Narcotic Medication Records Change in EPIC:

Situation

Narcotic medication records in EPIC will have the default dispense amounts removed

beginning September 6, 2017.

Background: Multi-modal analgesia initiatives have been implemented for post-op patients recently at

Mercy Medical Center. One of the goals of this initiative is to utilize less narcotics. While educating surgeons on this initiative, several surgeons have expressed concern regarding the defaulted dispense amount of the narcotic medication records in EPIC being too high.

The 2016 CDC recommends the following for acute pain “Long-term opioid use often begins with treatment of acute pain. When opioids are used for acute pain, clinicians

should prescribe the lowest effective dose of immediate-release opioids and should prescribe no greater than needed for the expected duration of pain severe enough to require opioids. Three days or less will often be sufficient; more than seven days will

rarely be needed” Legislation has been introduced that would limit the initial supply of opioids for acute pain

to 7 days.

Assessment: There are 1,679 EPIC narcotic medication records which are marked as controlled

substances, there are 154 of these which have defaulted dispense quantities. Common narcotics prescribed post op are Roxicodone, Lortab and Percocet to name a few.

Most of the medication records are defaulted for 10 days with a dispense amount of 30 tablets which many of the surgeons are changing to a smaller quantity.

Recommendation:

The same narcotic medication records are utilized for acute pain, chronic pain, cancer pain, end of life pain, etc. There is no one perfect number for the dispense amount due to

the various scenarios for prescribing them. It was approved to remove the default dispense amounts on the narcotic records

(providers can save their own default dispense amounts on their preference

lists). Approved by PLC Quality Committee, Medication Management Committee and
