Medical Screening: What Test Should I get?
Screening tests attempt to detect disease before systems begin. Therefore, it’s important to get the right screening test as part of a good preventative care plan. Tests should have adequate sensitivity and specificity to detect disease without excessive false positive; however, there is on-going research which conditions should be screened for and which tests to use. As a result, recommended screening tests can change with time.
The tests listed here are recommended for the general population, but your doctor may recommend other screenings based on your personal history. For example, colorectal cancer screening is recommended for both men and women ages 50 to 75 years of age. If you have a sufficient family history of colon cancer, your first screening may be earlier and the frequency of the test may be more often.
Keep in mind that no tests are perfect. Screening tests can be harmful to people. A false positive test that can show a disease when there is not one present can lead to worry, anxiety, extra testing, and high cost. A false negative test that shows normal when there is a disease present can lead to a false since of security. Some screening tests are invasive and carry a small inch of their own complication.
Even with the negative aspects, they are efficient at detecting disease and may save your life. Many diseases are easier to treat in an early stage. Early detection and treatment can also prevent other types of disease from forming.
The following are the current common preventative care screening tests recommended by the United States Preventive Services Task Force (USPSTF):• HeightandWeight• BloodPressure• AlcoholandTobaccoUse• Depression• Diabetes(especiallyifyouhavehigh
blood pressure)• BoneMineralDensityTest:Inwomen
less than 65 years old and at risk women age of 60–64 years old.

• ColorectalCancer:menandwomenages 50–75 years of age.
• Mammograms:Every1–2yearsforallwomen ages 40 years and above
• PAPSmearTest:Atleastevery3–5years until the age 65
• AorticAbdominalAneurysm:Ontimefor men during ages 65–75 who have ever smoked
• HIVScreening:Avoluntaryroutinescreeningforages13–64yearsofage

Home Care Definitions:Cardiac Care refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education.Cardiacrehabilitationisaprogramdesignedtohelpapersonwithheartdiseasereturn to health and modify risk factors to prevent further heart damage.
Caregiver Registry Services refer, but do not employ, independent caregivers. Clientscontrolallaspectsofthearrangement,includingcompensation,directlywiththeircaregivers. Registries may include free non-profit listing services or companies that require training,bondingandinsuranceoftheircaregivers.Clientsarechargedareferralfee.
Diabetic Management refers to services including patient assessment, diet information, management of the disease process, medication management and education on all oftheabove.ServicesareprovidedbyaRegisteredNurseunderthesupervisionofanM.D.Some agencies may employ certified diabetic nurse instructors or cardiac care specialty RN’s.
Home Health Care includes skilled nursing services, physical therapy, occupational therapy, speech language therapy medical social services. A home health care agency may choosetobemedicarecertifiedtobereimbursedfortheirservices.Homehealthagengiesmay also teach you how to care for your self.
Hospice is a comprehensive program of care to patients and families facing a life threateningillness.Hospiceemphasizespalliativeratherthancurativetreatment. The dying are comforted. Professional medical care is given, and sophisticated symptom reliefprovided.VeryfewHomeHealthAgenciesarealsolicensedforHospicecare.
In-Home Care/Personal Care is care related to activities of daily living, including: mobility, eating, toileting, bathing and personal hygiene, dressing and grooming or cognition support. These services are typically not covered by medical insurance and do not require a physician’s order.
Intravenous Therapy is any medical treatments/medications that need to be introducedtothebodyviaapatient’sveins.NumerousIVTherapiescanbeprovidedin-homeundersupervisionofaMD.Someoftheseinclude:inter-infective/antibiotictherapies,hydration therapy, pain management, chemotherapy and central line care and maintenance.
Medicaid Certified: A joint Federal and state program that helps with medical costsforsomepeoplewithlimitedincomeandresources.MedicaidCertifiedfacilities can offer services to individuals receiving Medicaid benefits in exchange for reimbursement from Medicaid. Some people qualify for both Medicare and Medicaid (these people are called “dual eligibles”).
Medical social services are assistance with social and emotional concerns related to an illness, including counseling or help finding community resources.

Medicare Certified facilities can offer services to individuals receiving Medicare benefits in exchange for reimbursement from Medicare. A Physicians order is required to be reimbursed by Medicare. Medicare is a Federal insurance health insurance program for those 65 and over, or who meet other criteria.
Nurse Delegated Tasks A licensed registered nurse may delegate specific nursing care tasks to qualified nursing assistants in a community-based and in-homecare settings.Onetasktaughtanddelegatedtoonecaregiverforoneclient.Beforedelegatinga task, the registered nurse delegator must determine that specific criteria are met and ensure that thepatient is in a stable and predictable condition.
Psychiatric Services refers to the agencies ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.
Respite Care is short-term or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the ill or disabled person.
Telemedicine is a developing application of clinical healthcare, where medical information is transferred and assessment derived through interactive audiovisual media. Telemedicine can also be used to conduct examinations and remote medical procedures, such as: tracking blood sugar, automated blood pressure and weight checks.
Therapy Services (ST, PT, OT, RT)
Speech Therapy (ST)� refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.
Physical Therapy (PT)� refers to services for the treatment and rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs to enhance basic mobility skills; to help lessen disability and pain and improve overall function and quality of life.
Occupational Therapy, (OT)� refers to services to assess and treat patients to help them develop,recover,ormaintaindailylivingandworkskills.Occupationaltherapistshelpclients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.
Respiratory Therapy (RT)� refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise & respiratory rehabilitation.

LTAC: You may have heard the term, but what is it?
AlicensedLongTermAcuteCarehospital(LTAC)specializesinthecareofpatientswith medically complex conditions requiringextendedhospitalizationtooptimizetheirrecovery.Servicesarefocused on improving the lives and medical conditions of patients who either require more acute care or who are not yet ready to be transferred to a lower level of care, such as a Skilled Nursing Facility (SNF) or other sub-acute facility.
What makes them different?
LongTermAcuteCarehospitalshaveestablished themselves by identifying and serving a medically complex population, filling an important role in the continuum of care. These hospitals compliment theservicesofShortTermAcuteCare(STAC)hospitals,whicharegenerallywhat people think of when they think of hospitals.STAChospitalshaveEmergencyDepartments,whereasLTACsdonot.Withoutemergencyroomsituations,theycan center on providing an acute level of care and services to patients requiring alonghospitalizationwithafocusonrehabilitating the patient back to their prior levelofcare.LTACsaddresstheneedsofa small, but growing, patient population who are too sick to be in a Skilled Nursing Facility or Sub-Acute Facility.
How long do patients usually stay?
The average length of stay for a patient inaLTACis25days.Dependinguponthe severity of illness and the progress of the patient, there are those who stay for a shorter period of time and those who stay for a longer period of time. The patient populationinanLTACisthoughttorepresent two to three percent of the total patient population, and can represent as much as 40 percent of the critical care dollars spent by acute care hospitals.

Drivingwhenyouarealittletiredisn’tthat big of a deal right? Actually -it is. A new study shows that driving while you are tired is just as dangerous as driving drunk. Many times individuals who are driving while fatigued are unaware they are tired or may be so used to driving tiredthattheydonotrecognizethedanger. The reduced alertness brought on by fatigue makes these drivers a danger to themselves and others.
Drivingwhilefatiguedissodangerous,that states have started to pass legislation to punish those who drive while fatigued. The first law of this kind was passed in 2003 and is named Maggie’s law. Maggie’sLaworiginatedin1997,fromatraffic crash in which 20-year-old Maggie McDonaldwaskilledwhenadrivercrossed three lanes of traffic and hit her car head on. The driver later admitted that he had not slept in 30 hours.
As awareness grows about the dangers of driving while fatigued, it is important that we make sure that we are not letting ourselves or the ones we love drive while they are tired.
Oneoftheprimarycausesoffatigueddriving is sleep apnea. Sleep apnea is a breathing-related sleep disorder that causes brief interruptions of breathing during sleep. These pauses in breathing canlastatleast10secondsormoreand can occur up to 400 times a night. Sleep apnea is a serious, potentially life-threatening condition that often goesunrecognizedandundiagnosed.
Becausesleepapneaaffectsyoursleep,it also affects your daytime alertness and performance. Many people with sleep apnea report falling asleep at red lights or having to pull over and take a nap because they are literally unable to stay awake.
Currentlysleepapneaisdetectedbyphysicians when patients complain that they are having trouble sleeping at night. It is estimated that over 20 million Americans suffer from the disorder. Currentlysleepapneatestingcanbetested in a sleep lab or also in the patient’s home with a home sleep testing device. If left unchecked, sleep apnea can cause hypertension, heart disease, diabetes, depression, obesity along with a major loss in quality of life. Many times the symptoms of sleep apnea are very difficult to notice. If you believe that you may suffer from sleep apnea or would just like to get tested for peace of mind, please ask your physician about sleep testing.

Onceasleepdisorderisidentified,yourdoctor may refer you to a sleep center. “Identifying and treating sleep disorders can improve one’s general health and qualityoflife,”saidDr.ThyeSchuyler,medicaldirectorfortheSalemHospitalSleepDisordersCenter.
Sometimes the problem is simply not getting enough sleep. Your mental health may decline in response to a lack of sleep. Untreated mood problems, like depression, anxiety, or extra stress, affect your quality of sleep and can be made worse by poor sleep habits. Poor sleep can also make you want to eat more than you should and make you crave unhealthy foods, increasing the risk for gaining weight.
“Weallareawareoftheday-to-dayeffectsof not getting enough sleep, such as drowsiness, mood swings and lack of focus, but many people don’t know that a lack of sleep may increase the risk for developing chronic medical conditions like diabetes and high blood pressure,” saidDr.Schuyler.
Trouble sleeping can also be a symptom of a more serious problem. Seemingly harmless symptoms like snoring and
daytime drowsiness can be indicators that you have sleep apnea. This disorder affectsanestimated18millionpeoplein the U.S. You may be at risk if you are overweight, have high blood pressure or have been told that you stop breathing during your sleep. It can be dangerous to have untreated sleep apnea, so talk to your doctor if you think you might be affected.

Gravesitesareassignedandcannotbereserved in advance. Assignments are made without regard to rank, ethnic or religious background, branch of service or factors. A husband and wife are interred in the same plot unless both were veterans. In that case, they may be buried side by side in separate plots. All veterans interred at a national cemetery are eligible for a headstone or marker. It normally takes less than 60 days following a burial for the headstone or marker to be completed and in place. At the time of the interment of the surviving spouse, a new marker will replace the previous one with the spouse’s name added.

Housing Definitions:Adult Day Care Programs offer a protective setting during the day, for those who cannot be left alone due to health care and social needs. The structured program within a communitysettingoffers:socialization,plannedactivities,careservices,companionshipandmeals.AdultDayCareprovidesdaytimerespiteforthecaregiverwhotypicallyworks,needstime for other responsibilities, to simply to take a break. Some programs are designed for older adults with dementia and/or physical limitations. Programs through senior housing communities that are listed in this guide must be able accept non-residents to be included.
Adult Care Home or Family Care Home are private residences that provides a home-like setting, and serves 5 or less residents. The caregiver may perform several functions, such as personal care, housekeeping and activities and group meals. There are threelevelsofAdultFosterCareHomelicenses.TheclassificationsystemisbasedonlevelofcaretheAdultFosterHomemayprovidetoresidentswholiveinthehomeaswellastheexperience and training of the providers and their ability to assist residents with: personal hygiene, mobility, eating, dressing, toileting, and behavior management. Residents may require very light assistance or may be dependent with several care needs; it is best to ask individual locations for their specific policies and license.
Assisted Living Communities are State-regulated rental properties where six or moreresidentsmayreside.Careassistanceisavailable,including:medicationmanagement,bathing, dressing, toileting and eating. Apartments must be fully self-contained private living units with a lockable door, private bathroom, and kitchenette facilities. The fee schedule is regular monthly rent along with additional fees for specific services and amenities. Assisted living communities are best suited for individuals who want to remain as independent as possible and who are able to direct their own care.
CCRCs (Continuing Care Retirement Communities) are communities that include a continuum of care from independent living, assisted living, residential care and skilled nursing services all on one campus, allowing individuals to live within the same community as their needs progress through the spectrum of care. They typically offer the full selection of amenities associated with retirement living. An endowment fee in addition to a monthly maintenance fee can be expected.
Independent Living Communities provide residents an independent living setting without the burden of home ownership. Typically, residency is established on a monthly rental basis. Residents live in fully equipped private apartments or cottages from studios to large two-bedroom units that may be rental-assisted or market-rate depending on the community. Amenities and hospitality services such as housekeeping, linen service, transportation and social and recreational activities may be included, provided for an additional charge, or may not be available at all. These communities do not provide assistance with activities of daily living or personal care.

Intermediate Care Facilities (ICF) are nursing facilities that are most appropriate for people who need 24 hour medical oversight in a structured setting. Most residents must share their room, but residents are allowed to bring personal items to encourage a more home-like atmosphere. Additionally, nursing facilities offer residents planned social, recreational and spiritual activities.
Memory Care or Dementia Communities offer or provide care for residentswithAlzheimer’sdiseaseorotherdementiainahomelikeenvironment.Thesecommunities, must receive an endorsement and is governed by additional regulations that are specifically intended to support individuals with dementia, including: a secure building that alerts staff if a resident has exited, a secure outdoor area that provides outdoor freedom safely, interior finishes that are non-glare and well lit, and visual contrastsbetweenfloors,wallsanddoorways.Alzheimer’sunitsmustalsohaveprograms,which include: gross motor, self care, social, craft, sensory enhancement and outdoor activities.
Rehabilitation & Therapy is treatment for an injury, illness, or pain with the goal of restoring function, including nursing and therapy services. Rehab is ordered by a physician and services are provided by nurses and physical, occupational, and speech therapists. Examples include working with a physical therapist to help you walk and with an occupational therapist to help you get dressed.
Residential Care Facilities provide housing and supportive services for six or more people who do not require 24 hour nursing care. Pricing can vary greatly dependingonthelevelofcareprovidedandthesizeandamenitiesofeachapartment.Accommodations are typically in a home like setting and may range from a shared bedroom with common bathroom, to private apartments with kitchenette and private bathroom. Monthly fees vary greatly based on unit amenities and care services. Many residentialcarefacilitiesspecializeincaringforindividualswithAlzheimer’sordementia.
Respite Care is short term relief for family caregivers to take a break from the caregiving responsibilities and allows for a healthier and better quality of life for both the caregiver and care receiver.. Respite services include: companionship, personal care, recreational activities and security. Respite care can be provided as short stay or for a longer period that allows the family caregiver to go on vacation or a business trip. Longer duration respite care is generally found at select senior housing communities although someCompanionservicesmayprovidethisserviceinthehome.
Skilled Nursing Facility (SNF) is a State-licensed facility that provides a safe, therapeutic environment for individuals who require rehabilitative care. Skilled Nursing Facilities offer 24 hour skilled nursing care and medical services by licensed nurses and support professionals. This is the highest level of care that can be provided that is not a hospitalization.Additionally,nursingfacilitiesofferresidentsplannedsocial,recreationaland spiritual activities. A good match between a community anda resident’s needs depends as much on the philosophy and services as it does on the quality of care.
The following suggestions can help you get started in your search for a safe, comfortable and appropriate assisted living community:
Whatareyourfutureneedsandhow will the community meet those needs?
Is the community close to family and friends? Are there any shopping centers or other businesses nearby (within walking distance)?
Visiteachcommunitymorethanonce,sometimes unannounced.
Visitatmealtimes,samplethefood, and observe the quality.
Observeinteractionsamongresidentsand staff and ask questions.
Ask about social, recreational, and spiritual activities.
Ask about the type and frequency of staff trainings.
Review state licensing reports.
These steps should also be considered:
Contactyourstate’slong-termcareombudsman to see if any complaints have recently been filed against the assisted living community you are interested in. In many states, the ombudsman checks on conditions at assisted living units and nursing homes.
If the assisted living community is connected to a nursing home, ask for information about it, too. (Information on nursing homes can be found on the Medicare website at http://www.medicare.gov/nhcompare/home.asp).

Influenza-alsoknowas“theflu”isacontagious respiratory illness caused by theinfluenzavirus.Itcancausemildtosevere illness, and can sometimes lead to death. Transmission is person to person bydropletsfromcoughing,orsneezing.Symptoms include fever, chills, cough, sore throat, headache, and runny or stuffy nose. Some people, such as older people, young children, and people with compromised immune systems, are at highriskforseriousflucomplications.Complicationsincludedehydration,electrolyte imbalances and pneumonia. Pneumoniaisresponsibleforover1million admissions to hospitals annually.
Prevention Strategies 1.Getimmunized!Fluvaccine-Seeyour
doctortoseeifyoucantakethefluvaccine.Vaccinesarepreparedeveryyeartoprovidethe maximum protection for that year. Usually lasts 6 months to a year.
Pneumovax- Individuals over age 65shouldalsobeimmunizedwithpneumovax. It consists of a mixture that protects against the 23 most prevalent or invasive pneumococcal types of Streptococcus pneumonias, including the six serotypes that most frequently cause invasive drug-resistant pneumococcal infections streptococcus pneumonia.
2.Antivirals-Ifyougettheflu,talkto your doctor about new anti-viral medicationssuchasTamifluwhichcanshorten the length of the illness, and lessen your symptoms.
3.HandHygiene-washyourhandsfrequently. Assume that viruses and bacteria are on every door handle, counter top, and hand rail in public places…becausetheyare!Washhandswith soap and water for 20 seconds, orusealcoholbasedhandsanitizersfrequently.Carryonewithyou.Coughinto sleeve or tissue, and not your hands.
4.Stayaway!Ifyoubecomesick,stayhome to protect others. If you know someone is sick, stay away. You can visit later when they are healthy.

Bothwillsandtrustsareeffectivetoolstomanage an estate. They perform almost the same function: specifying who gets each of yourassetswhenyoudie.However,therearesome essential differences.
A Will: A written document—signed andwitnessedinaccordancewithOregonlaw, that indicates how to distribute your property upon your death.
Whenitcomestocosts,willsareusuallycheaper to create, but more expensive down the road when beneficiaries need to probate the estate. Probate is a court supervised procedure for transferring the legal title of your assets after death to your beneficiaries and is an essential difference between a will and a trust. Property bequeathed through a will must go through probate, while trust property is not probated.
Probate proceedings are a matter of public record and the process involves: proving to the court that your will is authentic and valid; appointing a personal representative with authority to act on behalf of the estate; identifying and appraising the property of the estate; paying debts, taxes, and legal fees; and distributing the remaining property to the beneficiaries in accordance with the will. This process can be difficult and is time consuming, often taking a year or more.
A will is only effective upon death, and, therefore, does not provide assistance for asset management if you become incapacitated.
A Revocable Living Trust: A written agreement between the creator of the trust, called the settlor, and a trustee who agrees to hold assets for the trust’s beneficiary(s). Typically, one person is all three: settlor, trustee and beneficiary. The RLT addresses three distinct phases of a settlor’s life: First, the trust makes clear that while the settlor is alive and well, he or she will serve as trustee and manage trust assets for his or her own benefit. Second, if the settlor becomes incapacitated to the extent that he cannot manage his finances, the trust identifies a successor trustee, and directs the successor trustee how to manage trust assets. Third, the trust directs the successor trustee how to distribute remaining trust assets after the settlor dies. The settlor retains all rights to manage and amend or revoke the trust while he or she is alive and legally competent.
WhileaRLTismoreexpensivetocreatethan a will, it leaves fewer burdens on a spouse, children or other heirs later. A RLT is not only private, it allows for quicker distribution to the intended beneficiaries, with far fewer administrative costs than the more expensive, public, and time consuming probate process.
