RELEASE AND HOLD HARMLESS AGREEMENT In consideration for participating in club activity sponsored or organized by the Port Malabar Rifle and Pistol Club, Inc., the undersigned Participant HEREBY AGREES TO WAIVE, RELEASE, DISCHARGE, AND HOLD HARMLESS the Port Malabar Rifle and Pistol Club, Inc. from any and all claims for damages for death, personal injury, or property damage which they may have or which they may hereafter accrue as a result of participation in any Port Malabar Rifle and Pistol Club, Inc. program or related activity and events including, but not limited to unorganized practice, organized practice, training and instruction and organized shooting events.

By signing this Release and Hold Harmless Agreement, the undersigned recognizes that the sport of rifle, pistol, and shotgun shooting or related activities present certain inherent dangers that cannot be eliminated even by the exercise of reasonable care. As such, the undersigned Participant hereby expressly assumes the risk that is inherent to the sport of rifle, pistol, and shotgun shooting.

As a requisite condition to the Participant being allowed to attend and participate in any Port Malabar Rifle and Pistol Club, Inc. sponsored event or trip, the undersigned Participant hereby acknowledges that the State of F lorida has the most significant relationship with the parties to this Agreement rather then the destination state or providence. As such, the parties hereto expressly agree to F lorida as the Choice of Law and Choice of Forum to any claim, suit, or controversy that may arise by and between the p arties or any of its related affiliates, and further agree that any action, suit, or claim made by the Participant must be brought in the State of Florida as the proper Venue having exclusive subj ect matter jurisdiction to -any such claim, demand, or suit. To effectuate this expressed Choice of Law, Choice of Forum and V enue provision, the undersigned hereby agrees that the state or providence where the injury or cause of action arose shall not have either subject matter jurisdiction or in personam jurisdiction over the parties hereto .

By signing this Release and Hold Harmless Agreement, the undersigned Participant assumes the risk inherent to rifle, pistol, and shotgun shooting, and thereby consents to the Participant's p articipation in Port Malabar Rifle and Pistol Club, Inc. programs and related activities, and further acknowledge that he/she understands that all risks, whether known or unknown, are expressly assumed by the undersigned, and that all claims, whether known or unknown, are expressly waived in advance.

In addition to the foregoing, the undersigned Participant hereby expressly agrees and recognizes that the Port Malabar Rifle and Pistol Club, Inc., through its individual Match Directors, Match Chairmen, Range Officers, judges and referees, officers, directors, agents, individuals or entities perform volunteer services, and as such, said entities and/or individuals are immune from liability as volunteer leaders, assistants, and organizers.

Arbitration: In further consideration for being allowed to participate in any activities sponsored by the Port Malabar Rifle and Pistol

Club, Inc. the undersigned Participant recognizes and agrees that this contract involves interstate commerce which is necessarily controlled by the Federal Arbitration Act (9 USC § 2), and therefore, pursuant to the Federal "Arbitration Act (FAA) as well as the Florida Arbitration Act (Florida Statutes § 682.02) hereby agree to submit to binding arbitration any and all claims, demands, suits, or other disputes that may arise out of this contract, or that may be presented by or on

Friday/1 1 -23-1 2/2000 hours : Detective Cayenne received a phone call from Sergeant Paul advising our team was being called to 8251 Southside Boulevard (Gate Gas Station) in reference to a shooting in the parking lot. Sergeant Pau l advised that when we are done with our other crime scene we could meet at the Gate Gas Station.

Note: Let i t be noted that Detectives Stapp and Cayenne was handling another death investigation and never made it to the crime scene.

F riday/1 1 -23-1 2/231 0 hours: Detective Cayenne spoke with Sergeant Paul and advised him that Detectives Cayenne and Stapp were done with our death investigation where did he need us to respond. Sergeant Paul advised Detective Cayenne to respond to the Homicide Office.

Friday/1 1 -23-1 2/230 hours: Detective Cayenne arrived at the Homicide Office.

Tuesday/1 1 -27 -1 2/1 805 hours : Detectives Cayenne and Stapp responded to the strip mall located at 8221 Southside Boulevard to conduct a canvass of the businesses.

Detective Cayenne spoke with owners or employees of the fol lowing businesses:

U n it 3- Kaluby's Dance Club advised they had video survei l lance i nside the business and outside on the back side. There was no video outside the front of the business.

U n it 5 - The Loop Restaurant advised the business has surveillance inside and views outside the rear of the business. There was no video outside the front of the business.

U n it 1 1 - Tropical Smoothie advised the business has surveil lance inside and views outside the rear of the business. There was no video outside the front of the business.

Detective Cayenne spoke with employee who stated he was working on the n ight of the shooting. He advised he d id not hear any gunshots. remembered a police car had pul led over a veh icle in the Publix parking lot. The next th ing he remembered were multiple police cars responding to the scene. He did not learn of the shooting until later that night.

It does when it is the one using said corporations to spy on the citizens. Cable companies are

also not entirely private entities. They are highly subsidized by big government. If they were

truly private entities many non..;spying ones would pop up as soon all the major ones started

spying. Every home would have a choice of 2 or more cable providers but that isn't the case for most people.

I don't eat GMO's but would agree the government has no right to make companies label GMO products . Free Market works just fine because non-GMO foods make sure it is written al over. As a consumer you're left with many choices.

Cable companies are completely different. It's about the control of information. If cable companies tomorrow could tell you that you either put in a camera or they stop providing you
