    ServiceSource/CentralFairfax(SSCF)hasbeenservingNorthernVirginiaTrainingCenter(NVTC)

    residentssince1974andhasbeenananchorforcountlesscurrentandformerresidentsandtheir

    familiesastheyhavemadetransitionsandfacedchallengesthroughtheirlives.ServiceSource/CFhas

    alwayssupportedtheprovisionofservicesincommunitybasedsettingswhenpossible,andSSCFhas

    beentheprimaryandlargestproviderofcommunitybaseddayservicesinNorthernVirginiasinceits

    inception.ServiceSource/CFcurrentlyserves48ofthe56NVTCresidentswholeavethatresidential

    settingeachdaytoparticipateinacommunitydayprogram.

    ServiceSource/CFprovidesdayhabilitationservicesinanumberofdecentralizedcommunitycenters.

    WhenplansforNVTCclosurebecameknown,weexpandedourservicesbyopeninganewmodelday

    habilitationcenterinChantillytobuildadditionalcommunitycapacity.Wealsobeganplanninga

    DisabilityResourceCentercapitalcampaignwhichwouldincludeexpandedanddecentralizedservice

    options.Ourexpectationwasthatwewouldlikelycontinuetoservethe48peopleleavingNVTCfora

    communityresidentialprovider,aswellasbeingachoicefordayservicesbymanyoftheother

    approximately90peopleleavingNVTCbeforethescheduledclosurebyJuly,2015.

    TheDayHabilitationServicesprovidedbySSCFaredesignedtoprovidenecessaryandbestpracticeday

    servicesforagroupofpeoplewithsignificantmedical,physical,personalcareand/orbehavioral

    supportneeds.Toaccomplishthis,SSCFreliesonbothpublicandcharitablesupporttoprovidea

    rangeofqualityservices,nursingandtherapies.SSCFhasapublicdayhabilitationratethathasbeen

    negotiatedandestablishedthrougharigorous,costbasedprocurementprocessledbyFairfaxCounty

    andacceptedbythefiveregionalCommunityServiceBoardsandIntermediateCareFacilities,including

    NVTC.Throughthisregionalratesettingsystem,NVTChaspurchaseddayservicesdirectlyfrom

    SSCF.LocalCountiesalsoprovideProgramEnhancementfundingtosupplementotherpublicfunds.In

    additiontothispublicsupport,ServiceSourcescharitablefoundationandoperatingbudgetprovide

    additionalresourcestoSSCFprograms.

    Ithasbecomeclearthattherehasnotbeenasufficientfinancialallocationtothedevelopmentand

    sustenanceofcommunityresidentialordayservicesforindividualswhoarebeingaskedtoleaveNVTC

    andselectcommunityserviceproviders.IndividualsleavingNVTCareexpectedtoprimarilymove

    intoMedicaidWaiverresidentialanddayservicesfunding--whichdonotexistbecauseofan

    inadequateWaiverproviderratesystemforindividualswithspecializedandintensivesupportneeds.

    MedicaidWaiverservicesandrates,unliketheregionalratesettingprocess,arecappedatarbitrary

    levelsandwidelyacknowledgedtobeinadequate,especiallyinNorthernVirginia.Thiscreatesan

    immensebarriertocommunityproviderstomaintainordevelopneededcommunitycapacity.

    Forexample,theimpactonSSCFwouldbea40%ratecutforthose48individualsfromNVTCasthey

    moveintocommunityresidentialsettings.Thatamountstonearly$650Kperyearan

    unmanageablereductioninfundingjustforthosepeoplewhoarecurrentlyserved.Italsocreatesa

    prohibitivelyexpensivebarriertotheexpansionofservicesfortheapproximately90additionalpeople

    whoneedtofindcommunitydayservices.

    Forthisreason,SSCFhasputparticipationinNVTCdischargemeetingsonholduntilthisissuecanbe

    addressedbyStateofficials.WeareactivelyworkingwithlocalCounties,otherprovidersandfamilies

    toelevatethisissueandseeksomeimmediateresolutionsothatdischargeplanningforNVTCresidents

    canmoveforwardwithadequatecommunitybasedservicesinplace.

    AlthoughcommunitycaremayultimatelycostlessthanthetotalNVTCStateallocations,boththe

    capacitybuildingandtransitionperiodrequireadequateresourcesinordertomovepeoplesafelyandtimelytothescheduledNVTCclosuredate.Westronglyadvocateforamoneyfollowstheperson

    approachsothatindividualsleavingNVTCcontinuetoreceivethesameleveloffundingandtypesof

    servicetheycurrentlyreceiveatNVTC(ICFrateswithsupplementalstatesupport).Thiscouldbe

    accomplishedbysomecombinationofthefollowingoptions:

    -developmentofsmallICFresidencesinthecommunity,

    -reformingtheWaiverratestorecognizethespecializedserviceneedsofpeoplewithsignificant

    medical,physical,personalcareand/orbehavioralsupportneeds,

    -transferofgeneralfundsupporttodevelopcommunityservices.
