Presentation Notes
Introduction Agenda:We’re together today until about 4:45, which means that we have about 3 hours and 45 minutes to start down the road of learning and applying motivational interviewing to your dental practice. We’ll start with some lecture based material, but I truly believe MI is best learned by doing, so we’ll attempt to weave in as many activities as possible today. Based on my initial feelings, I think a break around 3 would be a good idea. How does that sound to everyone? Bike Rack Desired Outcomes – I have some ideas about what we can learn today, but I’m really interested in what you all would like to learn. What are you hoping to get out of this training? Group Guidelines Show up. Be Present. If you feel your mind drifting, gently guide yourself back to present. Beginner’s Mind – HUMILITY What guidelines would you like to ask for for the group?

Presentation Notes
A fundamental piece of Clinica’s culture and the context that makes MI work in or organization is our commitment to chronic disease management (and how we leverage the chronic care model in our system design). In your talk, I think this comes in after your description of Clinica’s stats and mission, as this information sets the stage for our focus on quality and how self-management support through MI helps accomplish this.

The crux of the chronic care model piece is that it is designed to support RELATIONSHIPS

What is Motivational Interviewing?

• Motivational interviewing (MI) is a person-centered, guiding method of communication and counseling to elicit and strengthen motivation for change

Page 9: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

What is Motivational Interviewing?

• Or more simply, MI is “dancing,” not “wrestling”

Page 10: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

What is Motivational Interviewing?

• Primary Belief: The capacity and potential for change and adherence is within every person!

• MI increases the possibility for change – it is not the only way, it is not a magic bullet, it does not work with every person.

Thinking about engaging…

• What was her demeanor like during the

encounter? • What happened when she started

downloading her knowledge? • What happened when she kept pressing

her knowledge?

Presenter
Presentation Notes
Debrief questions: What was her demeanor like during the encounter? What happened when she started downloading her knowledge? What happened when she kept pressing her knowledge? Knowledge does not equal engagement, or change

An Activity To Help Understand

Page 15: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Role Play - Patient

• You are a patient with a medical history of diabetes and obesity.

• Today you have a checkup with your PDP. You’ve cancelled several appointments recently, and don’t come to the dentist often.

• You are not interested in making any huge oral health changes, but you have been considering brushing your teeth at least once a day.

• You would like to hear how brushing more could help you avoid cavities.

Page 16: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Role Play - PDP • You are a PDP seeing a patient who has a history

of diabetes and obesity. • Your patient has no showed several times recently

(which is frustrating to you), doesn’t come to the dentist often, and it seems they aren’t very interested in their periodontal disease.

• Your goal is to explain the bad side-effects of diabetes on oral health and come up with a plan to change the patient’s habits.

• You have two minutes to start the conversation, explain how diabetes and periodontal disease are linked, and plan how the patient can get their periodontal disease under control.

Role Play

Page 18: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Core MI Skills

• O – Open Ended Questions • A - Affirmations • R – Reflective Listening Statements • S - Summarizations

Page 19: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Change Talk vs. Sustain Talk

• Change Talk – Any patient speech in favor of making a change

• Sustain Talk – Any patient speech in favor of staying the same

• This balance is important

Page 20: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Why Use MI?

• Because it addresses the biggest part of the pie we can influence!

Page 21: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Why Use MI?

• A related example from our medical colleagues: – Atul Gawande – Reith Lecture Series

Presentation Notes
Notes robust study from Mass General Took patients with Stage IV, incurable lung cancer The patients lived on average just 11 months Half of the patients got usual oncology care and the other half got usual oncology care plus saw a palliative care physician who would discuss with them what their priorities and goals might be for the end of life. Now the group who had that discussion ended up choosing to stop chemotherapy sooner. They were much less likely to go onto the fourth round of chemotherapy, in fact had one third less chemotherapy costs. They had one third fewer days in the hospital. They were much less likely to die in the hospital or in the intensive care unit. They started hospice earlier. They had less suffering at the end of life. And the kicker was they lived 25 per cent longer. If this were a drug, it would be a multi-billion dollar drug and we wouldn’t be asking oh could we afford it, how is this going to be possible? But you know in truth it isn’t even a matter of affording. These are basic skills around having conversations that enabled a win-win situation.

Benefits of MI in a Team Environment

• Consistent Messaging • Cultural Alignment • Decreased Burnout • Increased Health Outcomes → Increased

Access for Patients

Page 24: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Where can I do MI?

• Reflective listening and open-ended questions can help any role create better engagement with a patient

• True MI involves behavior change, and is directional

Page 25: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Why Dental?

• Because it works! • Cochrane meta-analysis called MI “the

all-purpose Swiss Army knife of therapy and treatment”

• SR found MI to be the most effective method for altering oral health behaviors in a clinical setting.

Presenter
Presentation Notes
Lots of systematic reviews and meta-analyses showing that MI works well in management of chronic disease

Challenges to Overcome

• Dentistry is often set up to reinforce the expert/novice paradigm – When patients don’t change, they simply

need more education

• Coding – Oral Hygiene Instruction – Nutritional Counseling

• Very one way

Page 27: Motivational Interviewing - NNOHA · Motivational Interviewing: Training and Sustainment . for the Dental Team . Matthew Allen, DDS and Amber Carlson, MS . Clinica Family Health

Benefits • One prong of a multipronged approach to support the

improvement of Caries at Recall Rate

Presenter
Presentation Notes
As you talk about the prongs here, this is also a good place to consider referencing back to chronic care model, as all of our activities came from thinking about that framework.
