SummaCare Individual Solutions 2015 Plan Highlights

Enrollment Timeline • November 15, 2014: This is the fi rst day you can

enroll in your plan of choice for 2015 coverage. • December 15, 2014: This is the last day you can

enroll in your plan of choice for coverage to be effective January 1, 2015.

• January 15, 2015: This is the last day you can enroll in your plan of choice for coverage to be effective February 1, 2015.

• February 15, 2015: This is the last day of Open Enrollment. You must have enrolled in your plan of choice by this date to ensure that you are not penalized for not having insurance. If you enroll by this date, your coverage will be effective March 1, 2015.

• If you experience a life-changing event during the year such as marriage, birth of a child or a change in job status, you may be eligible for a Special Election Period and can enroll in your plan of choice at that time.
