clear of obstructions.

    a) The Escape Line on Masts or Derricks should be a 1/2” minimum diameter wire rope in good condition. Aevacuation carriage with braking system equipped will be attached and run on the escape. Thesecondary protection device Trollmatic inclined plane descender ( DBI SALA/POTECTA AG399 99m)

    will be installed on derrick and attached to the evacuation carriage. kept at the Derickman’s WorkingPlatform, and secured in a manner that will release when weight is applied.

    b) Tension on the Escape Line will be periodically checked and adjusted to enhance safe landing of theuser. Tension should be set with SIX to TWELVE feet of sag in the middle, depending upon the length ofcable run. It is recommended that the ground anchor point of the escape line should be located aminimum lateral distance from the Derrick or Mast equal to TWO times the HEIGHT of the WorkPlatform. The ground anchor point should be able to withstand a pull of at least 3,000 lb (1350 Kg). If theRig configuration or location configuration will not permit use of the Escape System, an alternate meansof Fast Emergency Exit from the Derickman’s Working Platform to a safe place will be provided.

    Personnel shall not ride the Safety Buggy or Escape Equipment except in an Emergency. Personnel will betrained in the proper Procedure(s) for Escaping the Derrick or Mast according Geronimo manufactureroperators Manual.

    c) Evacuation

    •  Signs, signals, announcements and alarms.

    •  Escape routes and Muster point locations.

    •  Safety (Critical) equipment (e.g. breathing apparatus, etc.).

    d) First Aid

    •  Description of medical equipment on Drilling Area.

    •  Action in emergency, switch off machinery, call for the Site Doctor.

    •  Handling stretcher case within Drilling Area.

    e) Communication Facilities

    •  Description of Drilling Area communication equipment and systems.

    •  Portable VHF in restricted areas

    Note: Safety glasses shall be worn or keep face to one side, angled down, and well forward when ridingGeronimo escape device. Friction between brake shoe and cable creates bronze shavings which may lodgein unprotected eyes.

    RESCUE INTERVENTIONS 

    The many workers naturally assume that, once a fall has been arrested, the fall protection system hassuccessfully completed its job. Unfortunately this is not the case. A worker suspended with legs dangling in aharness of any type is subject to suspension trauma.

    Prior to attempt or perform a rescue. It is imperative that all rescue team members working at heights, beattached to proper fall arrest. This is the only way of protecting the rescuers from possible fall injuries and

    compounding the situation.

    1. ALERT THE RIG CREW. Immediately after recognizing an emergency situation it is imperative thatthe first action is to alert other members of the rig crew, the tool pusher and other personnel on site.This is accomplished by announcing by intercom system

    2. STOP!!! Tie down the brake handle. In all cases of a worker sustaining a fall or unconscious, all workbeing done in the immediate area must be terminated.

    3. ASSESS. Is the man engaged with the block? It is of utmost importance that you ensure the man isnot engaged with the block before they are moved. A worker whose fall arrest equipment is caughtup in the blocks is totally dependent on the blocks remaining stationary.

    4. CONTROL THE RIG AND THE HOLE. Shut in as per client’s requirement.5. ORGANIZE EQUIPMENT AND PERSONNEL. Rescue personnel must assemble at a rig floor and

    plan of action decided upon and initiated. The supervisor (team leader) should be in charge and

    make the final decision on information gathered and possible options discussed. The necessaryequipment should be assembled and be in place before the rescue is attempted. Failure to do thiscould result in loss of precious time and further injury to the worker and rescuers

    Riding hoisting equipment 

     A worker must not ride the traveling block hook, or elevators, nor slide down: pipe, Kelly hose, and cable orrope line.

    In an emergency an injured worker may be lowered from the derrick by means of appropriate stretcherattached to the air winch if the rotary table has been stopped and qualified worker operates the control.

    PERFORM RESCUE OPERATION

    •  One member of the rescue team should put on a general purpose harness, climb the derrick,( usingproper fall Arrest) to the level of the suspended or injured person and attempt to determine thecasualty’s condition.

    •  The rescuer should then relay this information to the remainder of the crew and waits for furtherinstruction.

    •  Needed rescue equipment should be raised to rescuer on the derrick and one more rescue memberwill climb to assist in rescue operations.

    •  Rescue team on the derrick will then affix the equipment to the casualty and directs the winch operatorto raise the casualty only as much as requires (if casualty is suspended) to release the fall arrestequipment. Place casualty face up in the basket

      Secure the restraining straps and to lock the casualty within basket•  Connect bridal straps to the air winch line

    •  Attach 2 tag lines to control the basket movement

    •  Position basket in such a way that allow air winch operator to lower the basket with low speed.

    •  Medic or rig nurse shall be ready with all necessary first aid equipments

    •  Rescue shall be completed within 20 minutes as soon as possible

    NOTE: A casualty with spinal injuries may first be installed on a backboard device prior to being laid into thebasket.
