Step1: Enter question name

The question name is for your purposes, to help keep your question bank organized. You are required to put something in this box.

Step 2: Enter question text

Enter your question into the question text box. This is the question or writing prompt that students will see on their quiz.

Step 3: Grade / Penalty Factor

Enter a default question grade. This assigns a point value to this question. Also enter a penalty factor. If you have enabled adaptive mode, the penalty factor shows how much will be subtracted from the question total for each incorrect answer. You are required to enter something in both of these boxes.

Step 4: General feedback

Enter general feedback. This is feedback that will be shown to the student after the question has been attempted, no matter what answer was given.

Step 5: Answer options

Decide how many answer options you want students to be able to select; if you want students to be able to select a and b for the same question, choose multiple answers.

Also decide if you want Moodle to automatically shuffle the possible answers for each new quiz attempt. For this feature to work, you must have “Shuffle within questions” enabled in the quiz settings.In this section you can also decide how you want choices to be numbered.

Step 6: Answer choicesEnter answer choices. You can assign a grade value for each answer choice to give full or partial credit; you must set one answer choice to a grade of 100% to select it as the best possible choice. You can give feedback for each specific response. If you want more answer choices, click on Blanks for 3 more choices. Any choice answer box that you leave empty should not be included in the question.

Step 7: Overall feedback

This feedback will be shown after the question has been attempted. You can tailor the feedback based off the kind of credit the response will be given.

Step 8: Fill in tags

You may fill in tags to help keep your question bank organized. Once completed, click on Next Page.

If it has not been added to the quiz already, your question should now be added to your question bank. It should be visible in the right column. To include this question in your quiz, check the box next to question and click Add to Quiz.
