Chapter 1. Executive Summary
1.1 PROJECT BACKGROUND
The Odisha Works Department (OWD) has a medium to long-term comprehensive programme to
rehabilitate, upgrade and expand the road network throughout the state to improve accessibility, con-
nectivity and enhance the overall development. It is the pre-eminent state level road authority in Od-
isha which deals with issues relating to Feasibility Study and preparation of Detailed Project Report
including the Route Selection, even for relatively low volume roads.
The Government of Odisha has secured a loan from International Bank for Reconstruction and
Development (The World Bank) towards the cost of Widening and Strengthening of existing car-
riageway to 2-lane for Chandbali–Bhadrak–Anandpur Road Project (88 Km).
Chandbali – Bhadrak – Anandpur Road Project is part of 825 km of roads under Phase-I which is
a Section for promoting trade and regional integration between districts by improving transport com-
munication. The project road Chandbali – Bhadrak – Anandpur is a part of the Phase-I road devel-
opment, which was built several years ago with two lane configuration and currently serves moderate
level of traffic. The project involves widening of the existing road which has an average carriageway
width of 5.5 m to 6.8 m and 1.5m. Earthen shoulder on both sides, to a 7.0 m carriageway and
2.5m shoulders on both sides in flat and rolling sections. The expected outcome of the project in-
clude reduced transportation time of goods and better connectivity between districts along with im-
proved safety standards; reduced transit time for import and export goods via port.
1.2 THE PROJECT
The Bhadrak–Pirhat (SH-9) Section from Km. 0/000 to Km. 27/500 is a part of Chandbali–Bhadrak–
Anandpur Road Project (88 Km). The road section Bhadrak–Pirahat starts from Chainage 0/000 Km.
at Bhadrak and ends at Km. 27/500 at Pirahat, in this Bhadrak is district headquarter.
The total length of the project road is 27.500 Km. the road passes completely through plain terrain.
Major stretch of the road is located in the District of Bhadrak. This stretch passes through major hu-
man settlements as Bhadrak, Tihidi & Pirhat.
The location map of the project road is shown below in Figure 1-1;

2.5 ROUTINE MAINTENANCE
As per Contract, the Contractor is required to maintain the portion of the road which has been handed over to him. The maintenance work is being done by the Contractor.
2.6 PROGRAMME OF WORK
The Contractor has given next two month’s individual work programme from July’18 to August’18 for an
amount of Rs.2.17 Cr. & Rs. 2.23 Cr. respectively.

2.10 STATUS OF EXTENSION OF TIME
The stipulated completion date of the project is 15.05.2018, and commencement date of the project is
16.11.2016 .The Project work could not be completed with the stipulated contract period and Contractor
has applied for the EOT for the completion of balance work. The EOT is under review of the Engineer.

Chapter 4. Status of Risk
4.1 HANDING OVER OF SECTION
As stipulated in the Contract, Section-1 & 2 of the road (SH9) has been handed over to the Contractor
on 20.10.2016 & on 10.0.2017 respectively. The Contractor had to examine it and inform the Employ-
er the details of encumbrances at site if any within two weeks of the handing over.
Contractor has informed regarding the encumbrances, which after the scrutiny, Engineer has submit-
ted it to PMU.

4.3 ANY OTHER ISSUE;-
The ‘Contract Agreement was signed on 07th Oct.’2016 for the balance work of Bhadrak-
Pirhat section of SH9. The work programme was prepared and submitted by the Contrac-
tor for the month of Nov.’16 to August’18.
The work planned for June’18 was for Rs. 2.14Cr. While, the achieved work done is Rs. 0.37 Cr. (with a short fall is Rs. 1.77 Cr.). The Contractor could not achieve the planned progress as per work programme given in June’18 mainly due to rains, water logging of borrow areas of soil and moorum, shortage in procurement of materials due to inaccessi-bility of the approach roads to borrow areas, shortage of labour gangs etc. This has af-fected the construction of SG, GSB and DLC layers. The PQC work has also suffered dur-ing the month due to the unseasonal rains.
4.4 RISK INVOLVED
The progress has been slow during the month mainly due to the rains. This will further delay the completion of work.

Construction Vehicles, Equipment and Machinery
The Contractor shall maintain a record of Pollution under control (PUC) certificates for all vehicles at site office:-
PUC certificates of some of the construction vehicles, equipment and machineries have been submitted to Engineer.

Identification, Operation and Rehabilitation of Bor-row Areas
The Contractor had identified soil borrow area no.s 1 & 2 in village Kamaria at Ch. 20+800 (RHS) of SH-09 and submitted documents to Engineer for approval of Borrow Areas At present, the borrow areas are closed and rehabilitation works have also been done.
7 Identification, Operation and Rehabilitation of Stone Quarry
The Contractor has not obtained lease of stone quarry. Aggregate is being procured through private suppliers from stone quarries at Chandikhole.
8 Identification, and Opera-tion of Sand Quarry
The Contractor has not obtained lease of sand quarry. Sand is being procured from third party supplier.

Arrangement for Construc-tion Water
As per the Central Ground Water Authority (CGWA) guidelines, there are two categories of area. (i) Notified area and (ii) Non-notified area. For notified areas, permission/approval is required while for non-notified areas only NOC is required for abstraction of ground water more than 100 m
3/day. Now this location comes under non-notified
area. Therefore permission/approval for extraction may not be re-quired. Also as the extraction quantity is less than 100 m
3/day. There-
fore NOC is also not required as per previous notification. But as per new notification published by CGWA, NOC is required also as the extraction quantity is less than 100 m
3/day. The Contractor has yet to
obtained NOC.
10 Clearing and Grubbing Only ground cover necessary for works is being removed.

Stripping, Stacking and Preservation of Top Soil
Top soil had been stacked and preserved at construction and borrow area site during operation.

Adequate PPEs to be provided to labourers and warning signage to be installed in high risk area:-
PPEs have been provided to labourers and implemented but its im-plementation needs to be improved, warning signages are inadequate in high risk area.
Precautions for electrical cables, wires and equipments:-
The Contractor has taken precautions for electrical cables, wires and equipment at base camp SH-09.
Readily available first aid kits:- The contractor has arranged first aid box at base camp’s office, laboratory and at store room of SH-09.
The Contractor shall take precautions to prevent danger due to mis-hap:- The Contractor has yet to be submitted the Construction Safety Plan for its implementation at site.

5.2 ROAD SAFETY
Though, barricading with bamboos and sand bags with reflective sticker and caution tape at site is
being done, but hard barricading and more sign boards are required in construction zones in con-
formance with codes & specifications.
Helmets, safety jackets, safety shoes, gumboot, nose mask and goggles etc. have been provided to
the workers but, these are required to be used regularly and adequately during construction activities.

A routine review meeting was held on 28.06.2018 by Resident Engineer at base Camp of the
project with the Managing Partner and Project Manager of Contractor.
On 30.06.2018 a review meeting was taken by the Hon’ble EIC-CUM-Secretary. CE (DPI &
Roads), SE (R&B) Works Department, EE (PMU), TL and Resident Engineer (CSC) & Contrac-
