That I have applied for NBP advance salary product of NBP, and I declare irrevocable and unconditionally

as under:

That the total monthly amortization payments being made by me inclusive off all consumer loansfrom any bank/DFI do not and shell not exceed 50% of my net disposable income at all material

times are such other limit as may be prescribed by the State Bank of Pakistan. And/or NBP from

time to time.

I and my dependents (as per SBP prudential regulations) have not previously availed clean cashlimit up to Rs. Rs. 500000/= from any bank/DFI apart from NBP.

Any branch of this undertaking shell without any further reference to me amount to a branch ofNBP Advance salary scheme and NBP shell be fully entitled to exercise its rights under the NBP

advance salary scheme. In this regard indemnify and hold NBP harmless against any losses or

liabilities of whatsoever nature that NBP may incur by reasons of sanctioning me the NBP

Advance Salary product in reliance of this undertaking and indemnity.

I declare & solemnly undertake that I will not/I have not applied/use the funds obtained throughthis loan/advance salary facility for the purchase of any initial public offering (IPOs)

I declare and solemnly undertake that I will/have not applied/use the funds obtained throughthis trading or trading of real estate, seta activities and gambling etc.

I declare and solemnly undertake that the loan/advance salary/funds will not be applied forlaundering smuggling opium/alcohol/banned items trade, unlawful, illegal activities.

I declare and solemnly undertake that the loan/advance salary/fund will not be applied for anyterrorist activities.

I solemnly declare that I am not involved in any money laundering terrorist group(s)/faction,banned religious group and/ or any ultra vied acts. I am not involved in any lawful/illegal

activities.

This undertake is effective from the date of sanction. I declare an solemnly undertake that the loan/advance salary/ funds will be utilized fro the

purpose that nave applied for n my application to the bank (NBP)

Whatever has been stated above is true and correct to the best of my knowledge and belief and noting

has been concealed therein.

I am availing the loan (NBP Advance salary) from NBP Civil SecretariatBranch Muzaffarabad AJK.

Therefore I authorize the District Accounts Officer/DDO not to transfer my salary including any

benefits thereof from my personal Account No. 12573-8 at NBP Civil Secretariat Branch

Muzaffarabad AJK without the NOC of the NBP branch.
