SourceNet, Inc is a community-based 501(c) 3 organization dedicated to advocacy to those vulnerable to barriers for care due to issues related to Chronic Homelessness.

SourceNet, Inc The “Enabling” service approach are culturally competent best practices of

“wrap-around resource coordination.” v  CDHC medical provider identify / refer v  Coordination of support with community providers i.e.

caseworkers and agencies. v  Assist client with building rapport and relationships

with other providers. v  Support patient’s application compliances.

The patient who is linked with programs and services through the assistance of SourceNet, Inc are varied in the type of care that is needed: v  Disability Documentation and Presenting v  Online Resource and Applications Processes v  Housing Advocacy v  Links to Community-based Mental Health services v  Transportation Direct and In-Direct v  Continuity of Medical Care through referral , coordinating

appointments and in attendance with client when appropriate due to patient vulnerability.

v  Assistance with securing legal items i.e. photo ID’s, Social Security cards, Birth Certificates, Medical Records and what

ever else may be required to complete processes for resources. v  Direct Funds when appropriate v  Assisting Client to address all barriers to care and resources.

SourceNet, Inc works with many partners within the Omaha and surrounding communities. Federal, State and County funded programs of training, housing, medical, behavioral health and any other resources to improve the quality of life and health for our patients.

SourceNet, Inc resource coordination and community partnerships enables greater access for the patients in programs such as:

v  Housing, Employment First, Psychiatric Services, Counseling – Individual and Family, Substance Abuse treatments, Medicaid, Medicare, Social Security and SNAPS (supplemental food stamps).

SourceNet, Inc provides continuous tracking and monitoring of patients. Monthly reports are provided to Charles Drew Health Center updating the number of patients that are active and/or resolved. The tracking tool provided by SourceNet, Inc: v  Shows the “link’ between the patient and identified program and/or service v  Shows monitoring and mediating between the chronically homeless patient, SourceNet, Inc, and the identified program and/or service
