NEVADA STATE BOARD OF PHYSICAL THERAPY EXAMINERS AGENDA FOR BOARD MEETING MEETING DATE: November 20, 2014 MEETING TIME: 9:30am MEETING LOCATION: Office of the Board 7570 Norman Rockwell Lane, #230 Las Vegas, NV 89143 The purpose of this Board is to control and regulate the licensure of all physical therapists and physical therapist’s assistants within the State of Nevada to ensure public protection. Items relating to the function of this Board can be placed on the Agenda by writing this Board. Items on the agenda may be taken out of order. The Board may combine two or more agenda items for consideration. The Board may remove an item from the agenda or delay discussion relating to an item on the agenda at any time. Public Comment Procedure : 1. Public Comment will be taken at the beginning and at the end of each Board meeting; 2. The purpose of this Board is to control and regulate the licensure of all physical therapists and physical

Item 9- Applicant Brian Shive requests to be licensed as a physical therapist. This
