Instructions:

1. Complete the cover sheet.

2. Complete the narrative form and indicate the category in which you are making a

nomination.

3. Each nomination must have a nomination form and narrative (see attached for questions

to be included in the narrative).

4. You may submit nominations in multiple categories. You may only submit one nomination

per category.

5. Responses to questions from the narrative form are limited to one 8 1/2 x 11 page document.

Please use a font of at least 11pt and margins of no less than .75. Indicate the nomination

category at the top of the narrative page. Please DO NOT refer the reader to attachments, as

that input will not be considered.

6. Letters of recommendation are required. However, they will only be accepted with the

original nomination package, not after the fact.

7. Please Note: due to a change in the selection process, you no longer must seek

permission first before nominating someone.

8. Do not enclose the instructions, list of previous award recipients, or the list of questions for

each category when you compile your package.

9. The deadline is Friday, August 31, 2018 (postmarked by that date). Sorry, no exceptions.

Selection: The panel of judges is picked from among community leaders who have agreed to be

part of the process. They will judge the nominees and select the winners. If, in the panel’s

judgment, no nominees meet the selection criteria, an award may not be given in a particular

category.

We are striving to recognize donors at all levels.

Nominees from San Antonio and surrounding communities are eligible to receive awards.

Since there are many in our community worthy of recognition, previous honorees are

not eligible (see attached list).

When considering nominees that have served your cause, please describe their

philanthropic involvement with other organizations as well.

Emphasize how the nominee meets the selection criteria, demonstrates consistent

commitment over time, and cite specific examples of how the nominee’s philanthropy

has had an impact on the community.

All nominations materials submitted shall remain the property of the AFP San Antonio Chapter.

2. Describe the results of the project. How did the nominee help change lives? (What impact did the nominee have on the community, including how many people were helped, the financial impact, what organizations were helped, etc.?)

G. Outstanding Volunteer Fundraiser – an individual who has demonstrated leadership in fundraising campaigns.

1. Describe the nominee’s quality of leadership and organizational ability in fundraising campaigns.

2. Describe the time, effort, and personal financial commitment made by the nominee.

3. Describe the nominee’s success in recruiting and motivating others.

4. Describe the fundraising goals achieved by the nominee’s efforts.

5. Describe the nominee’s participation in other local, national or international nonprofit organizations.

H. Outstanding Fundraising Professional – an individual who is committed to the field of fundraising and has had remarkable achievements. Must have at least 5 years of fundraising experience and be an AFP member.

1. Describe the nominee’s career achievements, length and tenure of fundraising background, experience and results. The recipient must have at least 5 years of fundraising experience.

2. Give evidence (examples) of the nominee’s quality of leadership. Describe how the nominee’s leadership is effective, creative and stimulating, and how the nominee acts as a team player.

3. Is the nominee an AFP member (required)? Give evidence (examples) of how the nominee practices and promotes the AFP Code of Ethical Principles and Standards of Professional Practice for fundraising professionals.

4. Describe the nominee’s commitment to fundraising and philanthropy through voluntary services and financial support of nonprofit organizations.

5. Describe the nominee’s commitment to continuing professional development, including certification (CFRE/ACFRE), conference participation, publication and professional education.
