Welcome to Issue 9

We would like to wish all nurses for “International Nurses Day“ on 12 May last week. We think nurses do a great job everyday and should have a day to recognise the hard work done to provide for our loved ones during hardships. This years theme is ‘Closing the Gap: Increasing Access and Equity’.

Congratulations goes to Amanda Harwood, our winner for the Front Cover competition this issue. Amanda has taken out a Student of the Year award at Challenger Institute’s Student Awards Ceremony in Fremantle last April. Amanda completed a Challenger Diploma of Nursing last year and has since commenced her enrolled nursing graduate placement at Hollywood Private Hospital in Nedlands.

Principal ObjectiveThe Nurse Unit Manager provides direction for the delivery of a high standard of patient care in order to achieve the desired patient outcomes. The NUM is both directly and indirectly responsible for the provision of focused, holistic nursing care. An opportunity has arisen for a Clinical Nurse Specialist – Neonatal Infant Feeding, who will be working within the Neonatal Service at Christchurch Women’s Hospital.

The CNS will be an acknowledged nursing leader for neonatal infant feeding. The CNS will work within a multi-disciplinary team as a role model and resource person for mothers, family/whanua and staff ensuring consistent and appropriate advice about infant feeding. This is achieved through providing quality programmes of education, training and advice.

The successful applicant will be able to demonstrate recent clinical experience with preterm and sick newborn infants and clinical expertise in infant feeding. In addition the applicant should hold an internationally recognised Lactation Consultant qualifi cation and demonstrated teaching, mentoring and assessment skills.

How do I get there?For mental health nursing, there is currently no direct entry to study mental health nursing; therefore it can only be completed as a post graduate qualification from previously studying nursing. You can also apply for graduate transition programs provided by some metropolitan and regional hospitals. These programs are usually partnered with select universities for accreditation of postgraduate subjects. They offer graduates the opportunity to complete educational preparation and receive clinical placements.

“In my aged care work a number of the residents have dementia and sometimes their frustration results in aggressive behaviour,” he said. “But, with the skills I learnt from the Marian Centre I have the confidence to assess

those situations and take action before there is an incident.”

Dante said he had not previously considered working in mental health but was now looking forward to his clinical placement at Graylands:

“I am grateful UWA offered the Marian course because it had has opened another door for me.”

The program takes an applied approach to teaching, with the focus on students to think and feel. Practical exercises are utilised for this to occur.

The program does not repeat what students are taught at the universities, but rather a complimentary value addition. Students are exposed to the different streams available and the many exciting opportunities in mental health.

The program has the support from the Commissioner of Mental Health, who identified programs like these as a positive move in delivering care in the mental health sector. A spike in Mental Health cases following recent natural disasters

According to the Australian Medical Association of Queensland (AMAQ), almost 60 per cent of doctors have reported that there has been a severe and sudden increase in mental health cases after the devastating summer of natural disasters in Queensland. The survey also showed that approximately half of the doctors that were surveyed believed the increase in cases reported will continue for at least the next twelve months.

The survey also revealed a quarter of people who know that they do have a mental illness refuse to receive treatment or they do not know how to seek help.

The president of AMAQ, Dr Gino Pecoraro has statedthatastate-widecampaignwillbelaunchedwith the hopes of encouraging and supporting friends and family to look after and take care of each other.

Mr Pecoraro also said that every community should keep an eye out for signs, both for themselves and for their loved ones.

“We know that about aw quarter of patients are saying they are aware that they’ve noticed feelings of anxiety or they’re not managing. We also know that a quarter of the people we’ve surveyed said that they wouldn’t feel comfortable coming out and asking for help, so what we’re trying to do is enlist the help of the community to look after each other.”

“They won’t go to social engagements whereas once upon a time they did and little things will set them off. If a small setback occurred at work, once uponatimetheycoulddealwithit-nowitblowsupinto a major thing. They’re simple things that people can see every day. If they see it in someone they love, we want them to engage with that person and encourage them to come and see somebody who can help them.”

Nurses constantly have to adapt to challenges in the workplace while also keeping up with their primary role: care of the patient. New graduates require support to bridge the theory-practice gap as they move over into the clinical setting. Experienced nurses require extra professional development if they change practice areas, or need support in the rapidly changing health care environment. These challenges however, provide a chance for nurses to mentor and support one another, assisting knowledge transfer. All of this contributes to a strong learning environment that is beneficial to everyone.

VALUEOften seen as a tool for students and new graduates, mentorship is important for all nurses. It offers practical guidance and a venue for experienced nurses to pass on their knowledge before retiring or moving on. Unlike the teacher-student relationship, which usually involves some form of evaluation, mentorship is a long-term relationship that cultivates active learning. A positive mentorship is based on guidance, partnership, consistency and mutual respect. Some qualities of a good mentor include: > Being a good role model for best practice > Feeling genuine concern for mentee > Aims to inspire mentee > Is keen and enthusiastic about job > Has a positive attitude > Is motivated to teach and support > Has realistic expectations

MEANINGFUL EXCHANGEEffective learning occurs when the mentor and mentee have a good relationship. In the same way, low morale in the mentee can occur if there is a mentor/mentee mismatch, or because of poor mentoring traits. Some of these attributes can include: > Lack of knowledge and expertise > Breaking promises > Delegating unwanted jobs to mentee > Overprotects, or “throws in the deep end” > Often dislikes job and may be disliked by other staff > Poor teaching skills with little structure It’s important to remember that the mentee is

MENTORSHIP:

not a passive entity in the mentoring relationship. To get the most out of the connection, the mentee should drive the relationship, taking on the following roles: > Identify specific skills, knowledge and /or goals to

achieve > Develop a plan, in conjunction with your mentor to

achieve these goals within a specified time frame > Take advantage of any opportunities for learning > Identify resources to aid you in your learning and

to achieve your goals. Resources may include other people, books and websites

> Provide feedback on the relationship and learning experience

MUTUAL BENEFITSOne of the biggest benefits of a mentoring relationship is that mutual learning can occur. Mentoring nurses may find themselves learning new theoretical knowledge, or simply benefiting from the enthusiasm of the mentees. In addition the relationship creates shared value, with older nurses often feeling valued as a result of passing on their knowledge. Nevertheless, just as the mentee has a role in the relationship, so too does the mentor. The following are some of the responsibilities of a mentor: > To provide guidance and support to the mentee

based on their developmental needs – without “spoon feeding”

> Provide feedback on mentee’s development and learning: constructive criticism is very important

> Act as a source of encouragement and support > Play devil’s advocate: the mentor should aid the

mentee’s learning through stimulating thought processes

POTENTIAL PROBLEMSThe nursing environment also plays a key role in its success or failure. As practicing nurses, both mentors and mentees will have competing demands on their time. There will be a conflict between providing patient care and fulfilling the mentor/mentee role. This may be particularly true for the mentor who could have additional responsibilities as a senior staff member. Both need to feel supported in their roles by direct and upper management, as well as by colleagues/team members.

For effective learning to occur as part of the mentoring relationship, both the mentor and mentee need to take time out of the clinical environment to meet, discuss learning and provide feedback. This requires both understanding and cooperation on the part of team members, as a workplace which is not supportive of mentorship will not bring about healthy mentoring relationships, and may instead cause feelings of bitterness and jealousy in other staff, at what may be perceived as “slacking off”. Running an education session on the roles and benefits of mentoring may be of help in this situation.

Working in a constantly changing environment influenced by new developments in health care, the nurse may sometimes feel overwhelmed and under supported. Entering into a mentoring relationship may provide a means of obtaining guidance from a

more experienced practitioner, allowing the nurse to grow and develop with encouragement. As one nurse mentor succinctly stated, “knowledge not shared is wasted” and that is what mentorship is all about.
