MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement made and entered into in the

__________________ this _____ day of ____________, 2013 by and between:

POLYTECHNIC UNIVERSITY OF THE PHILIPPINES, a

State University with principal address at Anonas Street,

Sta. Mesa, Manila, represented herein by its President,

DR. EMANUEL C. DE GUZMAN, and hereinafter

referred to as the UNIVERSITY.

-and-

DEPARTMENT OF EDUCATION, an instrumentality of

the Philippine government and duly represented by the

School Division Superintendent of Bulacan, DR. ROMEO

M. ALIP, CESO V and the Cooperating School

Principal with address at City of Malolos, Bulacan,

herein referred to as the DEPARTMENT.

WITNESSETH:

WHEREAS, as part of the mission and vision of the University to

undertake manpower development through education and training, the

UNIVERSITY, and the DEPARMENT desire to promote and achieve sustainable

quality basic education and to insure academic excellent through pre-service

education for teachers;

WHEREAS, the UNIVERSITY and the DEPARTMENT, in its commitment

to improve the quality of basic education in the Division of Bulacan as a major

key in reaching its goals, support the pre-training program that will provide

student-teachers opportunities to, to wit:

1. Develop the competencies set in the National Competency-Based

Teacher Standards (NCBTS);

2. Apply their knowledge and learning in actual classroom situation;

3. Gain comprehensive and realistic perspective of the teaching-learning

situations and day-to-day school activities; and4. Reflect on their qualities and commitment vis--vis the demands of the

teaching profession.

WHEREAS, the University and the Department believe that aside from the

usual practice teaching, intensive field study is necessary to adequately prepare

the student teacher for their functions and responsibilities.

NOW, THEREFORE, the UNIVERSITY and the DEPARTMENT, inconsideration of the mutual covenants and agreements made herein, by these

presents do enter into a Memorandum of Agreement on the following terms and

conditions, to wit: foregoing premises, the parties hereto have agreed as follows:

A. The University shall:

1. Coordinate with the School Division Superintendent and the School

Head on the deployment of the Student Teachers (ST for brevity);

2. Conduct regular supervisory visits during the stay of the ST in the

designated school;

3. Gather feedback from cooperating schools on the STs

activities/performance for the improvement of the program; and

4. Conduct exit conference with the cooperating school head.

B.The Department shall:

1. Conduct orientation/briefing in the school learning environment and the

community support system;

2. Allow ST to observe classes and school activities as required for the

Second Semester, SY 2012-2013;

3. Allow ST to complete Three Hundred Forty (340) hours of practice

teaching for the First Semester, SY 2013-2014;

4. Provide ST the best resource teachers/mentors;

5. Allow the University to supervise the activities of their ST in

coordination with Department Heads/Grade Level Chairs/Schools

Heads;

6. Allow ST access to school information for research and instructional

purposes;

7. Monitor and evaluate the performance of ST; and

8. Submit consolidated evaluation reports on ST.

This Memorandum of Agreement shall remain in force and effect for the

First Semester, Academic Year 2013-2014 unless sooner terminated by mutual

consent of the parties.
