8 Signed copies of the individual and/or consolidated seafarers’ agreements and/or contacts of employment
9 Collective Bargaining Agreement(s), if applicable
10 Record of employment (discharge books)
11 Monthly wage payment records (payrolls), showing any exchange rates used and the transmission of wage payments to seafarers’ accounts
12 Overtime records, if applicable
13 Shipboard working arrangement (Deck & Engine)
14 The record of hours of rest and work for all ship’s staff
15 A record of working hours and tasks assigned to young seafarers (seafarer between 16 to 18 years old).
16 Musters and fire drills, to ensure no disturbance of rest periods
17 A record of the Master’s regular inspections of the accommodation and work areas
18 A ship’s general arrangement showing accommodation, recreational facilities, hospital facilities and food preparation areas.
19 A record of frequent inspections of: 1) supplies of food & drinking water, 2) all spaces & equipment used for the storage and handling of food and drinking water; and 3) galley & other equipment for the preparation and service of meals.

21 Medical report forms (Master/shore medical)
22 Medical Log book
23 Records of regular inspections of the medical equipment and facilities
24 Health & Safety policy / procedures - Person designated to be responsible for OSH
25 Evidence of any on board occupational health and safety awareness training programmes
26 Records of the regular on board assessments of noise and vibration levels
27 Accidents / Incidents reports and investigations
28 Safety meeting records, proof of any risk assessments made, records of any corrective action taken and statistics relating to accidents, injuries etc.
29 Shore leave records
30 Proof of how the ship owners meets his liability obligations as per the requirements of the Convention
31 International list of doctors and medical care facilities available worldwide for emergency medical care to seafarers
32 The ship’s on board seafarers’ complaints procedures including a record of the complaints received and how they were addressed (complaint log)
