Minutes of the Monthly Meeting of the Mobile Medical Unit Advisory Council (MMUAC)

September 6, 2016 | 3:00 pm Location of Meeting: Juvenile Welfare Board 14155 58th Street North Clearwater, FL 33760 Present at Meeting: Sgt. Zachary Haisch (phone), Jerry Wennlund, Nicole Fischer*, Valerie Leonard*, Sean Murphy*, and Mark Dufva. Staff and community members present: Lourdes Benedict, Drew Wagner, Daisy Rodriguez, Rhonda O’Brien., Dale Williams, Elisa DeGregorio, Melissa VanBruggen (phone), and Preston* (guest of Nicole Fischer). (*Consumer) The regular meeting of the Mobile Medical Unit Advisory Committee (MMUAC) was called to order at 3:07 pm.

i. Chairman’s Report

i. New Member Appointment: No new members.

ii. Approval of Previous Minutes: A motion to approve the minutes dated August 2, 2016 was made by Mark Dufva and seconded by Nicole Fischer. The Council unanimously approved the minutes.

iii. Credentialing & Privileging: The Medical Executive Committee did not meet last month. No additional providers recommended this month.

iv. Unfinished Business: In follow-up to last month’s meeting, the following items were discussed:

a. UPass Implementation: Ms. Lourdes Benedict, Director of Human Services, provided an update on the UPass program. From May 31 through August 15, 3,337 UPass cards were distributed. Ms. Benedict reported that PSTA has begun rethinking the program and has contacted the County to begin discussions on changes to the program. Ms. Benedict acknowledges that we know that the program is important to clients and we are continuing to negotiate with PSTA. In the interim, effective September 5, 2016, clients were no longer able to use their “blue card” as a bus pass. Current UPass clients were being instructed to go to one of four PSTA Customer Service Terminals to obtain a paper bus pass. A copy of the letter sent to clients was included in the packet for the Council. New clients must attend a medical appointment prior to being provided with the opportunity to obtain a 10-day bus pass. A letter from the physician must be presented along with the blue card to PSTA. Nicole Fischer and Sean Murphy reported that other clients were “not on the list” at PSTA which did not allow them to obtain the bus pass. Ms. Benedict responded that the County did encounter some glitches with providing an accurate list to PSTA and is working to correct the error. In the meantime, please contact Human Services immediately to have it corrected.

Ms. Benedict also asked if any of the consumers had heard if any PSTA bus drivers were taking the clients blue card? She asked them to please inform other consumers that PSTA can not take a clients blue card. Ms. Benedict reiterated that we do not know what will happen with the program and when we have new information, we will keep the Council and the clients informed.

I. Governance/Operations

i. Strategic Planning: Ms. Rodriguez updated the Council that the revised brochure was at the printer and that we would begin distribution at homeless shelters and hospitals as soon as they were received. Ms. Rodriguez also reported that as part of improving our communications, we began a monthly e-newsletter for providers in the community who work with the homeless population in order to provide them with program updates, announcements, or upcoming events. The first email was sent in September to approximately 100 individuals. A copy of the email was included in the meeting packet. In addition, a resource list of community service providers was updated and posted to the website.

The staff continue to meet to review progress towards the communications, access and quality recommendations from the focus groups.

ii. MMU Calendar: The September calendar (included in the meeting packet) for the MMU and Bayside was presented to the Council by Andrew Wagner. Mr. Wagner noted that the clinic was closed on September 1st and 2nd due to Hurricane Hermine. On September 12th the staff will be in training between 12-5 pm; on September 17th there will be no services; and on Sept 16th they will only have a nurse in the afternoon due to a Physician’s meeting at the DOH.

iii. Patient Satisfaction Surveys: The results from the August 2016 patient satisfaction survey were late being entered into the system. Results from August and September will be included in next month’s meeting packet.

II. Fiscal

i. Notice of Awards: Ms. DeGregorio reported that the County received one new Notice of Award for a SAMHSA Grant for the Cooperative Agreement to Benefit Homeless Individuals. The new grant award is for a total of $800,000 per year for three years. The County partnered with WestCare Gulf Coast, Directions for Living, and Operation PAR along with several homeless housing providers in the County. A motion to accept the Notice of Award was made by Valerie Leonard and seconded by Mark Dufva. The motion passed unanimously.

III. Clinical

i. Trend Report for Patient Counts/Encounters: A copy of the August trend report was provided to the Council members indicating that a total of 1657 unduplicated patients were recorded from January 1st through August 31st for the program. The numbers are equal to the same time period last year but has been picking up. Mr. Wagner noted that based on our current projections and pace, we anticipate seeing 2,500 patients for the year, an increase over the 2,239 patients in 2015.

HEP – CHANGE IN LOCATION/TIME REQUEST: Mr. Wagner proposed to the Council that after several months of working with HEP leadership, we feel that the numbers for the every other Tuesday pilot program were still too low. He recommends keeping the van at HEP on Monday afternoons and changing the every other Tuesday location to St. Vincent de Paul in St. Petersburg starting October 1st. A motion was made by Mark Dufva to accept Mr. Wagner’s recommendation. It was seconded by Jerry Wennelund and it passed unanimously.

Dental: A copy of the dental trend report was provided to the Council as requested showing that since Bayside has opened on April 18th through August 31th, the clinic saw 255 unduplicated clients in 562 encounters. Overall, all HCH clients receiving dental care totaled 337 clients in 970 encounters at all dental locations in the County.

Phone: Mr. Wagner also presented the August call volume report for the Bayside Clinic/HCH program. In the month of August, the health center received 3038 phone calls broken down as follows:

• 142 ..... Pressed 1 for Office Hours/Locations • 968 ...... Pressed 2 for Medical Appointments/Cancellations • 298 ...... Pressed 3 for Dental Appointments/Cancellations • 48 ........ Pressed 4 for Behavioral Health • 66 ........ Pressed 5 for Specialty Care • 915 ...... Pressed 6 for All Other Questions • 270 ...... Pressed 0 for the Operator • 331 ...... Timed Out/Error

This represents a 1000 call increase over the previous month. Sean and Valerie reported that clients are having trouble getting through to a person regardless of the # pressed. The staff committed to meeting with BTS to identify any other options that can be employed to reduce client wait time.

IV. Other Updates

i. New Business: Hurricane Hermine: Pinellas County felt the long lasting rain effects of Hurricane Hermine over from approximately August 31st through September 2nd. County offices, including Bayside Clinic, were closed on September 1st and the clinic remained closed on September 2nd due to the FL Dept of Health closure ordered by the State. The Van left Pinellas Hope on Wed., Aug 31st at 2:45 pm due to flooding in the roadways. Mr. Wagner added that he left several boxes of bug repellant wipes at Pinellas Hope for its residents use. Pinellas Hope reported that they lost 63 tents used for resident shelters and are being replaced. Mr. Dufva indicated that they are working with the Southwest Florida Water Management District on a drainage Master Plan for the community that will hopefully reduce flooding into the long-term future. He noted that the road leading to the back apartments cannot be paved due to SWFWMD regulation. In the meantime, when tropical weather looms, they encourage residents to find a temporary shelter with family or friends, or when available, to utilize designated emergency shelters opened by the County in Hurricane conditions.

Ms. Rodriguez also relayed to the Council that our Project Officer from HRSA called to check in on the health center following the Hurricane to ensure that everything was ok… Prescriptions Following ER Discharge: Ms. Leonard noted that some patients were complaining of prescriptions being called in that were not being delivered. Mr. Wagner reminded clients that after the 3 day fill from the hospital, the clients must come to the clinic for an appointment to follow-up on the hospitalization and have the remaining prescription filled. Prescription delivery rules vary per shelter, so please work with your provider and shelter and be present at the scheduled delivery time. Not all shelters can hold prescription drugs on the resident’s behalf.

The meeting was adjourned at 4:54 pm. The next meeting will be held at 3:00 p.m. on Tuesday, October 4, 2016 at Juvenile Welfare Board.

This monthly publication will deliver the monthly calendar, announcements and upcoming events for the Health Care for the Homeless program. The Pinellas County Human Services Department and our partners at the Florida Department of Health are committed to providing quality care for our County’s homeless residents. Monthly Calendar The October Calendar for the Mobile Medical Unit and Bayside Health Clinic. Please note: BOTH the Mobile Medical Unit and Bayside Health Clinic will be CLOSED on October 13, 2016 for In‐Service Training by the Florida Department of Health. Did you know? The new Bayside Health Clinic, which opened on April 18, 2016, is open until 8:00 pm Monday through Thursday and on Saturdays from 8:00 am – 12:00 noon. Take advantage of these extended hours…walk‐in appointments available…visit us at 14808 49th Street, Clearwater, FL 33762.

Announcements Mobile Medical Van Schedule Change – Effective October 1, 2016

The Mobile Medical Van will be moving from Homeless Empowerment Program (HEP) in Clearwater every other Tuesday to St. Vincent de Paul St. Petersburg EVERY Tuesday. The van will continue service at HEP every Monday afternoon from 1:30 pm – 5:00 pm. This change is effective October 1, 2016.

Phone System Enhancements Client Feedback is Being Addressed: As the new Bayside Health Clinic came online, we began routing all calls to the clinic’s new phone system rather than to individual cell phones. As our clients became aware of the new single phone number, the volume increased significantly from month to month and caused long wait times to get through. The County has and continues to make adjustments to shorten waiting time and allow callers to leave targeted voicemails. We are continuing to track our call volume to make improvements, so let us know if you are still hearing concerns from clients about getting through to the clinic.

Homeless individuals are less likely than the general population to seek dental care, and are the least likely of all vulnerable populations to carry a toothbrush, partially due to lack of access to facilities where they can attend to personal hygiene. They have limited financial resources, often have no health insurance, and are unhoused or housed in unstable situations. The Tampa Bay Oral Health Coalition offers a web‐based community based dental providers listing who serve those clients most in need. Please share this dental community resource list with your clients seeking dental care and information.

News Article: St. Petersburg Free Clinic Providing Dentures to Uninsured Adults, (WUSF, July 10, 2016)

Homeless Empowerment Program Dental & Wellness Clinic announced the receipt of a $191,000 grant from the Florida Assoc. of Free and Charitable Clinics! The grant will cover many costs for the clinic, including the purchase of a new state‐of‐the‐art dental chair! Last year, the HEP dental program provided more than $600,000 worth of free dental procedures to HEP residents!

New Health Care for the Homeless Brochure Available (See Attached)

The County has updated its Health Care for the Homeless brochure to include the new Bayside Health Clinic. Printed copies are being distributed to each of the homeless shelters and local hospitals. If you would like to have printed copies, please email Daisy Rodriguez to submit your request. A PDF version for in‐house printing is attached to this email.

Substance Abuse – Medication Assisted Treatment Program Now Available The Health Resources and Services Administration (HRSA), awarded Pinellas County with the Medication Assisted Treatment grant that will 1) increase the number of patients screened for substance use disorders and connected to treatment, 2) increase the number of patients with access to MAT for opioid use and other substance use disorder treatment, and 3) provide training and educational resources to help health professionals make informed prescribing decisions. If you know a homeless individual seeking to address their substance abuse problems, contact the health center for more information – call 727‐464‐7866

Mental Health Services – Urgent Care Available for Clients Same Day Pinellas County has recognized that behavioral health continues to be a pervasive concern within our communities, often affecting the physical health and safety of our most vulnerable citizens. A critical lack of funding has impacted the availability and integration of services and information sharing, inhibiting stability for many individuals. The result is a greater demand for deep‐end, expensive crisis care and repeated entry into the criminal justice system and emergency rooms. A new grant award from AHCA has funded 4 licensed mental

health counselors that will be located on‐site at Bayside Health Clinic and with our Mobile Medical Van. The program is designed to meet an immediate need for behavioral health/substance abuse stabilization and intervention. The additional behavioral health staff will be available for assessment and treatment for mental illness and/or substance abuse. The role of these specialists will be both to intervene during mental health and substance abuse crises, stabilize, and refer to treatment, and, where indicated, engage in longer term treatment as appropriate. We are pleased to be partnering with BayCare Behavioral Health, Directions for Living and the Florida Department of Health in Pinellas County on this program.

Grant Specific Term(s)

1.

Health centers will be required to provide information on the QI activities supported through this one­time supplement via their FY 2017

Service Area Competition (SAC) application or FY 2017 Budget Period Renewal (BPR) progress report. More information will be provided

as part of the SAC and BPR instructions.

2.

This supplement must be used within 12 months of receipt of funds to support QI activities. Funds must be used consistent with all federal

cost principles as noted in 45 CFR 75. In addition, health centers must use these funds for QI activities, which include but are not limited to:

• Developing and improving health center QI systems and infrastructure, including training staff; developing policies and procedures;

enhancing health information technology, certified electronic health record, and data systems; data analysis; and/or implementing targeted

QI activities (including hiring consultants).

• Developing and improving care delivery systems, including purchasing supplies to support care coordination, case management, and

medication management; developing and implementing contracts and formal agreements with other providers; laboratory reporting and

tracking; training and workflow redesign to support team­based care; clinical integration of behavioral health, oral health, HIV care, and other

services; and/or patient engagement activities.

This award provides one­time funding for use during the period of September 1, 2016, through August 31, 2017. If funds are not fully

expended by the end of your current budget period, you must request carryover to use the remaining funds in your next budget period. In

order to use this funding in the upcoming budget period, the appropriate amount must be shown as un­obligated (UOB) on line 10.h of the

Annual Federal Financial Report (FFR), SF­425. In addition, a Prior Approval Request to carry over these funds must be submitted through

EHB immediately following the FFR submission. Please consult the Grants Management Specialist for questions regarding submission of

the FFR and/or Prior Approval Requests to carry over UOB funds.

Funds may not be used for fundraising, lobbying, incentives (gift cards, food), construction/renovation, facility or land purchases, or vehicle

purchases. Pursuant to existing law and consistent with Executive Order 13535 (75 FR 15599), health centers are prohibited from using

federal funds to provide abortion services (except in cases of rape or incest, or when the life of the woman would be endangered). This is

consistent with past practice and long­standing requirements applicable to grant awards to health centers. In addition, this supplemental

funding may not be used: to supplant existing resources; to support bonuses or other staff incentives; for moveable equipment individually

valued at $5,000 or greater (except equipment related to Health Information Technology and certified Electronic Health Record systems).

5.

The purposes of the Fiscal Year (FY) 2016 Health Center Quality Improvement (QI) Fund one­time grant supplement are to: (1) recognize

health centers that displayed high levels of quality performance in Calendar Year 2015 Uniform Data System reporting and/or significantly

improved quality of care from 2014 to 2015; (2) provide support for those health centers to continue to strengthen quality improvement

activities; and (3) to recognize and provide support for health centers with one or more sites with new and/or continued patient centered

medical home recognition.

All prior terms and conditions remain in effect unless specifically removed.

This award provides one­time supplemental funding for Fiscal Year (FY) 2016 Delivery System Health Information Investment (DSHII) supplemental funding activities for the

period September 1, 2016 through August 31, 2017. As such, a portion of these funds are being provided for use in the health center’s upcoming FY 2017 budget period. In

order to use this funding in the upcoming budget period, the appropriate amount must be shown as un­obligated (UOB) on line 10.h of the Annual Federal Financial Report

proposed use of DSHII funding aligns with the intent of the DSHII supplemental funding opportunity (HRSA­16­191) and complies with requirements in the Uniform

Administrative Requirements, Cost Principles, and Audit Requirements for HHS Awards at: http://www.ecfr.gov/cgi­bin/text­idx?node=pt45.1.75. Note that health centers must

implement allowable health information technology enhancements by the end of the 12­month funding period, even if the proposed activities change post award and/or if

implementation delays arise.

Health centers may not use Fiscal Year (FY) 2016 Delivery System Health Information Investment (DSHII) supplemental funding to supplant existing resources (federal, state,

local, or private). In addition, these funds may not be used on: costs incurred prior to award, patient care costs, direct hire personnel costs (i.e., salary and fringe benefits),

purchase or upgrade of an EHR that is not certified by the Office of the National Coordinator for Health Information Technology (ONC), construction (including minor alterations

and renovation), fixed/installed equipment, costs associated with the installation of equipment purchased with one­time DSHII funding, or facility, land, or vehicle purchases.

In the Approved Budget section of the Fiscal Year (FY) 2016 Delivery System Health Information Investment (DSHII) supplemental funding Notice of Award (item 11), all approved

costs are included in the ‘Other’ budget line item (line i). Health centers must expend DSHII supplemental funding in accordance with the distribution across budget line items

(otherwise referred to as object class categories) proposed in the approved application and/or any revisions submitted to HRSA prior to this award. (e.g., Equipment, Supplies,

Your organization will be required to report your progress on and the impact of the health information technology enhancements supported by the one­time DSHII award in future
