10. Licensed material may be used at 235 Pearlridge Center, Phase 1, 98-1005 Moanaluai Road, Aiea, Hawaii, and at temporary job sites of the licensee anywhere in the4 United States where the U. S. Nuclear Regulatory Commission maintains jurisdiction

for regulating the use of licensed material,

h11. Licensed material shall be used by, or under the supervision and in the physicalgi presence of, Ronald L. Soroos, George T'. S. How, Samuel P. Rombaoa, Christian P.(| Muller, Masayoshi Ogata, or. individuals who have.successfully completed the0 manufacturer's training program for gauge users and who have been designated by the

fN Radiation Protection Officer, Masayoshi Ogata. The licensee shall maintain recordsf,
