General Rules and Requlations

Governs all sections of this tariff unless otherwise provided within individual sections. Except as otherwise provided hereinthe rates named in this tariff include one pick-up and loading at point of origin and on delivery and unloading at point ofdestination.

Rule 1 MILEAGE AND TIME CALCULATION FOR INTRASTATE SERVICE

Calculation as otherwise provided herein by:www.mapquest.com provides calculations of mileage and time. Any dispute as to distance and time shall bereferred to the South Carolina Public Service Commission for determination.

Rule 2 EXTRA PICK-UP OR DELIVERY

Subject to Rule 3 portions of a shipment may be picked-up at one or more places en-route between origin anddestination. Charges will be for the total hours of service and transportation (if not a local shipment) or entireshipment for total distance from farthest point origin to ultimate destination. The total charges for picked-up ordelivered portions shall not, in any case exceed the total charges as would apply if computed to each portion asa separate shipment. A charge for each extra pick-up or delivery will apply (see Item 2 for application).

Rule 3 CONSOLIDATED SHIPMENTS

a, Property of two or more families or establishments located at different addresses will not be accepted fortransportation as a single shipment. Such property must be handled from each address as a separate shipmenton a separate Bill of Lading.

b. The name of only one shipper and one consignee shall appear on one Bill of Lading, but the Bill of Lading mayalso specify the name of a party to notify of the arrival of the shipment at destination.

Rule 4 LOADING AND UNLOADING

Except as otherwise provided herein, if shipment is delivered to or picked-up at a warehouse, the rates fortransportation include only the unloading or loading at door, platform, or other point convenient or accessible tothe vehicle.

Rule 5 VALUATION CHARGES

On all shipments moving under the provisions as provided for in this tariff, there are twooptions available to cover loss and/or damages.

OPTION 1: VALUATION CHARGES provided for in this item, WILL APPLY UNLESSSHIPPER EXPRESSLY RELEASES THE SHIPMENT TO A VALUE NOT TO EXCEED

SIXTY (60) CENTS PER POUND PER ARTICLE.

OPTION 2: Full value protection. This option provides coverage based on currentReplacement value at the time of loss or damage, up to the dollar amount of valuationdeclared. The additional cost is based on the actual value declared and the deductible, if

any, declared.

DECLARATIONPrior to the move, the shipper must select one of the options by completing theaddendum made a part of the shipping order and be evidenced by theirsignature and date. Should the shipper refuse to select one of the options, thecarrier will not be required to perform the move.

OPTION 1: Depreciated Value - $0.60 Per Pound No additional charge

OPTION 2: (a) Replacement Value (See NOTE A & B) - No deductible- Additional charge of $11.30 per one thousand ($1000.00) dollars of

declared value.(b) Replacement Value (See NOTE A & B) - $300.00 deductible

-Additional charge of $3.75 per one thousand ($1000.00)dollars of declared value.

Should carrier fail to secure the shipper's declaration completed and signed prior to anymovement, the shipper will be considered to have chosen Option 2 (a) at no charge to theshipper.

NOTE A - Declared value per room must be an amount equal to or exceeding $5,000.00 per room excluding halls,attics, garage, closets and baths. A self-storage unit up to size 10 X 10 shall constitute one room for thepurpose of valuation.

NOTE B - Some items will be deemed out of the control of the carrier per shipper's request or condition ofcraftsmanship of items requested to be moved. When these items arise a general release will be required toperform action releasing XMS from liability. Refer to page 18 for addendum to the uniform household goods billof lading - B, itemizing releases and limitation of liability.

MARKING AND PACKING

Articles of fragile or breakable nature must be properly packed.

Packages containing fragile articles or articles consisting wholly or in part of glass when packed by the shipperor his agent, must be marked by plain and distinct letters, designating the fragile character or contents.

When the articles of furniture, consisting wholly or in part of glass is covered or wrapped by the shipper or hisagents, such articles shall be wrapped in a manner to clearly expose glass surfaces or glass portions, so it canbe identified by carrier.

When articles are improperly packed, crated or boxed and by reason thereof the contents are more susceptibleto damage. Carrier will arrange to have such articles properly packed at an additional charge, or carrier hasright to refuse shipment of article.

COMPLETE ARTICLE (Defined)

Each shipping piece or package and contents thereof shall constitute an article. The total component parts ofany article taken apart or knocked down for handling or loading in vehicle shall constitute an article.

ARTICLES OF HIGH OR EXTRAORDINARY VALUE

Unless otherwise provided, the following property will not be accepted for Shipment:Bank bills, coins or currency, deeds, notes, drafts or valuable papers of any kind, jewelry, postage stamps,stamp collections, revenue stamps, redeemable trading stamps, letters or packets of letters, precious stones, orarticle or peculiarly inherent or extraordinary value; precious metals or articles manufactured there from.Should such articles come into the possession of the carrier with or without his knowledge, responsibility forsafe delivery will not be assumed.

ARTICLES LIABLE TO CAUSE DAMAGE

1) Carrier will not accept for shipment property liable to impregnate or otherwise damage equipment or otherproperty.

2) The carrier will not accept for shipment articles, which cannot be taken from the premises without damageto the article or the premises.

3) No Appliances will be moved with food.

INSPECTION OF ARTICLES

When carrier or his agent believes it is necessary that the contents or packages be inspected; he shall make orcause such inspection to be made or require other sufficient evidence to determine the actual character of theproperty.

HANDLING OF HEAVY ARTICLES

Except as otherwise specifically provided in this tariff, or as amended, the services covered by this tariff do notinclude the handling, loading or unloading of any single article weighing 600 (six hundred) pounds or moreexcept pianos. The extra handling, loading, or unloading in every instance must be provided by the shipper or ifthe carrier has additional personnel and equipment available, such extra services upon such articles must besecurely braced and blocked, and when such bracing or blocking require material not forming part of the regularequipment of the vehicle, or extra labor, such material must be furnished by the shipper. (Subject to Item 3)

STORAGE-IN-TRANSIT

STORAGE-IN-TRANSIT of household goods is the holding of a shipment in the warehouse of carrier or carrier'sagent for storage pending further transportation. This service will be performed only upon request of shipperand subject to the following provisions:

a. Carrier may require payment of accumulated transportation and accessorial charges when transit shipmentis delivered to temporary storage warehouse.

Shipments moving under this rule may be stored in transit only once and for a period not to exceed 180 (onehundred eighty) days from date of delivery to warehouse. When instructions for further transportations are notgiven prior to the expiration of 180 (one hundred eighty) day period, the in-transit character of the shipment willcease and the warehouse shall be considered the final destination of the shipment. When shipment remains instorage after the expiration of the 180 (one hundred eighty) days all accumulated transportation or accessorialcharges must be paid unless previously collected in accordance with paragraph (a).

STORAGE-IN-TRANSIT within 50 (fifty) miles of origin and/or destination - no travel time will be accessed.Transportation charges shall be assessed upon the basis of applicable rate published in tariff from initial point ofpick-up to STORAGE-IN-TRANSIT warehouse, plus applicable rate in tariff from STORAGE-IN-TRANSITwarehouse to point of final destination.

STORAGE-IN-TRANSIT Over 50 (fifty) miles of origin and or destination - no travel time will be accessed for.

1) Carrier will not accept for shipment property liable to impregnate or otherwise damage equipment or otherproperty.

2) The carrier will not accept for shipment articles, which cannot be taken from the premises without damageto the article or the premises.

3) No Appliances will be moved with food.

Rule 10 INSPECTION OF ARTICLES

When carrier or his agent believes it is necessary that the contents or packages be inspected, he shall make or

cause such inspection to be made or require other sufficient evidence to determine the actual character of theproperty.

Rule 11 HANDLING OF HEAVY ARTICLES

Except as otherwise specifically provided in this tariff, or as amended, the services covered by this tariff do notinclude the handling, loading or unloading of any single article weighing 600 (six hundred) pounds or moreexcept pianos. The extra handling, loading, or unloading in every instance must be provided by the shipper or if

the carrier has additional personnel and equipment available, such extra services upon such articles must besecurely braced and blocked, and when such bracing or blocking require material not forming part of the regularequipment of the vehicle, or extra labor, such material must be furnished by the shipper. (Subject to Item 3)

Rule 12 STORAGE-IN-TRANSIT

1. STORAGE-IN-TRANSIT of household goods is the holding of a shipment in the warehouse of earner or carrier'sagent for storage pending further transportation. This service will be performed only upon request of shipperand subject to the following provisions:

a. Carrier may require payment of accumulated transportation and accessorial charges when transit shipmentis delivered to temporary storage warehouse.

Shipments moving under this rule may be stored in transit only once and for a period not to exceed 180 (onehundred eighty) days from date of delivery to warehouse. When instructions for further transportations are notgiven prior to the expiration of 180 (one hundred eighty) day period, the in-transit character of the shipment will

cease and the warehouse shall be considered the final destination of the shipment. When shipment remains in

storage after the expiration of the 180 (one hundred eighty) days all accumulated transportation or accessorialcharges must be paid unless previously collected in accordance with paragraph (a).

STORAGE-IN-TRANSIT within 50 (fifty) miles of origin and/or destination - no travel time will be accessed.Transportation charges shall be assessed upon the basis of applicable rate published in tariff from initial point of

pick-up to STORAGE-IN-TRANSIT warehouse, plus applicable rate in tariff from STORAGE-IN-TRANSITwarehouse to point of final destination,

ATTEMPTED DELIVERY

Compensation to the carriers for attempted delivery to residence from STORAGE-IN-TRANSIT when failure todeliver is not the fault of carrier, will be as follows:

a. Mileage from warehouse to residence: If total mileage is 50 (fifty) miles or less, pick-up or deliverytransportation rates on STORAGE-IN-TRANSIT shipments will apply.

b. If total mileage is greater than 50 (fifty) miles, the transportation rate in the applicable line-haul rate tablewill apply.

General Rules and Regulations

a. Any claim for loss, damage, or overcharge shall be in writing and filed with the carrier within ninety (90)days after delivery. As a condition precedent to any claim adjustment or payment, said claim shall be

accompanied by the original paid Bill if not previously surrendered to the carrier. Carrier may requirecertified or sworn statement of claim.

b. Carrier shall be immediately notified for all claims for concealed and/or external damage and shall be given

reasonable opportunity to inspect alleged external damage or concealed damage in the original package. Itis incumbent upon the carrier or the designated agent of the carrier to inspect any and all reported damage

and to provide for repairs or compensation based in the level of liability selected. If damage is alleged to

have occurred to the shipper's dwelling or surrounding, for example but not limited to, damage to walls,floors, steps, ceilings, rails, doors, driveway, lawn, fence, patio or garage, it is the carrier's liability or that ofhis designated agent to inspect the alleged damage. If such damage is determined to be transit related, it is

the carrier's obligation to repair or restore to original condition or to otherwise make whole by

compensation, unless a release form has been signed by the shipper and or his agent to precede knowingdamage may occur to home and or freight. Transit related damage occurring to a shipper's dwelling or

surrounding area is not covered by the Shipper's Declaration of Value. Any claim for property loss and/ordamage must be reported within 48 hours of delivery/loading.

c. The carrier's liability shall not exceed the cost of repairing or replacing the lost or damaged property with

material of like kind and quality. It shall also not exceed the actual cash value of the property at the time andplace of loss, but in no event to exceed the released value as determined under Rule 5. Replacement or

repairing will be determined by the lesser of the two amounts.d. The carrier shall not be liable for loss or damage occurring after the property has been delivered to or

receipted for by the consignee or shipper or authorized agent of either. When the carrier is directed tounload or to deliver the property (or render any services) at a place at which the consignee or its agent is

not present, the property shall be at risk of the owner after unloading or delivery.e. Where the carrier is directed to load property from (or render any service at) a place or places at which the

consigner or its agent is not present, the property shall be at risk of the owner before loading.

f. The carrier's liability with regards to sets and matched pieces shall be limited to repair or depreciatedreplacement of the lost or damaged piece or pieces only and shall not extend to repair, replacement or

recovering of the entire set, but in no event to exceed the released value as determined under Rule 5.

Rule 16

a. Any carrier or party liable on account of loss or damage to any said property shall have the full benefit of any

insurance that may have been effected upon or account of said property so far as this shall not avoid thepolicies or contracts of insurance; provided that the carrier reimburse the claimant for the premium paid

thereon. Any claim for property loss and/or damage shall be filed in writing within 2 (two) business days afterdelivery.

PAYMENTS

The carrier will not deliver or relinquish possessions of any property transported by it until tariff rates and

charges thereon have been paid in cash, money order, or certified check, except where other satisfactoryarrangements have been made between the carrier and the consignor or consignee.

Nothing herein shall limit the right of the carrier to require, at time of or before shipment, the prepayment in part

or in full or guarantee of the charges.

Subject to the foregoing paragraphs, provision for payment of charges on STORAGE-IN-TRANSIT shipments iscontained in Rule 13.

All Shipments are COD unless otherwise agreed in writing.

Credit Card payments will be assessed a 5% charge for the use of the Credit Card. Shipper must be in good

standing to use a credit card for final payment.

HOURLY RATES

Charges based on time shall be computed by multiplying the hourly rate by the time involved. Fractions of an

hour will be disposed of as follows: Where the time involved is less than 15 (fifteen) minutes the charge shall be

for one quarter of an hour. When in excess of 15 (fifteen) minutes but not more than 30 (thirty) minutes chargefor one-half hour. When in excess of 30 (thirty) minutes but not more than 45 (forty five) minutes charge for

three-quarters of an hour. When in excess of 45 (forty-five) minutes charge for one hour. There will be a 3(three) hour minimum charge for hourly services, and 1 (one) hour travel time = to hourly rate

Rule 17 MINIMUM CHARGE FOR INTRASTATE MOVES

Except as may be otherwise specifically provided for in this tariff or as amended, a shipment labor time is lessthan 5 hours shall be accepted only at a labor time of 5 hours and at the applicable rate shown for 5 hours.

SERVICING SPECIAL ARTICLES

The transportation rates in this tariff do not include servicing or de-servicing articles or appliances such asrefrigerators, deep freezers, cabinets, front loading washing machines, plasma television sets, air conditioners,pool tables, grandfather clocks, hot tubs and the like, which if not properly serviced, may be damaged in, orincident to, transit; nor is liability assumed for any such damage unless said articles or appliances are servicedand de-serviced as provided below.

a, If carrier does not possess the qualified personnel to properly service and de-service articles orappliances, carrier will upon request of shipper, owner, or consignee and as agent for them, engage thirdpersons to perform the servicing and de-servicing. When third persons are engaged by the carrier toperform any service, the carrier will not assume responsibility for their activities or conduct, amount oftheir charges, nor the quality or quantity of service furnished.

b. All charges of the third persons must be paid by the shipper and are in addition to all other charges in thistariff. Such charges will be advanced by the carrier and billed as an advanced charge.

IMPRACTICAL PICK-UP OR DELIVERY AND AUXILIARY SERVICES

It is the responsibility of the shipper to make shipment accessible to carrier or accept delivery from carrier at apoint at which the road haul vehicle may be safely operated.

When it is physically impossible for carrier to perform pick-up of shipment at origin address or to completedelivery of shipment at destination address with normally assigned road haul equipment, due to the structure ofthe building, its inaccessibility by highway, inadequate or unsafe public or private road, overhead obstructions,narrow gates, sharp turns, trees, shrubbery, the deterioration of roadway due to rain, flood, snow or nature of anarticle or articles included in the shipment, the carrier shall hold itself available at point of pick-up or tenderdelivery at destination at the nearest point of approach to the desired location where the road haul equipmentcan be made safely accessible.

Upon request of the shipper, consignee or owner of the goods, the carrier will use or engage smaller equipmentthan its normal road haul equipment or provide extra labor for the purpose, of accomplishing the job ortransferring the shipment between the residence and the nearest point of approach by the carrier's road haulequipment. Charges for this auxiliary service to cover labor and additional vehicle (if used) will be as provided inItem 1, 15 and 30 and shall be in additional to all other transportation or accessorial charges.

If the shipper does not accept the shipment at nearest point of safe approach by carrier's road haul equipmentto the destination address, the carrier may place the shipment, or any part thereof not reasonably possible fordelivery, in storage at the nearest available warehouse of the warehouse of the carrier, or, at the option of thecarrier, in a public warehouse, subject to a lien for all lawful charges. The liability on the part of the carrier willcease when the shipment shall be considered as having been delivered.

Transportation charges to cover the movement of shipment or part thereof from point which it was originallytendered to warehouse location shall be computed on basis of weight of shipment of that part of shipmentstored in warehouse, subject to applicable rate as provided in tariff from point at which it was originallytendered. All accrued charges on the shipment or any part thereof shall be due and payable upon delivery ofsame to the warehouse. Any subsequent movement from warehouse shall constitute a new shipment.

HOISTING OR LOWERING

Hoisting or lowering service will be performed only at points where carrier possesses necessary equipment andexperienced personnel to perform such service. Otherwise, upon request of shipper, consignee, or owner willendeavor to arrange for qualified service, if available, at the expense of the shipper, consignee or owner of thegoods. In such instances, the carrier will not be responsible for damage to shipment or property.

RIGGING SERVICE

When, because of the size of nature of the landing, it is necessary to utilize the services of a rigger, suchservice must be provided by the shipper, consignee or owner of the goods. If requested by shipper, consigneeor owner, carrier will, as agent for them and at their expense, arrange for such rigging service, if available, butcarrier assumes no responsibility for the activities or conduct of third persons, amount of their charges nor thequality or quantity of service furnished.

Rule 21 HOISTING OR LOWERING

Hoisting or lowering service will be performed only at points where carrier possesses necessary equipment andexperienced personnel to perform such service. Otherwise, upon request of shipper, consignee, or owner will

endeavor to arrange for qualified service, if available, at the expense of the shipper, consignee or owner of thegoods. In such instances, the carrier will not be responsible for damage to shipment or property

Rule 22 RIGGING SERVICE

When, because of the size of nature of the landing, it is necessary to utilize the services of a rigger, suchservice must be provided by the shipper, consignee or owner of the goods. If requested by shipper, consigneeor owner, carrier will, as agent for them and at their expense, arrange for such rigging service, if available, butcarrier assumes no responsibility for the activities or conduct of third persons, amount of their charges nor thequality or quantity of service furnished.

Furniture manufactured from press board, particle board and/or engineered wood is designed to go into a boxfrom the manufacturer, to the retailer, and then to the customer unassembled. It is not constructed to withstandthe normal stress of a move as an assembled unit. Most is not designed with the extra wood structural pieces toadequately brace the unit for movement out of or into a residence, and may not withstand the normal truckvibration, even in air-ride trailers. Usually chips or dents are not repairable. Surface impressions can be madeon the furniture when writing on a single piece of paper. Assembly instructions frequently suggest thatconnecting hardware pieces be glued in place. This does not significantly improve the structural integrity of thepieces and makes disassembly impossible without creating substantial, un-repairable damage. When ashipper elects to ship an article as defined herein, and notwithstanding the language contained in thisNotice, in no case shall the liability of the carrier exceed $.30 per pound per article, up to $50.00 percomplete article. Shipper understands that any claim for damage to the press board, particle board,and/or engineered wood furniture may be denied due to inherent vice, based upon the fact that fullyassembled press board, particle board and/or engineered wood furniture is inherently susceptible todamage as outlined above.

HOLIDAYS

Additional fees will be subject for charge (see Item 15). The following holidays will be recognized: New Year'sDay, Easter, Fourth of July, Thanksgiving and Christmas.

FUELSURCHARGE

The fuel surcharge to be charged on shipments 50 (fifty) miles or less are subject to Accessorial Services ChartItem 12 and shipments over 51 (fifty-one) miles are subject to Item 11 on page 11. If the DOE fuel price pergallon exceeds $4.999, the fuel surcharge herein, will be increased by an additional one (1.0%) percent forevery fifteen ($0.15) cents (or fraction thereof), per gallon increase in the price above $4.999 per gallon.

LIMITATION OF LIABILITY ON READY TO ASSEMBLE FURNITURE

MADE FROM PRESS BOARD, PARTICLE BOARD, AND ENGINEERED WOOD

Furniture manufactured from press board, particle board and/or engineered wood is designed to go into a box from the manufacturer, tothe retailer, and then to the customer unassembled. It is not constructed to withstand the normal stress of a move as an assembled unit.

Most is not designed with the extra wood structural pieces to adequately brace the unit for movement out of or into a residence, and may

not withstand the normal truck vibration, even in air-ride trailers. Usually chips or dents are not repairable. Surface impressions can bemade on the furniture when writing on a single piece of paper. Assembly instructions frequently suggest that connecting hardware

pieces be glued in place. This does not significantly improve the structural integrity of the pieces and makes disassembly impossible

without creating substantial, un-repairable damage. When a shipper elects to ship an article as defined herein, and notwithstanding the

language contained in this Notice, in no case shall the liability of the carrier exceed $0.30 (thirty cents) per pound per article or$50.00per article, whichever is greater. Shipper understand that any claim.for damage to the press board, particle board, andor

engineered wood.furniture may be denied due to inherent vice, based upon the fact that.fully assembled press board, particle board,andor engineered wood furniture is inherently susceptible to damage as outlined above.

Storage, Loading/Unloading. Xtreme Moving & Storage does not assume liability for items after they are delivered to public

storage areas. IfXtreme Moving & Storage is providing Loading services for a Shipper's rental truck or other container/vehicle, ourliability will end when items are placed into/onto that vehicle/container. If Xtreme Moving & Storage is providing Unloading Services

for a Shipper's rental truck or other container/vehicle, our liability does not include any damage resulting from improper loading of

vehicle/container or from shifting of items during transport of said vehicle/container. It is the Shipper's responsibility to provide

padding (blanket, bedspreads, thick towels, padded paper, etc.) to protect their goods. Damage Claims must be made within 96 hours of

your move.

Weather. Xtreme Moving & Storage does not assume liability for any damages that may occur due to weather conditions.

Shipper Packing (PBO) Xtreme Moving & Storage does not assume any liability for items packed improperly. All fragile

items should be packed properly prior to the move date. Xtreme Moving & Storage will not disconnect any computer equipment orelectronics, or assume any liability for these items that have not been properly and completely disconnected prior to being moved.

Xtreme Moving & Storage is not liable for the mechanical or electrical condition of any appliance, stereo equipment, TV, piano,

computer, DVD, gaming equipment, clocks, etc.

High Value Items. Xtreme Moving & Storage does not assume any liability for jewelry, money, handguns, or firearms.

Unsafe/Unique Moving Circumstances. Xtreme Moving & Storage is not responsible for any direct or indirect damage to

items or surroundings, as a result of a specific Shipper request. The items (s) are:

Live Plants & Their Containers. Xtreme Moving & Storage will not be liable for any damages (including breakage,

freezing or burning due to weather conditions) to live plants and/or their containers.

Damage Claims must be made within 96 hours ofyour move.

APPLIANCE ACKNOWLEDGEMENT Shipper & crew leader has inspected all appliances (Refrigerators, Washers, &

Dryers) and agree they have been installed properly and not leaking. AM PM TIME

Shipper hereby releases Xtreme Moving & Storage and all of its employees from liability associated with any of the activities

described above. Shipper assumes all liability for any damage resulting from any of the activities described above.
