You will receive an email with an access link, typically in your frst week. You have 45 days to take your Health Risk Assessment to enroll in Get Healthy Now to receive your $40 monthly premium incentive.

Q& A How do I enroll in Get Healthy Now (GHN)? Enroll by completing the online Health Assessment within 45 days of your hire date and annually during Open Enrollment. • Go to: uoflgethealthynow.cafewell.com. • Follow instructions to set up your new user account (employee ID# will be needed) or login with your

username and password if you already have an account.

Having trouble accessing the site? • Call 1-888-886-4684. • Please note, the Health Assessment is activated after your benefits elections are processed by the Human

Resources — Benefits Department (with exception of the Open Enrollment event).

When do I start receiving the $40 monthly premium incentive for participation in GHN? It will take 30-60 days after you complete the Health Assessment for the first $40 premium incentive to be applied to your paycheck. To confirm you are receiving the premium incentive every month, login to Ulink and review your paycheck. Look for “GHN Inctv” located in the “Before Tax Deduction” panel. Don’t see your reward? Contact us at [email protected]. Please note: some employees may need to take additional action steps throughout the year to receive the monthly premium incentive. See next question for more information regarding GHN participation guidelines to earn the premium incentive.

How do I earn the $40 monthly premium incentive? After you complete your Health Assessment, a Health Advisor will call you to review your results and determine if additional action steps are needed to maintain the $40 monthly premium incentive. Additional action steps are required for those with an Overall Wellness Score (OWS) of 66 or below. Participants can choose from either of the following options: • Wellness Coaching: Telephonic or Online 1-888-886-4684 (option 2). • Good Health is in the Cards Program: refer to Get Healthy Now website to learn more about this enhanced

option to earn rewards. • ULP Health Management Services — information and appointments 502-588-0770.

When can I expect my Health Advisory Call? Upon completing the Health Assessment, you will receive a confidential Health Advisory Call within 2-3 weeks. This 10-15 minute call gives you the chance to review your results and determine if additional action steps are needed to maintain the $40 monthly premium incentive.

How is my information protected? GHN is considered a group health plan and is in compliance with HIPAA and all other state and federal regulatory agencies. UofL partners with HealthFitness to ensure confidentiality and compliance regarding protection of your personal health information. All information you provide is confidential. Only you and your Health Advisor/Coach have access to your individual level personal health information.

Can I still participate in Get Healthy Now if I waive UofL’s health coverage? Yes. If you waive UofL‘s health coverage, you can still participate in GHN wellness initiatives. However, the $40 premium incentive is integrated into the health plan so if you waive health coverage you are not eligible to receive it.

UL2FCTR Enrollment Guide Our self-enrollment process makes it easy to register and install the Duo Mobile application on your smartphone or

tablet. Duo prompts you to enroll when you log into a server, or web application.

Supported Browsers: Chrome, Firefox, Safari, Internet Explorer 8 or later, and Opera.

1. Welcome Screen: Click Start setup to

begin enrolling your device.

2. Choose Your Authentication Device

Type: Select the type of device you’d like to

enroll and click Continue. We recommend

using a smartphone for the best experience,

but you can also enroll a landline telephone,

a U2F token, or iOS/Android tablets. You can

enter an extension if you chose “Landline”.

3. Type Your Phone Number: Select your

country from the drop-down list and type

your phone number. Use the number of your

smartphone, landline, or cell phone that

you’ll have with you when you’re logging in

to a Duo-protected service.

Then double-check that you entered it

correctly, check the box, and click Continue.

PayCard Frequently Asked Questions

Q. What is a PayCard? A. It’s a prepaid, reloadable VISA® Card issued in your name that looks and works just like a debit card or as a check if you choose. On payment date, your funds will be immediately available on your PayCard. You can then use your PayCard to deposit funds into your own bank account. Or you use it to make purchases or withdraw cash anywhere VISA debit cards are accepted – over 6 million locations in the United States and over 24 million worldwide!

Q. Can I still deposit funds into my bank account? A. Yes. Rather than waiting for a check to arrive in the mail, you can go to your bank on the day of funding and deposit funds from your PayCard into your own bank account. You can either use a bank teller or an ATM machine to withdraw cash from your PayCard and then deposit that cash into your bank account. You will have one free bank or ATM transaction per payment cycle.

Q. Is my PayCard actually a credit card? A. No. It’s a prepaid VISA card that is loaded with your funds on the morning of every payment date (every other Friday). Each time you use the PayCard, the transaction is deducted from the amount of money available on the PayCard.

Q. How do I know how much money I have left on my PayCard? A. It’s always a good idea to keep track of your money as it is loaded on the PayCard every payment date and as you spend it. If you are not sure of your balance, check it at http://www.pncpaycard.com or by calling 1-866-453-5071. These services are available 24x7, 365 days a year. You can also get account history online. Remember, merchants can’t tell you how much money is on the PayCard.

Q. How do I use my PayCard? A. You will be able to use your PayCard at any merchant that accepts VISA debit cards. Just give the PayCard to the cashier, waiter or attendant, and sign for your purchase. If you want to get cash back, use your Personal Identification Number (PIN) at stores that offer this service – grocery and convenience stores usually do. Use your PIN at an ATM, and when prompted press “Checking”. You can also use the PayCard to make purchases online, via telephone, and by mail order. Note daily spending limits may apply to your PayCard.

Q. What is a PIN? A. Your PIN (Personal Identification Number) is a secret four-digit code that gives access to your PayCard account. Always keep it separate from your PayCard, and never give it to anyone, especially over the phone.

Q. What if the cashier asks “debit” or “credit”? A. Both responses will access the balance available on your PayCard. If you say “credit”, you will have the convenience of just signing for your purchase; if you say “debit” you must enter your PIN. If you want to receive cash back from a merchant that provides this service, you generally will be required to enter a PIN.

Q. Can I use my PayCard to buy something that costs more than the amount of money I have on my PayCard? A. Some merchants will allow you to pay with two forms of payment. By using another form of payment in addition to your PayCard, you can still make purchases that exceed your available balance on your PayCard. You can pay for part of your purchase with your PayCard and pay the other part with a second type of payment.

Q. What happens if I need to return an item that I purchased using my PayCard? A. Every store has its own return policy and will handle the return in the same way as any other VISA transaction. The store may “credit” your PayCard, or provide a cash refund or store credit. A credit to your PayCard may take up to one week to process before it’s available for use.

Q. What should I do if the merchant tells me the PayCard was “declined”? A. Declines generally occur when the purchase amount is more than the available funds on your PayCard. Check your available balance and then you may offer to pay the merchant partly with the funds on your PayCard and partly by another payment method acceptable to the merchant. Note that daily spending limits may also result in a decline.

Q. What happens if my PayCard is lost or stolen? A. You must immediately call 1-866-453-5071 to report a lost or stolen PayCard and order a replacement. As long as you report your PayCard lost or stolen right away, you will not lose any money if someone uses the PayCard without your permission. This is one of the major benefits of having your funds loaded on a PayCard.

Q. Whom do I contact if I have a question regarding my PayCard? A. You can contact PayCard Services at 1-866-453-5071, 24 hours a day 7 days a week.

Q. Will I receive a statement showing my transaction activity? A. Online, printable account statements for the last 12 months are available at www.pncpaycard.com.

Q. Does my PayCard have an expiration date? A. Yes. You will receive a replacement PayCard the month before your PayCard expires.

Q. Do I receive a new PayCard each time a balance is loaded? A. No. You will continue to use the same PayCard until it expires.

Q. How does my PayCard work at gas stations and restaurants? A. When you’re paying at the pump, some stations will check to see if you have enough money to fund a full tank (up to $75) because they don’t know how much you are going to buy. If your PayCard is declined, and you know you still have money available on your PayCard, go inside the station and tell the attendant how much gas you want to buy and sign the purchase receipt for that amount. As for restaurants, some will verify that your PayCard has enough money to cover the cost of the meal and a 15%-20% tip. If it doesn’t, the transaction may be declined. On the receipt, you may write in a tip in any amount, or leave a cash tip. Only the amount you sign for will be deducted from your account.

General Information on Disclosure Requirements

Conflict of Interest A conflict of interest (COI) is any situation that may compromise or appear to compromise a covered individual’s (employee or researcher) professional judgment in carrying out their University of Louisville (U of L) activities because of an external relationship/interest of the employee or a member of their immediate family.

Reporting External Interests & Potential Conflicts External interests and activities must be reported on the Attestation and Disclosure Form (ADF). This includes, off campus, non-University commitments, such as lectures, speaker bureaus, or consulting activities for external entities, as well as, equity holding, board positions, and other activities or ownerships. The questions on the ADF specify what external interests to disclose and other potential COIs that are required to be disclosed.

If you are unsure if you should disclose an activity/interest, it is best to disclose and provide additional information in the open text box(es).

How to Disclose The Attestation and Disclosure Form (ADF) is the means to disclose external interests and activities. The ADF is available at http://iris.louisville.edu. Instructions for completing the form are available http://louisville.edu/conflictofinterest.

Annual Disclosure Requirement Effective October 1, 2017, all covered individuals are required to complete an ADF at least on an annual basis. These individuals include:

• All University of Louisville Employees. • Non-employees who are engaged in research under the auspices of U of L, regardless of compensation.

This includes all individuals with a research appointment.

Managing Possible Conflicts of Interests If you have reported disclosures, the submission will be reviewed to determine if a possible conflict of interest exists. If it is determined that your conflict requires management, an approved management plan will be developed for the COI. You will have to agree and sign off on the management plan and your Appropriate Authority will be copied on the plan. Unique COI situations are brought before the Conflict Review Board for a determination on the management, reduction, or elimination of a conflict of interest.

Overview

The Information Security Office (ISO) serves as the University's resource for guidance on information security compliance and administers the University's Information Security Program. The ISO oversees information security policies and standards; provides compliance oversight, and assessments; coordinates information security efforts, user awareness and incident response. The ISO works in conjunction with IT Enterprise Security, Audit Services, Institutional Compliance and officials in compliance areas such as HIPAA, FERPA, PCI and Export Controls to maintain regulatory compliance and to protect the confidentiality, integrity and availability of all University information assets.

Policy Scope and Applicability

The University’s policies are applicable to all persons while conducting/performing work, teaching, research or study activity or otherwise using university re-

sources. Also includes all facilities, property, data and equipment owned, leased and/or maintained by the

University or affiliates.

Compliance

Failure to comply with these policies and standards

and/or any related information security and/or infor-

mation technology policy, standard or procedure may result in disciplinary action up to and including termina-

tion of employment, services or relationship with the

University and/or action in accordance with local ordi-

nances, state or federal laws.

What is ‘sensitive’ information?

Information of a confidential or proprietary nature and other information that would not be routinely published

for unrestricted public access or where disclosure is

prohibited by laws, regulations, contractual agreements or University policy. This includes (but is not limited to) full name or first initial and last name and employee ID (in combination), identifiable medical or health records,

grades and other enrollment information, credit card,

bank account and other personal financial information, social security numbers, grant reviews, dates of birth

(when combined with name, address and/or phone numbers), user IDs when combined with a password,

etc.

User Responsibilities Never share or post your user password and keep security codes, keys, equipment, etc. secure

Know your data’s sensitivity level and any regulations that apply to it — handle and safeguard accordingly

Immediately notify your supervisor or the Security Office if you suspect or become aware of an incident or information breach

Never share or store ‘sensitive’ data with external parties without appropriate agree-ments and University approval (includes cloud storage and texting)

RETURNING TO CAMPUS: EMPLOYEE SELF-ASSESSMENT REQUIREMENTS AND ATTESTATION

The University is committed to re-opening our campuses in a way that promotes the health and safety of our employees and students. This includes, in part, the enforcement of physical distancing, wearing of masks, access to hand sanitizer and encouraging hand washing. The University cannot be successful, however, without the commitment of its employees to do their part to similarly promote the health and safety of our Cardinal Family. We all have a personal responsibility in fulfilling this commitment.

When returning to work on campus for the first time after May 11, 2020, or as soon as practical thereafter, it is my responsibility to complete this Attestation of Self-Assessment Requirements and submit this confirmation to my immediate supervisor.

I understand and affirm that PRIOR to reporting to work each day, I will conduct a daily health assessment, which consists of asking myself 2 simple questions:

Question 1: Have I had any of the following symptoms since my last day at work? Fever of 100.4 or higher Cough Shortness of Breath Sore Throat Muscle Aches Chills Gastrointestinal symptoms (i.e. nausea, diarrhea, vomiting, etc., unrelated to an underlying medical condition or pregnancy)

Question 2: Have I been in close contact with anyone who has been diagnosed with COVID-19?

If I answer yes to EITHER of these two questions, I will 1) Stay at home 2) Immediately notify my supervisor I will not be reporting to work because I answered yes to questions 1, 2 or

both. (Please do not share specific symptom information with your supervisor) 3) Call Campus Health at 852-6446 and notify them I am a UofL employee who has answered yes to questions 1, 2

or both so I can be scheduled for an appointment. 4) When I am released to return to work, I will provide my supervisor with a return to work notice from my medical

healthcare provider. (This document should not include a diagnosis or confidential health information.) I am not permitted to return to work without an appropriate return to work notice.

Campus Health will provide COVID-19 testing, if deemed necessary by a Campus Health medical professional, at no cost to the employee.

By returning to campus for work, I acknowledge that I understand my requirements to perform a daily self-assessment and that I will, in good faith, complete this health assessment prior to coming to work each day.

The University retains the right to conduct temperature screenings of employees at any time or send home employees who exhibit symptoms affiliated with COVID-19.
