Contractors are not required by law to have an I.D. If your company doesn’t have an MSHA
Contractor I.D. just leave this section blank.���

The training time on your certificate must add up to at least 24 hours. ���
This includes both classroom and “On The Job” training. What is “On The Job Training”?���

may receive this training from a different instructor at a new location. ���
