Reprints of this Notice are available from the Editor. Manuscripts: Three typewritten copies should be submitted-one original and two dupli- cates-with all material (text, quotations, tables, legends, and references) double-spaced, on one side of the sheet, 8V2 • 11 inches, with ample margins (at least one inch) on all four sides. Original articles are considered and accepted for publication on the condition that they have not been published, or submitted for publication, elsewhere. The Editor reserves the right to edit manuscripts, to fit articles within space available, and to ensure concise- ness, clarity, and stylistic consistency. A synop- sis or abstract of not more than 100 words summarizing the material should accompany the article. The senior author will receive the edited manuscript for checking before publi- cation. Article titles: Titles should be descriptive but as concise as possible. (Long titles tend to discourage reading, present typographical and layout problems, and create difficulties in librarT indexing.) Author Degrees: Affiliations and degrees must be provided. References: Double spaced on a separate sheet, they should follow the Journal's style for literature references, keying the references to the text numbered consecutively in the order of their appearance. For journal references, give the authors' name, article title, journal name, volume number, issue number, inclu- sive pages of article, and year of publication. 1. Gaffney. Illustrations: Line and halftone illustrations should be of high quality for satisfactory reproduction and three copies should be sub- mitted. Illustrations must be numbered and cited in the text. X-ray films are not accept- able; radiographs should be submitted as glossy prints with bone areas in white. Photographs: Three sets should be submitted. Photographs should be unmounted black- and-white glossy prints of high quality. On the back of each photograph, very lightly and in pencil, the figure number and the top edge should be indicated. Ira figure contains two or more photographs, the parts should not be mounted; on the back of each photograph, written lightly in pencil, the figure number and part letter (Fig 1, A), and the top edge should be indicated. Drawings: Figures, charts, and graphs should be neatly and carefully drawn and lettered in India ink by a trained illustrator, l,ettering must be large enough to be read after the drawings are reduced to column width (small, simple drawings) or page widths (large, more complex drawings). Glossy black-and-white prints of drawings (three copies), rather than original art work, should be submitted. Photo- graphs, drawings, illustrations, and artwork will be kept in the Editorial Department for a year after the date of publication. Authors may request return of the material only within that time. Legends: Figure legends should be typed as a group on a separate sheet, not on the illustra- tions or on individual sheets attached to the illustrations. For photomicrographs, legends should specify magnification and stain. Tables: Each table should be logically orga- nized and should supplement the article. Each table should be typed on a separate sheet. Tables should be numbered consecutively and tailored to fit witbin column width or page width. Table title and table footnotes should be on tbe same page with the table. Do not show vertical rules in tables. Permissions, waivers, informed consents: These should accompany the manuscript when it is submitted for publication. Permis- sion ofautbor and publisher must be obtained for the direct use of material (text, photos, and drawings) under copyright that is not that of the author, (Up to a hundred words of prose material usually can be quoted without per- mission, provided the material quoted is not the essence of the complete work.) Waivers must also be obtained for the publication of photographs showing persons, unless faces are masked to prevent identification. Waiver forms are available from the Editor. Manu- scripts that report or describe the results of experimental investigations on human sub- jects must contain the following statement: "The informed consent of all human subjects who participated in the experimental investi- gation(s) reported or described in this manu- script were obtained after the nature of the procedures and possible discomforts and risks had been fully explained." Costs: Most articles are published at no cost to the author, but special arrangements with the Editor must be made for publishing articles containing extensive illustrative or tabular material, formulas, or four-color illustrations. A charge of $285 will be made to the authors for each Journal page or part of a page contain- ing color illustrations. On submission of the manuscript, the author must include a state- ment to agree to pay $285 for each page using color illustrations should the manuscript be accepted for publication. Reprints: The corresponding author is given 200 free reprints of the article. For additional reprints, arrangements must be made with a duplicating service (not with the ADA). Per- mission to reprint forms are sent to the author with the edited manuscript. 288
